21 Possible Causes for Bicytopenia, Bleeding Diathesis

  • Gaucher Disease

Clinical manifestations include hepatosplenomegaly, thrombocytopenia, bone disease and a bleeding diathesis, frequently resulting in presentation to haematologists.[]

  • Severe Aplastic Anemia

Patients suffering from anemia, thrombocytopenia and a bleeding diathesis should receive transfusions of the respective blood products.[] In our case, the patient presented with bleeding due to thrombocytopenia.[] Dengue infection further worsens the disease process in patients with aplastic anaemia due to uncontrolled hemorrhagic diathesis and major organ failure, which may prove fatal[]

  • Disseminated Intravascular Coagulation

Bleeding diathesis must be corrected before surgery in order to prevent massive intraoperative bleeding.[]

  • Paroxysmal Nocturnal Hemoglobinuria

Progression from bicytopenia to pancytopenia or a new episode of bicytopenia or pancytopenia after recovery was not considered as a first progression to bicytopenia or pancytopenia[] The indications included pancytopenia (all three cell lines), bicytopenia (two cell lines), and unicytopenia (one cell line).[] The cumulative incidence of bicytopenia or pancytopenia ( Figure 2 B) was similar in patients with classic and intermediate disease ( P .63).[]

  • Promyelocytic Leukemia

The patient was treated with all-trans retinoic acid under a diagnosis of APL with improvement of the bicytopenia.[] Nevertheless, APL remains associated with a significant incidence of early death related to the characteristic bleeding diathesis.[] These patients need close monitoring by the oncology nurses as they are prone to bleeding diathesis, infections, multiorgan failure and death.[]

  • Sepsis

DIAGNOSIS: The present patient met 5 of the HLH criteria: fever, splenomegaly, bicytopenia, hypertriglyceridemia and/or hypofibrinogenemia, and hyperferritinemia.[] GCS Light sedation, early rehabilitation Blood coagulation system (DIC) Intravascular coagulation, microvascular damage, systemic thrombin generation, endothelial injury Bleeding[] diathesis, microthrombi and tissue ischemia Platelets 3 /μl (prolonged prothrombin time, increased FDP) Antithrombin, recombinant thrombomodulin, concentrated platelet preparation[]

  • Leukoerythroblastic Anemia

Pancytopenia was seen in 9.55% (17) cases and bicytopenia in 50.5% (90) patients.[] […] anemia, Sideroblastic anemia Blood tests Normocytic - Microcytic - Macrocytic - Normochromic - Hypochromic Other Methemoglobinemia Coagulation / platelets / coagulopathy / bleeding[] Pancytopenia was seen in 9.55% (17) cases and bicytopenia in 50.5% (90) patients. Only eight cases had bone marrow correlation.[]

  • Mantle Cell Lymphoma

Case report: A 64-year-old male initially presented with fatigue, splenomegaly, and bicytopenia.[] Management with cryoprecipitate, steroids, rituximab, and antifibrinolytic therapy resolved the bleeding diathesis and suppressed the inhibitor.[] BACKGROUND: Acquired Factor (F)XIII deficiency is a very rare bleeding diathesis with a potentially fatal outcome, previously described in the context of autoimmune disorders[]
