Presentation
We present a case report of an unusual CSD presentation, persistent hepatic granulomatous disease due to Bartonella henselae infection despite combination therapy with doxycycline and rifampicin. Furthermore, a review of literature was conducted. [ncbi.nlm.nih.gov]
Immune System
- Cervical Lymphadenopathy
We described an immunocompetent patient with clinical manifestation of chronic cervical lymphadenopathy after a cat-scratch in her forearm. This case shows B. henselae infection persistence even after prolonged antibiotic treatment. [ncbi.nlm.nih.gov]
Entire Body System
- Fever
Osteomyelitis due to B. henselae should be considered in cases of prolonged fever of unknown origin. [ncbi.nlm.nih.gov]
- Anemia
Severe anemia and cholestatic hepatitis are associated with bartonella infections. [ncbi.nlm.nih.gov]
This disease has two distinct phases: Oroya fever: During this phase, fever, headache, muscle aches, abdominal pain, and severe anemia may occur. [cdc.gov]
At admission, he was found to have fever (Temperature 102-103), acute kidney injury (Cr 2.5), gross hematuria, and anemia. Glomerulonephritis was suspected. [onlinejacc.org]
- Weakness
The results showed a weak positive correlation between the degree of infection of ticks and humans living in the same geographic region. [ncbi.nlm.nih.gov]
[…] hypertension (medically managed), rheumatic fever resulting into aortic regurgitation (s/p bovine pericardial bioprosthetic aortic valve replacement in 2009), supraventricular tachycardia, and chronic NSAID use, presented with fever, dyspnea and generalized weakness [onlinejacc.org]
- Congestive Heart Failure
She was afebrile at initial presentation but was found to have massive splenomegaly, and signs of congestive heart failure. No causative organism could be identified on routine blood and tissue cultures. [ncbi.nlm.nih.gov]
Respiratoric
- Dyspnea
A 65-year-old Japanese man was admitted with a 4-month history of fatigue and exertional dyspnea. Transthoracic echocardiography revealed a vegetation on the aortic valve and severe aortic regurgitation. [ncbi.nlm.nih.gov]
[…] controlled on sotalol), hypertension (medically managed), rheumatic fever resulting into aortic regurgitation (s/p bovine pericardial bioprosthetic aortic valve replacement in 2009), supraventricular tachycardia, and chronic NSAID use, presented with fever, dyspnea [onlinejacc.org]
Gastrointestinal
- Regurgitation
Examination confirmed aortic regurgitation, palpable spleen and left facial droop. Multiple peripheral blood cultures were negative. [ncbi.nlm.nih.gov]
Case A 54-year old male with past medical history of paroxysmal atrial fibrillation (controlled on sotalol), hypertension (medically managed), rheumatic fever resulting into aortic regurgitation (s/p bovine pericardial bioprosthetic aortic valve replacement [onlinejacc.org]
Cardiovascular
- Heart Disease
We report a 42-year-old male with underlying surgically corrected congenital heart disease who was diagnosed with Bartonella henselae endocarditis involving the right-ventricle-to-pulmonary-artery conduit. [ncbi.nlm.nih.gov]
- Tachycardia
[…] medical history of paroxysmal atrial fibrillation (controlled on sotalol), hypertension (medically managed), rheumatic fever resulting into aortic regurgitation (s/p bovine pericardial bioprosthetic aortic valve replacement in 2009), supraventricular tachycardia [onlinejacc.org]
Is POTS (Postural Orthostatic Tachycardia Syndrome) typical for ME? POTS and its relationship with ME was discovered a long time ago. This was described then by researchers from Hopkins. POTS is also a disease that has several causes. [markhouse-projects.com]
- Hypertension
Case A 54-year old male with past medical history of paroxysmal atrial fibrillation (controlled on sotalol), hypertension (medically managed), rheumatic fever resulting into aortic regurgitation (s/p bovine pericardial bioprosthetic aortic valve replacement [onlinejacc.org]
Skin
- Angiomatosis
Treatment with clarithromycin resulted in resolution of the bacillary angiomatosis, fever, anemia, panserosites, and hepatitis. [ncbi.nlm.nih.gov]
A species of gram-negative bacteria that is the etiologic agent of bacillary angiomatosis (ANGIOMATOSIS, BACILLARY). This organism can also be a cause of CAT-SCRATCH DISEASE in immunocompetent patients. [connects.catalyst.harvard.edu]
- Skin Lesion
Although our data are preliminary, we report that a cutaneous swab of the skin lesion from a patient in the early stage of cat-scratch disease can be useful for diagnosis of the infection. [ncbi.nlm.nih.gov]
Urogenital
- Hematuria
At admission, he was found to have fever (Temperature 102-103), acute kidney injury (Cr 2.5), gross hematuria, and anemia. Glomerulonephritis was suspected. [onlinejacc.org]
Neurologic
- Irritability
The older son developed intermittent disorientation and irritability, and the mother experienced fatigue, headaches, joint pain and memory loss. [ncbi.nlm.nih.gov]
[…] neuropsychiatric symptoms. 4 - 6 A clinical syndrome in youth, defined by the sudden onset of neuropsychiatric symptoms including obsessions/compulsions or food restriction, has been designated pediatric acute-onset neuropsychiatric syndrome (PANS). 7 Depression, irritability [doi.org]
There’s a relationship between ME/cfs and IBS, Irritable Bowel Syndrome. A lot of people suffer from it, yet there are also many people who don’t have ME but do have IBS. [markhouse-projects.com]
- Seizure
Focal presentations concerning for stroke or partial seizure activity may have underlying infectious etiology. [ncbi.nlm.nih.gov]
Typical symptoms: Papulae/rash and regional lymphadenopathy Atypical symptoms: tiredness (nearly 100%), headache (80%), amyostasia, muscle twitches, tremors, seizures, fever in the mornings (30%, in phases up to six weeks, otherwise 1-3 weeks), chills [arminlabs.com]
- Agitation
[…] tiredness (nearly 100%), headache (80%), amyostasia, muscle twitches, tremors, seizures, fever in the mornings (30%, in phases up to six weeks, otherwise 1-3 weeks), chills, skin pain, swollen lymphnodes, arthralgia (often), myalgia, insomnia, depression, agitation [arminlabs.com]
Case Report A 14-year-old boy was in his usual state of good health until October 2015, when he developed psychiatric symptoms including feeling overwhelmed, confused, depressed, and agitated. [doi.org]

- Blood Culture Positive
PCR for bartonella was negative, but bartonella serology was strongly positive leading to a diagnosis of bartonella endocarditis as the inciting event based on Duke's criteria of: blood cultures positive for endocarditis, predisposing heart condition, [onlinejacc.org]
Treatment
The duration of treatment in patients with bartonella endocarditis is not clearly defined, and we decided to treat our patient with a prolonged course of antibiotic. Surgical treatment is usually necessary and was also successful in our patient. [ncbi.nlm.nih.gov]
Prognosis
These mechanisms suggest a novel link between communicable and non-communicable human diseases, and put forward the possibility that Bartonella henselae could enhance the susceptibility and worsen the prognosis in cardiovascular disease. [ncbi.nlm.nih.gov]

Epidemiology
The epidemiologic association with cats was first made in 1931 and further substantiated throughout the years, refining the interaction predominantly to kittens. [ncbi.nlm.nih.gov]
Consideration is also given to epidemiological aspects in human and animal infections, with special emphasis on the application of new molecular methods for the characterization of isolates. [amazon.fr]
