Presentation
The present study reports the case of an elderly patient who presented with crusted scabies combined with bullous scabies. [spandidos-publications.com]
Crusted scabies rarely presents with scalp involvement [ 5, 6, 7, 8 ]. We present a series of four patients with crusted scabies in whom scaly scalp was an evident component of the disease. [escholarship.org]
Combined case–contact pairs presented a median of 12 times per year (range, 4–16) with presentations related to simple or crusted scabies during the combined 57 years of audited records. [mja.com.au]

Weight Loss
Recurrent infections can lead to weight loss as a result of altered metabolism, nutrient loss and decreased appetite. [mja.com.au]
It usually involves the subungueal area with nail hyperkeratosis and dystrophy. 2 A 56-year-old man, alcoholic, presented with a seven-month history of pruritus (with nocturnal exacerbation), weight loss and muscle weakness. [bjid.org.br]
Seven months after complete clearance of crusted lesions, her general health suddenly worsened with intermittent fever, severe weight loss of 10 kg, enlarged lymph nodes, fatigue, night sweating, malaise, and bruised skin. [onlinelibrary.wiley.com]

Splenectomy
The patient had previously undergone splenectomy. She had been taking azathioprine and prednisolone daily for more than 1 year. The rash had begun as itchy minor eruptions 3 months previously. [cmaj.ca]

Cheilitis
Wide oval patches of nonscarring alopecia developed on the scalp, and atrophic glossitis and angular cheilitis were noted on the oral mucosa, along with a white pseudomembranous exudate on the palatum. [mdedge.com]

Nikolsky's Sign
Nikolsky sign testing of the blisters was negative. The general physical examination was normal. Biopsy of a tense bulla excised from the left thigh revealed the formation of an subepidermal blister. [spandidos-publications.com]

Maculopapular Rash
The primary manifestations of noncrusted scabies are the production of a maculopapular rash with occasional vesicles and scales and intense pruritus that worsens at night ( 1 – 5 ). [jcm.asm.org]

Alopecia
Taking into account the patient’s history of autoimmune hypoparathyroidism, hypothyroidism, oral candidiasis, and alopecia areata, he was diagnosed with autoimmune polyglandular syndrome. [mdedge.com]

It has been described in patients who do not scratch because of an absence of pruritus or immobility, such as occurs in patients with mental illness, sensory neuropathy, paresis, debilitating illness, or severe arthropathy. [jamanetwork.com]

People with crusted scabies should receive quick and aggressive medical treatment for their infestation to prevent future outbreaks of scabies. [1] Ivermectin, a drug used to treat parasitic infections, is commonly used for treatment. [2] Last updated [rarediseases.info.nih.gov]

Prognosis
Due to impaired immune response, manifestations may occur in a atypical pattern and pruritus may be mild or absent, which can lead to late diagnosis and worse prognosis. 1–4 A female 47-year-old renal transplant recipient (transplant 16 years ago), with [revistanefrologia.com]
Outlook (Prognosis) Itching may continue for 2 weeks or more after treatment begins. It will disappear if you follow the provider's treatment plan. Most cases of scabies can be cured without any long-term problems. [pennmedicine.org]
Etiology
Crusted scabies also has been shown to develop in Australian natives with normal immunity, though the etiology of the increased susceptibility in this patient population remains unclear. [mdedge.com]
Etiology The disease is caused by the human mite, Sarcoptes scabiei var. hominis. For the taxonomy of the scabies mite, see the article on Scabies. [dermatologyadvisor.com]
Hypotheses include infection-associated, mite-induced basement membrane zone-related, and other etiologies. [dovepress.com]
Epidemiology
Since scabies is an infestation, not an infection, it is NOT reportable as a communicable disease to OCHCA Epidemiology, unless associated with an outbreak. Outbreaks should be reported immediately by phone (714-834-8180) to OCHCA Epidemiology. [ochealthinfo.com]

Prevention
Drawing on experience with a patient who had successfully used short-term treatment creams in a preventive fashion, as well as a preventive model proposed in 2004, 7 the working group approved a provisional protocol to prevent recurrences of crusted scabies [mja.com.au]
“What we’re doing is recommending a preventive treatment,” said MacDonald. [globalnews.ca]
Where to Start You can obtain information on this topic from the Centers for Disease Control and Prevention (CDC). [rarediseases.info.nih.gov]
The Centers for Disease Control and Prevention were contacted and suggested a 7-dose regimen of ivermectin. The patient started to improve remarkably after the third dose, and the skin eruption was resolved after 7 doses. [jddonline.com]
In addition to the patient being treated, the environment must be thoroughly cleaned to remove infective eggs and mites to prevent reinfection ( 1 ). [jcm.asm.org]
