603 Possible Causes for Confusion, Dysarthria, Hemiparesis

  • Stroke

SSEH presenting as a hemiparesis is less common and in such situations, it can be mistaken for a cerebrovascular accident (CVA).[ncbi.nlm.nih.gov] Dysarthria is more prevalent in left than in right hemisphere lesions.[doi.org] The symptoms of a stroke include sudden numbness or weakness, especially on one side of the body; sudden confusion or trouble speaking or understanding speech; sudden trouble[ninds.nih.gov]

  • Cerebral Hemorrhage

During the course of illness, the patient had an acute cerebral hemorrhage, which manifested with hemiparesis, dysarthria, and pathologic crying.[ncbi.nlm.nih.gov] Doctors: Sharon stroke caused by bleeding in the brain • Sudden numbness or weakness of the face, arms or legs • Sudden confusion or trouble speaking or understanding others[cnn.com] Two days after the surgery, he developed left hemiparesis. CT scans showed a large cerebral hemorrhage on the right temporal lobe with midline shift by brain metastases.[ncbi.nlm.nih.gov]

  • Subarachnoid Hemorrhage

CASE DESCRIPTION: A 73-year-old woman was transferred to our hospital for sudden headache and subsequent left hemiparesis.[ncbi.nlm.nih.gov] Case Description: A 53-year-old male patient suddenly developed severe headache associated with dysarthria and an altered state of consciousness.[surgicalneurologyint.com] On postoperative day 2, he became confused; and head CT demonstrated intracranial SAH with blood products along the superior cerebellum and bilateral posterior Sylvian fissures[ncbi.nlm.nih.gov]

  • Hypertensive Encephalopathy

A 32-year-old man presented with slight left hemiparesis. He was slightly confused, and his blood pressure was extremely high.[ncbi.nlm.nih.gov] Abstract A 47-year-old man with poorly controlled hypertension presented with headaches, right-sided weakness and dysarthria.[ncbi.nlm.nih.gov] The patient was initially presented with a 4-day history of confusion.[ncbi.nlm.nih.gov]

  • Lyme Disease

Abstract A 46-year-old man presented with recurrent left hemiparesis and headache.[ncbi.nlm.nih.gov] […] nerve deficits ( figure 2 jpg ppt ); - in 9, bladder dysfunction; - in 7, sensory disturbances; - in 6, ataxia; - in 4, altered personality; - in 2, flaccid paresis; - in 2, dysarthria[doi.org] […] requires additional attention by physicians, since the disease has a tendency to progress to the late, recurrent stage or the chronic form, and the associated headache can be confused[ncbi.nlm.nih.gov]

  • Multiple Sclerosis

A severe and sudden left-sided sensorimotor hemiparesis occurred 1 year later.[doi.org] Articulation abilities were evaluated using the articulation subtest from the Fussi assessment (dysarthria scores).[ncbi.nlm.nih.gov] In this review we describe conditions that may be confused with MS because they can present as lesions disseminated in time, space, or both.[ncbi.nlm.nih.gov]

  • Bacterial Meningitis

Meningitis may be localized for a time to one hemisphere or the other, cause jacksonian convulsions, hemiparesis, or even hemianopia.[atsu.edu] Fever was the most common presenting sign/symptom, observed in 8 patients, followed by otalgia, neck stiffness, headache, and confusion.[ncbi.nlm.nih.gov] Symptoms: Viral meningitis - Fever, headache, stiff neck, sensitivity to light, drowsiness, confusion.[childrenshospital.org]

  • Pneumococcal Meningitis

After 7 to 19 days, patients suddenly deteriorated, developing headache, fever, a decreased level of consciousness, brainstem signs, or hemiparesis.[ncbi.nlm.nih.gov] This mutism changed into dysarthria and finally into normal speech. Magnetic resonance imaging revealed lesions in both cerebellar hemispheres, suggesting cerebellitis.[ncbi.nlm.nih.gov] […] features like altered mental status and confusion.[symptoma.com]

  • Insulinoma

The clinical features of an insulinoma are fasting hypoglycemia with neuroglycopenic symptoms including confusion and unusual behavior, while hypertension is usually not associated[ncbi.nlm.nih.gov] Upon regaining consciousness he exhibited severe dysarthria and several interhemispheric disconnection signs such as intermanual conflict, left-hand dysgraphia, left hemispatial[ncbi.nlm.nih.gov] Pediatricians should be aware of confusing and nonspecific symptoms, especially when children with insulinoma present mental or neurological retardation.[ncbi.nlm.nih.gov]

  • Meningococcal Meningitis

Abstract We describe a healthy, afebrile 26-year-old man who presented to the emergency department with left hemiparesis and cranial nerve deficits caused by meningococcal[ncbi.nlm.nih.gov] Its symptoms also include confusion, or irritability in infants, and a stiff neck.[healthline.com] Later symptoms may include paralysis of one side of the body (hemiparesis), hearing loss, or additional neurological abnormalities.[rarediseases.org]
