Presentation
We present two cases of cardiac sarcoidosis whose first presentation was in pregnancy. All findings confirmed the diagnosis of sarcoidosis with cardiac involvement in both patients. [ncbi.nlm.nih.gov]
In most cases, it presents with sudden death, arrhythmia, conduction disorders or cardiomyopathy. [ntvg.nl]

- Military Personnel
In Japanese persons, ophthalmologic and cardiac involvement are more common than in other races. [18] It is more common in certain occupations, namely firefighters, educators, military personnel, persons who work in industries where pesticides are used [en.wikipedia.org]
Gastrointestinal
- Nausea
[…] breast cancer, leukemia, lung cancer, lymphoma, and osteosarcoma. [1] Types of autoimmune diseases it is used for include psoriasis, rheumatoid arthritis, and Crohn's disease. [1] It can be given by mouth or by injection. [1] Common side effects include nausea [en.wikipedia.org]
Cardiovascular
- Heart Failure
He developed recurrent heart failure requiring several admissions to hospital and was finally referred to our institution with severe congestive heart failure. Despite medical treatment with inotropic agents, his symptoms gradually worsened. [ncbi.nlm.nih.gov]
It usually manifests as either heart block, heart failure due to direct myocardial involvement, or cor pulmonale. We present the case of a patient with cardiac sarcoidosis who had ventricular tachycardia and congestive heart failure. [pediatrics.aappublications.org]
Cardiac lesions due to sarcoidosis may lead to fatal arrhythmias or heart failure. [sarcoidosisnews.com]
Cardiac sarcoidosis can cause complete heart block, ventricular and atrial arrhythmias (heart rhythm abnormalities affecting the bottom and top parts of the heart), and congestive heart failure. [ahajournals.org]
- Tachycardia
We present the case of a patient with cardiac sarcoidosis who had ventricular tachycardia and congestive heart failure. Although there was other organ system involvement, the cardiac manifestation was the first to become clinically apparent. [pediatrics.aappublications.org]
A 70-year old Caucasian man with recurrent ventricular tachycardia and progressive biventricular failure attributed to arrhythmogenic right ventricular cardiomyopathy/dysplasia was evaluated for heart transplantation. [ncbi.nlm.nih.gov]

- Heart Block
Cardiac sarcoidosis is one of the uncommon causes of heart failure. Generally, it presents in the form of varying clinical manifestations ranging from asymptomatic to fatal arrhythmias such as ventricular tachycardia and complete heart block. [ncbi.nlm.nih.gov]
Complete heart block may result in extremely slow heart rate, causing lightheadedness, loss of consciousness, or even death. [ahajournals.org]
It usually manifests as either heart block, heart failure due to direct myocardial involvement, or cor pulmonale. We present the case of a patient with cardiac sarcoidosis who had ventricular tachycardia and congestive heart failure. [pediatrics.aappublications.org]
- Chest Pain
The initial clinical manifestations were high-grade atrioventricular conduction block (n=4), heart failure (n=2), ventricular extrasystole (n=2) and acute myocarditis with chest pain and ST-segment elevations (n=1). [academic.oup.com]
Symptoms: Irregular heartbeat (pounding or fluttering sensation, or a ‘skipping of beats’) Dizziness and/or fainting spells Shortness of breath Chest pain Swelling of the legs (in later stages) Pulmonary Hypertension Pulmonary sarcoidosis (sarcoidosis [sarcoidosisuk.org]

Patient history: A 50 year old mixed race man was admitted in Coronary Care Unit after an episode of syncope and sudden onset chest pain associated with palpitations. [scmr.org]
It is less likely to be diagnosed ante-mortem because less than 10% of patients have cardiovascular symptoms. 1,2 It is, therefore, important to recognise overt and non-specific symptoms such as chest pain, breathlessness, palpitations and dizziness, [bjcardio.co.uk]
- Heart Disease
Sarcoid heart disease should be considered in every patient with diagnostic sarcoidosis and heart disease but also in every case of difficult heart disease without an obvious aetiology. [ntvg.nl]
A 58-year-old male with no history of heart disease was admitted to hospital for congestive heart failure due to severe left ventricular dysfunction, and clinically diagnosed with dilated cardiomyopathy. [ncbi.nlm.nih.gov]

Blocks
- Right Bundle Branch Block
The first patient displayed no cardiac symptoms; the electrocardiogram showed an incidental right bundle branch block. [ncbi.nlm.nih.gov]
Electrocardiogram: Showed sinus rhythm with different degrees of AV block, included periods of complete AV block and right bundle branch block (RBBB)-like escape rhythm. [scmr.org]

Intraventricular right bundle branch are most frequent [ 2, 8, 14, 16, 99, 100, 101 ], sinus blocks are exceptional [ 102 ]. [em-consulte.com]

Treatment
Five months later, she presented with acute progressive heart failure that was refractory to conventional treatment. Her sIL2R and ACE levels decreased spontaneously over time, without steroid treatment. [ncbi.nlm.nih.gov]
The treatment protocol was infliximab 5mg/kg iv infusions every 8 weeks. The mean treatment time on infliximab was 14.8 (4–37) months. Eight of nine patients had received other immunosuppressant(s) prior infliximab. [academic.oup.com]

Cardiac involvement in sarcoidosis has been reported to be as high as 50% of cases and it is well documented that it is associated with a poorer prognosis. [ncbi.nlm.nih.gov]
Early treatment prevents irreversible damage of the heart and seems to be associated with better prognosis. [orbi.uliege.be]
Etiology
Sarcoidosis is a systemic disease of unknown etiology, in which granulomas develop in various organs, including the skin, lungs, eyes, or heart. [ncbi.nlm.nih.gov]
This case is presented to illustrate a somewhat unusual, but nevertheless important, etiology of ventricular tachyarrhythmias. [pediatrics.aappublications.org]
