Presentation
We present a case of a 48-year-old man with histologically confirmed malignant transformation of a benign ameloblastic fibroma 10 years after initial presentation of a localized facial mass. [ncbi.nlm.nih.gov]
Her age at presentation was 22 years old, a little younger than the mean age of presentation, but still in the third decade of life, like most patients from previous reports. [hindawi.com]

Jaw Pain
Unfortunately, in most cases, jaw pain is interpreted as of dental origin and the patient is treated accordingly. Dentists should be familiar with signs and symptoms of malignant tumors of the jaws, especially rare cases with unusual presentation. [ncbi.nlm.nih.gov]

The usual clinical presentation consists of a patient who complains of a painful but occasionally painless facial mass with accompanying paresthesia or dysesthesia. The duration of symptoms varies widely from a few weeks up to 2 years [7]. [hindawi.com]

Treatment
This potential transformation alone does not justify radical treatment of all these benign lesions. [ncbi.nlm.nih.gov]
Surgical resection remains the standard treatment, as no clear or consistent pattern exists for the use of adjuvant therapy in the primary treatment of AFS. [ochsnerjournal.org]
Other treatment modalities such as adjuvant chemotherapy and radiotherapy were found to be ineffective. [11] Our patient has remained tumor free till date after wide surgical excision which is the recommended treatment for AFS. [article.sapub.org]
Prognosis
Its behaviour is relatively benign, with absence of metastatic disease, and the prognosis is reported to be good. It is a paradoxical neoplasm with "sarcomatous" morphological and immunohistochemical patterns but with a favourable clinical course. [ncbi.nlm.nih.gov]

Some authors have proposed that AFS should be considered as low grade fibrosarcoma [7] in view of its more favourable prognosis in comparison to fibrosarcomas of the orofacial region. [article.sapub.org]

Epidemiology and Demographics Fibrosarcoma accounts for about 5% of all primary bone sarcomas. Incidence Fibrosarcoma is a rare disease. Worldwide, the incidence of fibrosarcoma is 0.05 per 100,000 persons. [wikidoc.org]

Pathophysiology Fibrosarcoma is a tumor of mesenchymal cell origin that is composed of malignant fibroblasts in a collagen background. There are two main types: primary and secondary. [emedicine.medscape.com]
Prevention
Knowledge of the malignant potential in the mesenchymal spindle cells of ameloblastic fibroma will assist in determining the management of these benign tumours, and may prevent malignant transformation to ameloblastic fibrosarcoma. [ncbi.nlm.nih.gov]
