38 Possible Causes for Abdominal Distension, Bicytopenia, Hepatosplenomegaly

  • Gaucher Disease

Headache, diarrhea, abdominal distension, and abdominal pain were also reported as related events in 3, 4, 1, and 1 placebo‐treated patients, respectively, during the 9‐month[ncbi.nlm.nih.gov] We report here the case of a young woman with hepatosplenomegaly, leukopenia, and thrombocytopenia.[ncbi.nlm.nih.gov] [ncbi.nlm.nih.gov] Less common signs of the disease are hepatosplenomegaly, ichthyosis, arthrogryposis and facial dysmorphy.[symptoma.com]

  • Mantle Cell Lymphoma

A 58-year-old man was admitted to our hospital with epigastric pain, abdominal distension, nausea, and melena.[ncbi.nlm.nih.gov] Most patients present with advanced stage disease with lymphadenopathy, hepatosplenomegaly, and bone marrow involvement.[icd9data.com] Further investigation revealed bicytopenia with splenomegaly and elevated serum creatinine.[ncbi.nlm.nih.gov]

  • Primary Myelofibrosis

A 53-year-old man with a long history of primary myelofibrosis, presented with abdominal distension and inguinal mass.[ncbi.nlm.nih.gov] The main clinical manifestations of PMF are anemia, bleeding, hepatosplenomegaly, fatigue, and fever.[ncbi.nlm.nih.gov] Patients with primary myelofibrosis may have pancytopenia or bicytopenia and in that respect mimic patients with oligoblastic leukemia (myelodysplasia [MDS]; see Chap. 88)[patient-help.com]

  • Cytomegalovirus Infection

., 6-weeks-old, born small for gestational age, was admitted in our Hospital for gastrointestinal signs: diarrhea, abdominal distension, observed three days earlier.[ncbi.nlm.nih.gov] […] should arouse suspicion of intrauterine CMV infection can be distinguished into CNS signs such as ventriculomegaly or microcephaly and extracerebral infection signs such as hepatosplenomegaly[ncbi.nlm.nih.gov] Laboratory investigations showed bicytopenia from complete blood count. Chest radiography and high-resolution computed tomography of the chest revealed lung nodules.[symbiosisonlinepublishing.com]

  • Chronic Myeloid Leukemia

Approximately 5 months after the initiation of nilotinib for CML, he developed upper abdominal distension with intermitting abdominal pain, and based on abdominal computed[ncbi.nlm.nih.gov] Due to presenting complaint of bleeding diathesis and absence of hepatosplenomegaly, the case was undiagnosed for CML until the patient reported to us.[ncbi.nlm.nih.gov] In contrast to patients with RA and only dyserythropoiesis, patients with multilineage dysplasia have bicytopenia or pancytopenia, a higher incidence of cytogenetic abnormalities[doi.org]

  • Chronic Myelomonocytic Leukemia

Affected individuals present with monocytosis, cytopenias, and extramedullary manifestations like lymphadenopathy and hepatosplenomegaly.[symptoma.com] In contrast to patients with RA and only dyserythropoiesis, patients with multilineage dysplasia have bicytopenia or pancytopenia, a higher incidence of cytogenetic abnormalities[doi.org] Hepatosplenomegaly (in most cases).[cancer.gov]

  • Familial Hemophagocytic Lymphohistiocytosis

Herein, we report a case of a 14-month-old Chinese boy who presented with fever, abdominal distension and thrombopenia, and died within 3 days of admission to the hospital[ncbi.nlm.nih.gov] Reported here is the case of a 12-year-old boy, from a poor family, with a 6-year history of visual loss and fever for 5 months, and transient hemiparesis with hepatosplenomegaly[ncbi.nlm.nih.gov] Hemogram revealed bicytopenia and bone marrow aspirate showed hemophagocytosis.[link.springer.com]

  • Hepatosplenic Lymphoma

Case presentation A 13-year-old Indian boy presented to the hematology out-patient department with a history of progressive abdominal distension of 9 months’ duration and[jmedicalcasereports.biomedcentral.com] Computed tomography scans and sonography solely displayed a marginal hepatosplenomegaly.[annonc.oxfordjournals.org] There was bicytopenia (hemoglobin - 101 g/L, platelet count - 38 10 9 /L, and total leukocyte count - 5 10 9 /L) with only a few blasts in peripheral blood film [Figure 1][ijpmonline.org]

  • Castleman Disease

The patient returned 5 years later with abdominal distension, weight loss, cervical and inguinal lymphadenopathy, hepatoslenomegaly and vescicobullous skin rashes for 3 months[jcp.bmj.com] For multicentric disease: multi-region CT bilateral hilar and mediastinal lymphadenopathy centrilobular nodules diffuse abdominal lymphadenopathy hepatosplenomegaly ascites[radiopaedia.org] Investigations showed bicytopenia, nephrotic range proteinuria with hypoalbuminemia, hypogammaglobulinemia, and features of hyaline-vascular type Castleman disease in a lymph[ncbi.nlm.nih.gov]

  • Disseminated Histoplasmosis

Six months later, she had abdominal distension and received prednisone, hydroxychloroquine and azathioprine.[ncbi.nlm.nih.gov] Histoplasmosis is an uncommon cause of hepatosplenomegaly in South Africa.[ncbi.nlm.nih.gov] On further investigations for bicytopenia, histoplasmosis had been diagnosed on bone marrow trephine biopsy.[ncbi.nlm.nih.gov]
