Presentation
Three patients with diverticulum of the ejaculatory ducts, 2 with prostate cysts and 1 patient with seminal vesicle cyst are presented. [ncbi.nlm.nih.gov]
It was found in the present case, however. Language English Title of host publication AMER.J.THER.CLIN.REP. [miami.pure.elsevier.com]
The presenting symptoms and management of our patient is similar to symptomatic seminal vesicle cysts, which is a rare disorder that can be congenital or acquired [15] and often presents with complaints of dysorgasmia and perineal pain [10]. [bjuinternational.com]
Case report A young 31-year-old male was presented to us with hemospermia with a duration of more than 4 years. Consecutive hemospermia was present in each ejaculation with abundant blood clots. [elsevier.es]
Entire Body System
- Fever
There were no others symptoms like perineal pain, fever, dysuria, nocturia or urgency. No urinary tract infection or prostatic infection was reported. [elsevier.es]

The edema and slight enlargement of the prostate with acute inflammation may cause acute rectal, lower back, or perineal pain along with fever. There can be dysuria. The prostate is enlarged and tender. [library.med.utah.edu]
SV abscess in a 63-year-old man with fever. Oblique transrectal US image (a) and contrast-enhanced CT scan (b) show a thick-walled cystic lesion (A) in the right SV, a finding consistent with an abscess. [pubs.rsna.org]
- Swelling
Ejaculates showed very low concentration of fructose, leukocytospermia, teratozoospermia, asthenozoospermia, hyperviscosity, marked sperm agglutination and a poor hypoosmotic swelling test. Clinical value of these findings is discussed. [link.springer.com]

The SV develops as a bulbous swelling of the distal mesonephric duct at 12 weeks gestation (, 1 ) (, Fig 1 ). The mesonephric duct later becomes the VD. [pubs.rsna.org]
- Surgical Procedure
Given the difficulty in identifying and dissecting the inferior hypogastric plexus of nerves during surgical procedures such as retroperitoneal lymph node dissection, iatrogenic injuries may also occur. [emedicine.medscape.com]
SV Abscess An SV abscess may be related to diabetes mellitus, instrumentation, or a surgical procedure. It is often associated with a prostatic abscess. A thick-walled cystic lesion can be seen in the SV at US, CT, or MR imaging (, Fig 12, ). [pubs.rsna.org]
Respiratoric
- Aspiration
Transrectal ultrasound, computed tomography and magnetic resonance imaging together with aspiration of the cyst establish diagnosis and probably prevent unnecessary exploration. [ncbi.nlm.nih.gov]
The introductory chapter includes a dis-cussion on the role and limitations of immunohistochemistry and fine needle aspiration in the diagnosis of prostatic carcinoma. [books.google.de]
The cyst content can be aspirated completely using TRUSG. However, TRUSG-evoked cysts are usually at increased risk of recurrence and infection. Therefore, it is not a long-term effective treatment method [5] [9] [10] [19]. [edoriumjournals.com]
Surgical excision of the seminal vesicle cyst is the preferred definitive management because simple aspiration is often complicated by a recurrence of the cyst and infection of the area. [medical-dictionary.thefreedictionary.com]
Gastrointestinal
- Vomiting
Case Report A 30-year-old man presented with complaints of right abdominal pain, abdominal distension, vomiting, irregular bowel movements, burning micturition and suprapubic pain. Patient had appendectomy 17 years ago. [ispub.com]
Urogenital
- Hematuria
Symptoms included hematospermia in 12 (75%) patients, urinary frequency in 8 (50%), hematuria after ejaculation in 6 (27.5%), perineal malaise in 6 (27.5%), infertility in 3 (13.7%), pain after ejaculation in 3 (13.7%), scrotal pain in 2 (12.5%) and dysuria [ncbi.nlm.nih.gov]
In symptomatic patients, the usual presenting features include perineal pain, abdominal pain, ejaculatory pain, dysuria, hematuria, increased frequency of micturition, urinary tract infections and infertility. [radiopaedia.org]
Others include dysuria, urinary frequency, hematuria, urinary tract infections, infertility, hemospermia and, rarely, enuresis. [appliedradiology.com]
[…] src=mp&spon=34&uac=121613SZ) The first step of the history is to rule out pseudo-hematospermia by determining if hematuria is being misinterpreted as hematospermia or if the blood may have been from the patient's sexual partner (e.g., ask about his partner's [esculape.com]
The most commonly reported symptoms include abdominal, perineal, and pelvic pain; constipation; ejaculatory pain; dysuria; increased urinary frequency; hematuria; hematospermia; urinary tract infection; and symptoms of epididymitis and prostatitis [7,9,13,14,15 [urotoday.com]

Most patients with this anomaly are asymptomatic or present during early adulthood with non-specific symptoms such as prostatism, urinary urgency, dysuria, painful ejaculation and perineal discomfort. [ispub.com]
Classically, EDO presents with blood in the ejaculate (hematospermia), painful ejaculation, or infertility. Associated risk factors include prior urinary tract infection, epididymitis, perineal injury, and testis pain. [theturekclinic.com]
Symptoms of seminal vesiculitis can include vague back or lower abdominal pain; penile, scrotal, or perineal pain; painful ejaculation; hematospermia ; irritative and obstructive voiding symptoms; and impotence. [13] It is usually treated by administration [en.wikipedia.org]
Large cysts cause symptoms such as surrounding organ pressure, cause lower urinary tract symptom (LUTS), prostatitis, pelvic pain, ejaculation disorder and epididymitis [5] [6] [9] [10]. [edoriumjournals.com]

Treatment
Treating blood in semen The treatment your GP or urologist recommends will depend on what they think is the underlying reason for the blood in your semen. [nhs.uk]
Surgical treatment is needed in symptomatic patients. [edoriumjournals.com]
In case of an ectopic ureter, open or laparoscopic nephroureterectomy with removal of seminal vesicles is the most common treatment alternative, since renal function is often absent. [urology-textbook.com]
The patient received also adjuvant chemotherapy and radiation treatment. After 10 years, the follow-up is still negative. [ncbi.nlm.nih.gov]
Prognosis
The current literature suggests that stromal tumor hemorrhage indicates poor prognosis. [dovepress.com]
Treatment and prognosis Depending on the size of the seminal vesicle cyst, surgery may be helpful. Other causes of male pelvic cystic masses can be included as a differential diagnosis. [radiopaedia.org]
The grade and the stage correlate well with each other and with the prognosis. The prognosis of prostatic adenocarcinoma varies widely with tumor stage and grade. [library.med.utah.edu]
Etiology
CONCLUSIONS: Seminal vesicle cysts are rare but should be considered in men with hematospermia and otherwise inexplicable bladder irritation symptoms, perineal discomfort or other genitourinary complaints of unknown etiology. [ncbi.nlm.nih.gov]
The efficacy of seminal vesiculectomy for eliminating our patient’s symptoms further implicates the seminal vesicles as a prime component in the etiology of dysorgasmia. [bjuinternational.com]
Etiology Congenital Congenital cysts occur due to insufficient drainage as a result of atresia of the ejaculatory ducts causing distension of seminal vesicles and further leading to the formation of a cyst. [radiopaedia.org]
Etiological factors include chronic prostatitis as a cause of lateral prostatic cysts and congenital disease as a cause of midline cysts. [elsevier.es]
Open surgery was the final choice since the case was asymptomatic, giant cysts were present and etiological cause could not be completely explained through examination. Operation cystectomy and partial seminal vesiculectomy were made. [edoriumjournals.com]

Pathophysiology
If problematic, the pathophysiology relates to local obstruction of the neck of the bladder. [1] Pathophysiologic variants Blocked duct Blockages of the posttesticular portion of the reproductive tract may occur. [emedicine.medscape.com]
Prevention
Transrectal ultrasound, computed tomography and magnetic resonance imaging together with aspiration of the cyst establish diagnosis and probably prevent unnecessary exploration. [ncbi.nlm.nih.gov]
Care was taken to spare the bladder neck and verumontanum to prevent retrograde ejaculation ( Fig. 2 ). The resection resulted in drainage of a clear fluid. [elsevier.es]
