289 Possible Causes for Exertional Dyspnea, Medication Noncompliance, Tachycardia

  • Asthma

A meta-analysis by DiMatteo et al [ 50 ] revealed that patients with a chronic disease and depression were three times more likely to be noncompliant with medical treatment[dx.doi.org] A 62-year-old man with asthma presented with a 1-month history of wheezing and exertional dyspnea.[ncbi.nlm.nih.gov] The tachycardia, hypertension, and neurological symptoms improved.[ncbi.nlm.nih.gov]

  • Heart Failure

Pathophysiology – Congestive Heart Failure - Diastolic Diastolic dysfunction occurs when the left ventricular myocardium is noncompliant and not able to accept blood return[healio.com] Here we present an 80-year-old Asian woman complaining of progressive exertional dyspnea.[ncbi.nlm.nih.gov] We describe the novel use of bilateral cardiac sympathectomy in a woman with end-stage heart failure caused by ventricular tachycardia refractory to standard medical therapy[ncbi.nlm.nih.gov]

  • Pulmonary Edema

In both procedures, patients developed severe hypertension and tachycardia, with subsequent oxygen desaturations with noted pulmonary edema.[ncbi.nlm.nih.gov] ) Patient noncompliance with medications (eg, diuretics) Severe anemia Sepsis Thyrotoxicosis Myocarditis Myocardial toxins (eg, alcohol, cocaine, chemotherapeutic agents such[emedicine.com] High altitude pulmonary edema (HAPE) is a form of high altitude illness characterized by cough, dyspnea upon exertion progressing to dyspnea at rest and eventual death, seen[ncbi.nlm.nih.gov]

  • Iron Deficiency Anemia

Symptoms may include: Abnormal paleness or lack of color of the skin Irritability Lack of energy or tiring easily (fatigue) Increased heart rate (tachycardia) Sore or swollen[stanfordchildrens.org] Most merely are noncompliant with therapy.[emedicine.medscape.com] Treatment Transfusion should be considered for patients of any age with IDA complaining of symptoms such as fatigue or dyspnea on exertion.[aafp.org]

  • High Output Heart Failure

Medication noncompliance: Frequently, diuretics are not taken as prescribed due to the urinary side-effects.[healio.com] A 51-yr-old man presented exertional dyspnea as a consequence of iliocaval fistula combined with paradoxical pulmonary embolism and high-output heart failure.[ncbi.nlm.nih.gov] Hyperthyroidism causes tachycardia-mediated cardiomyopathy and thus high output heart failure and is often accompanied by atrial fibrillation.[symptoma.com]

  • Chronic Obstructive Pulmonary Disease

The severity of COPD correlates with various arrhythmic manifestations such as atrial fibrillation, atrial flutter, and either sustained or nonsustained ventricular tachycardia[ncbi.nlm.nih.gov] […] with medications, and the presence of serious comorbidities. 6,7 GOALS OF TREATMENT The main goals of treatment for patients with COPD are to optimize their pulmonary capacity[pharmacytimes.com] According to the history of chronic cough and exertional dyspnea, he was treated with inhaled corticosteroids for COPD.[ncbi.nlm.nih.gov]

  • Pneumonia

A 23-month-old girl visited the emergency department with high fever, cough, first wheezing episode, chest retraction and tachycardia.[ncbi.nlm.nih.gov] […] independently associated with greater durations of mechanical ventilation: development of acute renal failure (hazard ratio 1.47, 95% CI 1.02 to 2.12) and prescription of an IDSA-noncompliant[doi.org] She was emergently admitted to our hospital due to the worsening exertional dyspnea and severe hypoxemia.[ncbi.nlm.nih.gov]

  • Atrial Flutter

There will never be a S4 heart sound present during atrial flutter, as this heart sound is produced when atrial contraction forces blood into a noncompliant left ventricle[healio.com] The most encountered signs are fatigue, exertional dyspnea and general discomfort. However this intolerance is often mild. Certain forms are even asymptomatic.[fac.org.ar] There are three major types of SVT including: Atrial fibrillation Paroxysmal supraventricular tachycardia (PSVT) Atrial Flutter & Atrial Tachycardia Atrial Flutter & Atrial[hopkinsmedicine.org]

  • Pulmonary Embolism

DOACs do not require monitoring, and noncompliance will not be readily apparent. Direct oral anticoagulants.[clevelandclinicmeded.com] Dyspnea may be absent or occur only with exertion. The onset of dyspnea may occur over seconds to days. Orthopnea occurs with PE as well as heart failure.[ncbi.nlm.nih.gov] Neurological examination returned to base line status but sinus tachycardia and low saturation value on room air were continuing.[ncbi.nlm.nih.gov]

  • Dilated Cardiomyopathy

Increased dosage frequency, timing of drug administration during school hours, and unpalatability increase noncompliance.[transplantbuddies.org] Left ventricular dysfunction causes exertional dyspnea and fatigue due to elevated left ventricular diastolic pressure and low cardiac output.[merckmanuals.com] Here, we report a Chinese family with typical DCM, ventricular tachycardia, syncope, and SCD.[ncbi.nlm.nih.gov]
