36 Possible Causes for Bicytopenia, Hepatosplenomegaly, Lactate Dehydrogenase Increased

  • Mantle Cell Lymphoma

Most patients present with advanced stage disease with lymphadenopathy, hepatosplenomegaly, and bone marrow involvement.[icd9data.com] Further investigation revealed bicytopenia with splenomegaly and elevated serum creatinine.[ncbi.nlm.nih.gov] MCL manisfests preferentially in middle-aged to older male patients with advanced stage disease with lymphadenopathy, hepatosplenomegaly, and bone marrow involvement.[codes.iarc.fr]

  • Primary Myelofibrosis

The latter was associated more frequently with anaemia, thrombocytopenia, peripheral blood blasts and increased lactate dehydrogenase levels.[ncbi.nlm.nih.gov] The main clinical manifestations of PMF are anemia, bleeding, hepatosplenomegaly, fatigue, and fever.[ncbi.nlm.nih.gov] Patients with primary myelofibrosis may have pancytopenia or bicytopenia and in that respect mimic patients with oligoblastic leukemia (myelodysplasia [MDS]; see Chap. 88)[patient-help.com]

  • Autoimmune Hemolytic Anemia

Unconjugated bilirubin is usually, but not always, elevated and urine urobilinogen is increased. Lactate dehydrogenase is usually elevated into the thousands.[clinlabnavigator.com] A 7-month-old boy, presented with prolonged fever, erythematous rash, severe pallor and hepatosplenomegaly.[ncbi.nlm.nih.gov] Autoimmune bicytopenia in pulmonary tuberculosis. Report of a pediatric case. Respir Med CME 2008;1:281-3. 9. Safe IP, O'brien C, Ferreira FR, Souza ML, Ramasawmy R.[jahjournal.org]

  • Acute Leukemia

Physical examination showed jaundice, hepatosplenomegaly, and peripheral facial nerve palsy in addition to dermal nodules.[ncbi.nlm.nih.gov] A 43-year-old female presented with leukocytosis and bicytopenia.[ncbi.nlm.nih.gov] Symptoms include anemia, fatigue, weight loss, easy bruising, thrombocytopenia, granulocytopenia with bacterial infections, bone pain, lymphadenopathy, hepatosplenomegaly,[medical-dictionary.thefreedictionary.com]

  • Chronic Myeloid Leukemia

[…] serum lactate dehydrogenase level, anemia, and palpable splenomegaly.[doi.org] Due to presenting complaint of bleeding diathesis and absence of hepatosplenomegaly, the case was undiagnosed for CML until the patient reported to us.[ncbi.nlm.nih.gov] In contrast to patients with RA and only dyserythropoiesis, patients with multilineage dysplasia have bicytopenia or pancytopenia, a higher incidence of cytogenetic abnormalities[doi.org]

  • Acute Myelocytic Leukemia

In all three patients, radionuclide imaging with technetium-99m sulfur colloid showed hepatosplenomegaly, decreased liver uptake, and increased splenic activity.[ncbi.nlm.nih.gov] In contrast to patients with RA and only dyserythropoiesis, patients with multilineage dysplasia have bicytopenia or pancytopenia, a higher incidence of cytogenetic abnormalities[bloodjournal.hematologylibrary.org] Symptoms include anemia, fatigue, weight loss, easy bruising, thrombocytopenia, granulocytopenia with bacterial infections, bone pain, lymphadenopathy, hepatosplenomegaly,[medical-dictionary.thefreedictionary.com]

  • Hepatosplenic T-cell Lymphoma

We report a case of a man presenting with an unexplained fever, pancytopenia and hepatosplenomegaly without lymphadenopathy.[ncbi.nlm.nih.gov] There was bicytopenia (hemoglobin - 101 g/L, platelet count - 38 10 9 /L, and total leukocyte count - 5 10 9 /L) with only a few blasts in peripheral blood film [Figure 1][ijpmonline.org] Initial blood work showed mild pancytopaenia and imaging studies revealed hepatosplenomegaly.[ncbi.nlm.nih.gov]

  • Disseminated Intravascular Coagulation

Plasma exchange only improved lactate dehydrogenase levels. We clinically diagnosed this case as atypical HUS and started eculizumab treatment.[ncbi.nlm.nih.gov] Still's disease is a rare inflammatory condition of unknown origin characterized by high spiking fever, arthralgia, arthritis, myalgia, salmon-colored evanescent rash, and hepatosplenomegaly[ncbi.nlm.nih.gov] The patient's platelet count increased, her renal dysfunction improved, and the gangrene on her feet was ameliorated.[ncbi.nlm.nih.gov]

  • Pancytopenia

In the follow-up laboratory examinations, leucocyte and platelet counts in addition to serum bilirubin and lactate dehydrogenase levels normalised.[ncbi.nlm.nih.gov] We present case of two Syrian refugee infants with severe vitamin B12 deficiency with pancytopenia, hepatosplenomegaly and leukoerythroblastosis.[ncbi.nlm.nih.gov] […] of bicytopenia and pancytopenia.[ijpmonline.org]

  • Follicular Lymphoma

A bulky tumor (  10 cm), increased lactate dehydrogenase, age  60 years, and International Prognostic Index (intermediate/high risk), but not Follicular Lymphoma International[ncbi.nlm.nih.gov] Splenomegaly is present in approximately 50% of patients at presentation, and hepatosplenomegaly may occur.[emedicine.com] In addition, the presence of an autoimmune bicytopenia and a Klinefelter syndrome complicated the clinical context of the patient.[ncbi.nlm.nih.gov]
