Abstract We present a case of amantadine intoxication that developed unexpectedly in a hospitalized patient with craniocerebral trauma. The intoxication confused the clinical situation and contributed to the patient's deterioration and death. [pubmed.ncbi.nlm.nih.gov]

Anticholinergic manifestations include dry mouth, urinary retention, and mydriasis. Obstructive acute renal failure due to urinary retention has also been reported. Interval changes on the ECG, such as QT prolongation and QRS widening, may be seen. [accessmedicine.mhmedical.com]
Physicians should be aware of urinary retention in patients treated with amantadine as a first sign of intoxication that could lead if untreated to obstructive acute renal failure. [read.qxmd.com]
Although amantadine has not been shown to possess direct anticholinergic activity, clinically, it exhibits anticholinergic-like side effects (dry mouth, urinary retention, and constipation). [5minuteconsult.com]
Neurologic

Use of the extended-release products is contraindicated in end-stage renal disease (CrCl <15 mL/minute/1.73 m2). • Seizure disorder: Use with caution in patients with a history of seizure disorder. [5minuteconsult.com]

Clinical presentation Amantadine intoxication causes agitation, visual hallucinations, nightmares, disorientation, delirium, slurred speech, ataxia, myoclonus, tremor, and sometimes seizures. [accessmedicine.mhmedical.com]
Abrupt discontinuation of amantadine may cause agitation, anxiety, delirium, delusions, depression, hallucinations, paranoia, parkinsonian crisis, slurred speech, or stupor. Upon discontinuation of amantadine therapy, gradually taper dose. [5minuteconsult.com]
- Dizziness
Disease-related concerns: • Cardiovascular disease: Use with caution in patients with heart failure, peripheral edema, or orthostatic hypotension; dizziness, syncope, orthostatic hypotension, presyncope, postural dizziness, and hypotension have been reported [5minuteconsult.com]
Workup
The patient’s workup for encephalopathy included EEG, negative for seizures, repeated head CT, and a lumbar puncture that was negative for infection. Due to continued deterioration, the family decided to proceed with comfort measures only. [shmabstracts.org]
Rhythm
- Torsades De Pointes
Ventricular arrhythmias, including torsade de pointes (See Circulation) and premature ventricular contractions, may occur. Amantadine has also been reported to cause heart failure. [accessmedicine.mhmedical.com]
Treatment
Once the amantadine treatment is stopped, the corneal oedema may be reversible but endothelial density remains low. [pesquisa.bvsalud.org]
Discussion: This case reviews an occurrence of amantadine overdose in the treatment of Parkinson’s disease. [shmabstracts.org]
Although amantadine is no longer recommended for the treatment or prophylaxis of influenza because of resistance, it has been studied as a potential treatment for hepatitis C, Huntington's disease, brain injury or encephalopathy, and cocaine dependence [accessmedicine.mhmedical.com]

抄録 <p>Background: Amantadine is widely used to improve reduced spontaneity and prevent aspiration pneumonia. There are few reports on cases of amantadine intoxication in the field of rehabilitation. [cir.nii.ac.jp]
It is also useful for the prevention of aspiration pneumonia.1) In rehabilitation hospitals, amantadine is widely used to improve reduced spontaneity in patients with cerebrovascular disease or head injuries and for the prevention of aspiration pneumonia [jstage.jst.go.jp]
Mechanism of toxicity Amantadine is thought to increase dopamine levels in the peripheral and central nervous systems by enhancing the release of dopamine and preventing dopamine reuptake. [accessmedicine.mhmedical.com]
It appears to primarily prevent the release of infectious viral nucleic acid into the host cell by interfering with the transmembrane domain of the viral M2 protein. Amantadine is also known to prevent viral assembly during replication. [5minuteconsult.com]
