There are two types of stroke, hemorrhagic and ischemic.[web.archive.org] The common chameleons were initially diagnosed as altered mental status (AMS) (29, 31%), syncope (15, 16%), hypertensive emergency (12, 13%), systemic infection (10, 11%),[ncbi.nlm.nih.gov] Paramedics bring a 26-year-old man to the ED with acute-onset altered mental status. He has a history of back pain. An open, unlabeled pillbox was found.[emedicine.medscape.com] We evaluated the relevant literature to analyze the causes of cerebral hemorrhage in similar patients.[ncbi.nlm.nih.gov]

  • Epidural Hematoma

One patient was managed primarily with NPDT for uncontrollable intraoperative hemorrhage during posterior cervical laminectomy.[ncbi.nlm.nih.gov] The group failing observation was more likely to present with altered mental status (RR 18.8; 95% CI 8.7-49.6), has larger median bleed thickness (observed 5.6mm versus failed[ncbi.nlm.nih.gov] Signs of increased ICP include altered mental status, seizures, pupillary changes, and vomiting.[clinicaladvisor.com]

  • Hypertensive Encephalopathy

Our primary outcome was the composite of subsequent ischemic stroke or intracerebral hemorrhage.[ncbi.nlm.nih.gov] His altered mental status, however, resolved with control of blood pressure.[ncbi.nlm.nih.gov] Clinical manifestations include headache; nausea; vomiting; seizures; altered mental status (in some cases progressing to coma); papilledema; and retinal hemorrhage ICD-9-[icd9data.com]
