38 Possible Causes for Phosphate Decreased, Starvation Ketosis

  • Diabetic Ketoacidosis

Phosphate concentration decreases with insulin therapy.[] It is possible that starvation ketosis contributed to the development of the high anion gap metabolic acidosis in our patient.[] Starvation ketosis results from poor carbohydrate availability. In starvation ketosis, the anion gap will be mildly elevated.[]

  • Malnutrition

Ketosis, loss of body weight and muscular power accompany a lower metabolic rate.[] It is compatible with life and the same metabolic changes occur as in starvation but to a lesser degree.[] […] ingested. malnutrition the term used to describe the condition caused by a diet that contains all of the essential nutrients but in suboptimal amounts—an intermediate stage to starvation[]

  • Gout

Over 80 % are pure uric acid stones, the rest are mixed calcium oxalate stones or calcium phosphate stones.[] Decreased tubular secretion of urate occurs in patients with acidosis (eg, diabetic ketoacidosis, ethanol or salicylate intoxication, starvation ketosis).[] Stones form due to increased uric acid excretion, acidic urine and substances decreasing uric acid solubility.[]

  • Chronic Alcoholism

Ethanol also causes structural abnormalities in the liver mitochondria which leads to ketosis superimposed by starvation, chronic malnutrition [ 13, 14 ].[]

  • Anorexia Nervosa

Consequently, there may be marked electrolyte derangement (decreases in serum potassium, phosphate, and magnesium levels) and expansion of the extracellular fluid compartment[] ) further magnifies the impact of the malnutrition on the brain, owing to it being in a starvation-illness mode.[] 10] During prolonged food restriction in genetically vulnerable individuals, the ensuing malnutrition perpetuated by the biochemical changes induced by weight loss (ie, ketosis[]

  • Renal Tubular Acidosis

Without NH 3 , the other buffers, primarily phosphate, are overloaded. As a result, the urine pH is acidic.[] […] and alcohol use resulting in acidosis is treated with intravenous glucose, which is administered to stimulate insulin secretion and stop lipolysis and ketosis.[] […] may be normal, due to renal tubular acidosis (RTA) URAEMIC ACIDOSIS Uraemic acidosis results from the loss of functional nephrons involves injury to glomeruli and tubules decreased[]

  • Respiratory Alkalosis

[…] concentration (delta[PO4]p, 0.14 mmol/liter, P less than 0.005) associated with increased fractional excretion of Ca ( 0.5%, P less than 0.005), decreased phosphate clearance[] Another example of increased production of acids occurs in starvation and diabetic acidosis.[] In the placebo group, fractional excretion of phosphate (FEPi) decreased from 24.1% /- 3.4% in NV to 19.2% /- 2.6% in RA.[]

  • Glycogen Storage Disease Type 0

) Increased glycolysis and decreased gluconeogenesis: Increased NADH, NADPH, glycerol and acetylCoA – hypertriglyceridemia.[] Undergoes HMP shunt: Generates excessive ribose-5-phosphate which generates purine nucleotides and they are degraded through the “salvage pathway” into urice acid (hyperuricemia[]

  • Alcoholic Ketoacidosis

[…] level May be normal or elevated initially, but usually decreases with treatment Serum potassium level May be low, normal, or elevated Serum sodium level Usually low Patient[] ketosis.[] […] or greater than 1:2 dilution Serum magnesium level Can be low or normal because of osmotic diuresis Serum osmolality Greater than 320 mOsm per kg (320 mmol per kg) Serum phosphate[]

  • Acidosis

The increased production rate is explained by acidosis-induced hypophosphatemia/cellular phosphate depletion resulting at least in part from decreased renal tubular phosphate[] , alcohol and DKA) Download PDF of GOLDMARK mnemonic Osmolar Gap common differential diagnosis Ketoacids (DKA, AKA, starvation ketosis) Alcohols Sepsis Ischemia Sodium bicarbonate[] Patients have decreased reabsorption of calcium and phosphate, leading to hypercalciuria and hyperphosphaturia.[]
