78 Possible Causes for Bilateral Arm Weakness, Proximal Muscle Weakness, Shoulder Pain

  • Brachial Plexus Neuritis

weakness.[] Nerve conduction velocities generally were slowed maximally in the distal part of the peripheral nerve, despite clinical weakness in the proximal muscle bed.[] Acute brachial plexus neuritis is an uncommon disorder characterized by severe shoulder and upper arm pain followed by marked upper arm weakness.[]

  • Lambert Eaton Myasthenic Syndrome

Patients present with proximal muscle weakness and fatigability, often combined with areflexia. Only 5% of reported cases are children.[] A patient with typical Lambert-Eaton myasthenic syndrome (LEMS) has a clinical manifestation of proximal muscle weaknesses, a larger-than-100% incremental change in repetitive[] Clinical symptoms are usually characterized by proximal muscle weakness and mild dysautonomia.[]

  • Polymyalgia Rheumatica

Shoulder, hip, neck and upper arm pain associated with PMR is usually bilateral and symmetrical.[] Although there was no evidence of proximal muscle weakness, EMG was done which was reported as normal.[] After stabilization for bleeding he was evaluated for shoulder pain. He reported increasing pain and stiffness in his shoulder and pelvic girdle over 1 month.[]

  • Cervical Spondylosis

We found 19 patients with weakness and atrophy of the intrinsic hand muscles, and 13 patients with weakness proximal to the hand muscles to serve as a control group.[] It is a good exercise to remove neck as well as shoulder pain. Stand straight.[] There is often proximal weakness of the lower extremities (mild to moderate iliopsoas muscle weakness occurs in about 50%) and a spasticity of the lower extremity.[]

  • Shoulder Fracture

After 18 days the patient was admitted to our burn centre, complaining of pain, ecchymosis, and bilateral swelling in the arms and shoulders.[] This can occur for many reasons including poor alignment of the fracture once it is healed or from weakness in the muscles that move the shoulder.[] Symptoms of this shoulder injury include pain and swelling at the front of the shoulder.[]

  • Facioscapulohumeral Muscular Dystrophy

The patient had moderate facial weakness and marked winging and high-riding of the scapulae with prominent pectoral and proximal arm muscle atrophy and weakness.[] We report a 12-year-old patient with mental impairment and proximal muscle weakness who had marked involvement of the shoulder girdle and facial muscles.[] We describe a case in a high school throwing athlete that presented as shoulder pain with throwing a baseball.[]

  • Dermatomyositis

Neurological examination revealed bilateral symmetric muscle weakness affecting the shoulders and hip girdle muscles, with strength assessed at 3 of 5 bilaterally.[] Later she developed progressive, symmetrical proximal muscle weakness.[] Muscle pain is less frequent; when muscle pain does occur, it commonly involves pain in the muscles of the shoulder and hip.[]
