Cardiogenic Pulmonary Edema
Cardiogenic Pulmonary Oedema

Presentation

On presentation, the mean Pco 2 was 64.9 mm Hg and mean pH 7.20; after several hours of treatment, pH was normal and Pco 2 minimally reduced. [jamanetwork.com]

CPO may be a feature of several different types of acute heart failure presentation (see Figure 1). [rcemlearning.co.uk]

[…] doi: 10.1097/TME.0b013e3181946fd8 ARTICLE Abstract Author Information Authors Article Metrics Metrics Acute cardiogenic pulmonary edema (CPE) is a pathology frequently seen in patients presenting to emergency departments (EDs) and can usually be attributed [journals.lww.com]

Introduction Acute pulmonary oedema may be the first presentation of heart failure or an exacerbation of existing known heart failure. [handbook.ggcmedicines.org.uk]

We present our methods and our results below. METHODS The patients were studied in the supine position and all pressure measurements were referred FULL TEXT [annals.org]

Entire Body System

  • Pain

Consider slow titrated small increments of intravenous diamorphine or morphine if associated chest pain or severe distress. Also consider antiemetic. Do not give opiate if patient is drowsy, exhausted or hypotensive. [handbook.ggcmedicines.org.uk]

Infusion is commenced at 2-3mcg/kg/min and increased as required.(1) (d) Disability Morphine should be given early for patients who are agitated and distressed or complaining of chest pain. [rcemlearning.co.uk]

Morphine is therefore no longer recommended for routine use in acute pulmonary oedema. 19 It may be beneficial if there is ongoing chest pain resistant to nitrates. 20 Low doses of morphine (1–2.5 mg) can be useful to facilitate the tolerance of non-invasive [ncbi.nlm.nih.gov]

  • Falling

As their hypoxia worsens they may become agitated and with worsening respiratory failure may become hypercapnoeic causing their conscious level to fall. [rcemlearning.co.uk]

[…] striking feature of acute pulmonary edema: when the work of breathing is greatly intensified, increased ventilation may be associated with such large increases in CO 2 production from respiratory muscles that arterial pCO 2 actually rises instead of falling [academic.oup.com]

Respiratoric

  • Cough

In each patient the onset of pulmonary edema was evidenced by acute dyspnea, cough, mental distress, and the development of râles throughout the lung fields. In one patient frothy pink sputum was expectorated. [annals.org]

Signs and symptoms [ edit ] The most common symptom of pulmonary edema is difficulty breathing, but may include other symptoms such as coughing up blood (classically seen as pink, frothy sputum), excessive sweating, anxiety, and pale skin. [en.wikipedia.org]

Symptoms of ACPE can include dyspnoea, orthopnoea, peripheral oedema, cough, fatigue, and weight gain (Wang 2005). [cochranelibrary.com]

  • Rales

[…] blown into the stomach Gastric distension - large volumes of air swallowed can overcome resistance of lower oesophageal sphincter Anxiety - hypoxia and tight fitting mask can induce anxiety and panic When to stop CPAP: Continue CPAP until chest clear of rales [handbook.ggcmedicines.org.uk]

The heart sounds may be inaudible over the rales from the lungs but a gallop rhythm may be present. Murmurs, especially mitral regurgitation and aortic stenosis, should be listened for, and may reveal a precipitating cause. [rcemlearning.co.uk]

Signs of ACPE can include the presence of a third heart sound, jugular venous distension, rales, lower extremity oedema, wheezing, and ascites (Wang 2005). [cochranelibrary.com]

Cardiovascular

  • Hypertension

Another cardiogenic cause is severe systemic hypertension - specifically a blood pressure that is greater than 180 systolic or 110 diastolic. [osmosis.org]

Recurrence of FPE is thought to be associated with hypertension [16] and may signify renal artery stenosis. [17] Prevention of recurrence is based on managing hypertension, coronary artery disease, renovascular hypertension, and heart failure. [en.wikipedia.org]

This latter group are often hypertensive and have stiff left ventricles which are unable to relax. [rcemlearning.co.uk]

The cause of APE was myocardial ischemia in seven cases and uncontrolled hypertension in remaining 11 cases. Twelve patients were experiencing APE for the first time; history of myocardial infarction was present in 10 patients. [academic.oup.com]

This includes the management of hypertension with antihypertensive drugs, reduction of pulmonary congestion and peripheral oedema with diuretics, and rate control for atrial fibrillation. [ncbi.nlm.nih.gov]

  • Tachycardia

The examination findings are tachycardia, tachypnoea, hypoxia and bibasal inspiratory crepitations. A gallop rhythm may be present. Blood pressure is generally high. [rcemlearning.co.uk]

It also may be secondary to another cause e.g. atrial fibrillation (AF), other tachycardias or bradycardia, critical cardiac ischaemia, valvular disease or renal artery stenosis. [handbook.ggcmedicines.org.uk]

Other adverse effects include reflex tachycardia and paradoxical bradycardia. 13 Nitrates are also associated with tachyphylaxis, with tolerance developing within 16–24 hours of continuous administration. 9 Diuretics There is a lack of controlled studies [ncbi.nlm.nih.gov]

Urogenital

  • Oliguria

A diagnosis of ACPE can have symptoms and clinical signs of hypoperfusion or congestion, or both, such as dyspnoea, pulmonary congestion, jugular venous distension, congestive hepatomegaly, peripheral oedema, confusion, oliguria, and cool extremities [cochranelibrary.com]

Treatment

On presentation, the mean Pco 2 was 64.9 mm Hg and mean pH 7.20; after several hours of treatment, pH was normal and Pco 2 minimally reduced. [jamanetwork.com]

In these cases, dobutamine is usually first-line treatment. [ncbi.nlm.nih.gov]

Because of improved technologies for respiratory support and effectiveness of early treatment with continuous positive airway pressure (CPAP), 1–3 nowadays many patients start CPAP as first-line treatment: 4 the rationale for this choice is the presence [academic.oup.com]

Treatment failure data suggest that not all participants meeting treatment failure criteria were intubated. [cochranelibrary.com]

Prognosis

Only small boluses of 2.5-5mg are recommended as opiates may cause hypotension and/or respiratory depression.(1) Prognosis & Followup strategies Cardiac failure has a very poor long term outcome and the prognosis can be worse than that of many cancers [rcemlearning.co.uk]

Pathophysiology

The pathophysiology of pulmonary edema and shock is discussed in Chaps. 47e and 324, respectively. FIGURE 326-1 Pathophysiology of cardiogenic shock. [accessmedicine.mhmedical.com]

In conclusion, pulmonary congestion in AHF is a complex pathophysiologic process, beyond fluid overload and hemodynamics. [revespcardiol.org]

Prevention

Preclinical studies in ARDS and sepsis suggest that preventing microvascular leak may represent a viable therapeutic approach to prevent or ameliorate organ dysfunction. The challenge is to now go further with carefully designed clinical trials. [oxfordmedicine.com]
