Adenocarcinoma of the Appendix

Appendix Cancer Adenocarcinoma

Presentation

  • A 40 years old male patient presented in Dhaka Community Medical College Hospital, Dhaka, Bangladesh on 31st October 2013 with mucous adenocarcinoma of the appendix, which primarily presented as heaviness in right lower abdomen and atypical abdominal[ncbi.nlm.nih.gov]
  • Five of the patients presenting were under 60 years of age. During the same period, the total number of appendicectomies performed annually declined by 50%.[ncbi.nlm.nih.gov]
  • They usually present as acute appendicitis or as a right lower quadrant abdominal mass when associated with a mucocele.[ncbi.nlm.nih.gov]
  • The investigation and subsequent management with a review of the literature is presented. We report a case of appendicular adenocarcinoma found unexpectedly in a 43 year old male who presented with urinary symptoms.[ncbi.nlm.nih.gov]
  • Perforation of an adenocarcinoma of the appendix in a pregnant woman is rare and may present as a pelvic mass and acute appendicitis.[ncbi.nlm.nih.gov]
Soft Tissue Mass
  • Cross-sectional imaging usually shows an ill-defined soft tissue mass in the appendix, which can invade adjacent structures 3.[radiopaedia.org]
Aspiration
  • The laparoscopic approach allows thorough exploration of the abdomen, as well as irrigation and aspiration of the thick mucinous material using a 10-mm suction cannula and the instillation of mucolytic agents such as 5% dextrose solution.[ncbi.nlm.nih.gov]
Papule
  • A 69-year-old Japanese woman with erythema, severe edema on the face, Gottron's papules and poikiloderma was diagnosed as having dermatomyositis.[ncbi.nlm.nih.gov]
Muscle Weakness
  • She also noticed muscle weakness in her extremities, although her electromyogram showed neurogenic patterns. Her levels of CA19-9 and CEA were elevated and a CT of her abdomen revealed a giant, multilobular, cystic lesion in the pelvis.[ncbi.nlm.nih.gov]

Workup

Pyuria
  • Here we report a 57-year-old male patient with a history of refractory pyuria and nephrotic syndrome. The kidney biopsy confirmed the diagnosis of not only MGN, but also metastasis of appendix adenocarcinoma.[ncbi.nlm.nih.gov]

Treatment

  • Resection of the ileocecal segment is the first choice treatment in all stages except Dukes A. In Dukes A appendectomy alone is sufficient treatment.[ncbi.nlm.nih.gov]
  • A review of the literature showed the potential for early extension and nodal metastasis in this lesion and led to the recommendation of right hemicolectomy as the treatment of choice.[ncbi.nlm.nih.gov]
  • As with other malignancies, surgical treatment is described. As with other malignancies, surgical treatment is most effective during the early stages of disease.[ncbi.nlm.nih.gov]
  • Abstract Cytoreductive surgery (CRS) followed by hyperthermic intraperitoneal chemotherapy (HIPEC) is a radical but effective treatment option for select peritoneal malignancies.[ncbi.nlm.nih.gov]
  • They exemplify the diverse situations of intraoperative recognition with immediate definitive treatment, or surgical management after appendectomy alone. A review of the literature confirms the rarity of appendiceal adenocarcinoma in older patients.[ncbi.nlm.nih.gov]

Prognosis

  • Well-differentiated adenocarcinomas of the colonic type generally had a better prognosis than signet-ring cell (or microglandular) tumors. The most reliable criterion for correlating prognosis was the Duke's method of staging.[ncbi.nlm.nih.gov]
  • The gross pathologic and microscopic characteristics of this disease are the same as for colon carcinoma and carry a serious prognosis. We report a case of a 64-year old male patient presenting symptoms of acute appendicitis.[ncbi.nlm.nih.gov]
  • The overall prognosis appears to be the same as that for carcinoma of the colon.[ncbi.nlm.nih.gov]
  • Prognosis and histology closely resemble that of colonic adenocarcinoma. Resection of the ileocecal segment is the first choice treatment in all stages except Dukes A. In Dukes A appendectomy alone is sufficient treatment.[ncbi.nlm.nih.gov]
  • The presence of perforation did not necessarily predispose a poor prognosis. Synchronous appendiceal and other colonic neoplasms occurred in 2.7 percent of patients.[ncbi.nlm.nih.gov]

Etiology

  • It was not possible to perform etiological diagnosis in the preoperative period for any of them.[ingentaconnect.com]

Epidemiology

  • METHODS: We queried all patients with MA captured in the Surveillance, Epidemiology, and End Results (SEER) database from 1973 to 2007. Demographics, and tumor and therapy characteristics were extracted.[ncbi.nlm.nih.gov]
  • Different from the appendiceal mucinous neoplasms, these tumors share similar epidemiology and pathology with colorectal adenocarcinoma. As colorectal cancer, these tumors commonly occur in elderly patients, often in the 6 th decade of life 1.[radiopaedia.org]
  • According to a report published by the National Cancer Institute, using the Surveillance, Epidemiology, and End Results (SEER) database, appendix cancer account for about 0.4% of gastrointestinal tumors. There are several subytpes.[rarediseases.info.nih.gov]

  • We also thank the Surveillance, Epidemiology, and End Results Program (SEER) database which provided the free data, and the website ( ) which provided the free R software.[nature.com]

Pathophysiology

  • In this report, the pathophysiology of appendiceal carcinoma and guidelines for therapy are outlined. Emphasis is placed on the possible occurrence of a malignant appendiceal lesion in the elderly patient with acute appendicitis.[ncbi.nlm.nih.gov]

Prevention

  • Posted by on Feb 26, 2013 in Colorectal cancer In a nutshell The present study classified tumors of the appendix and reviewed treatments that aim to prevent cancer spread to the abdominal cavity.[medivizor.com]
