Cyst of the Seminal Vesicles

Seminal Vesicles Cyst

Presentation

  • Three patients with diverticulum of the ejaculatory ducts, 2 with prostate cysts and 1 patient with seminal vesicle cyst are presented.[ncbi.nlm.nih.gov]
  • Abstract A case of an infertile man with bilateral seminal vesicles cysts with ipsilateral renal agenesis is presented with emphasis on the semen analysis.[link.springer.com]
  • We present a case of a boy, followed up in our hospital since birth for right renal agenesis who at the age of 4 years presented a right paravesical cyst on ultrasound. The cyst was initially considered as an ureterocele.[archive-ouverte.unige.ch]
Infertility
  • Abstract A case of an infertile man with bilateral seminal vesicles cysts with ipsilateral renal agenesis is presented with emphasis on the semen analysis.[link.springer.com]
  • Seminal vesical cysts are usually asymptomatic but can be associated with infertility.[advancedradteaching.com]
  • This paper presents four cases illustrating the uncommon disorders that TRUS can demonstrate in the seminal vesicles: ectopic ureter entering the seminal vesicle, symmetrically enlarged seminal vesicles with male infertility, a pelvic arteriovenous malformation[ncbi.nlm.nih.gov]
  • Ejaculatory duct obstruction is the cause of infertility in 1-5% of infertile men. Although unusual, ejaculatory duct obstruction is diagnosed and treated with minimally invasive techniques, many of them pioneered by Dr. Turek.[theturekclinic.com]
  • He was admitted due to primary infertility which had developed three years ago. The his semen analysis revealed oligospermia. No evidence of a family history of renal dysplasia was reported.[jpatholtm.org]
Asymptomatic
  • Seminal vesical cysts are usually asymptomatic but can be associated with infertility.[advancedradteaching.com]
  • Small cysts of the ejaculatory ducts were asymptomatic.[ncbi.nlm.nih.gov]
  • They are usually asymptomatic but symptomatic when they all of large size. Ectopic ureteral opening is less common with seminal vesicle. Treatment of seminal vesicle cyst in symptometic patients is surgery.[edoriumjournals.com]
  • As long as the contralateral kidney remains functional, unilateral renal agenesis is asymptomatic. The contralateral kidney usually becomes hypertrophic and enlarged to compensate. Contrast enhanced CT shows no left kidney in the renal fossa.[med-ed.virginia.edu]
  • SVC is usually asymptomatic [13] . Those smaller than 5 cm in diameter can remain asymptomatic and are usually discovered incidentally [11,14] . However, the cyst can grow and induce inflammation and stimulate surrounding viscera [13] .[urotoday.com]
Swelling
  • Ejaculates showed very low concentration of fructose, leukocytospermia, teratozoospermia, asthenozoospermia, hyperviscosity, marked sperm agglutination and a poor hypoosmotic swelling test. Clinical value of these findings is discussed.[link.springer.com]
  • 70% of the ejaculate. en — Flat velvety lesions not progressing to vesicles ro Şi pentru următorul judecător la Curtea Supremă de Justiţie en Application site reactions including pain, ulcer, irritation, paraesthesia, anaesthesia, erythema, oedema, swelling[ro.glosbe.com]
  • The SV develops as a bulbous swelling of the distal mesonephric duct at 12 weeks gestation ( , 1 ) ( , Fig 1 ). The mesonephric duct later becomes the VD.[pubs.rsna.org]
Surgical Procedure
  • Given the difficulty in identifying and dissecting the inferior hypogastric plexus of nerves during surgical procedures such as retroperitoneal lymph node dissection , iatrogenic injuries may also occur.[emedicine.medscape.com]
  • SV Abscess An SV abscess may be related to diabetes mellitus, instrumentation, or a surgical procedure. It is often associated with a prostatic abscess. A thick-walled cystic lesion can be seen in the SV at US, CT, or MR imaging ( , Fig 12 , ).[pubs.rsna.org]
Pathologist
  • CRC Press , 31.12.1988 - 328 Seiten Written expressly for the surgical pathologist, the pathology resi-dent and the urologic surgeon, this volume describes the histopathologic appearances of diseases affecting the prostate and the seminal vesicles.[books.google.de]
Aspiration
  • Transrectal ultrasound, computed tomography and magnetic resonance imaging together with aspiration of the cyst establish diagnosis and probably prevent unnecessary exploration.[ncbi.nlm.nih.gov]
  • The introductory chapter includes a dis-cussion on the role and limitations of immunohistochemistry and fine needle aspiration in the diagnosis of prostatic carcinoma.[books.google.de]
  • Surgical excision of the seminal vesicle cyst is the preferred definitive management because simple aspiration is often complicated by a recurrence of the cyst and infection of the area.[medical-dictionary.thefreedictionary.com]
  • The cyst content can be aspirated completely using TRUSG. However, TRUSG-evoked cysts are usually at increased risk of recurrence and infection. Therefore, it is not a long-term effective treatment method [5] [9] [10] [19] .[edoriumjournals.com]
Abdominal Distension
  • Case Report A 30-year-old man presented with complaints of right abdominal pain, abdominal distension, vomiting, irregular bowel movements, burning micturition and suprapubic pain. Patient had appendectomy 17 years ago.[ispub.com]
Vomiting
  • Case Report A 30-year-old man presented with complaints of right abdominal pain, abdominal distension, vomiting, irregular bowel movements, burning micturition and suprapubic pain. Patient had appendectomy 17 years ago.[ispub.com]
Suprapubic Pain
  • Case Report A 30-year-old man presented with complaints of right abdominal pain, abdominal distension, vomiting, irregular bowel movements, burning micturition and suprapubic pain. Patient had appendectomy 17 years ago.[ispub.com]
Urinary Retention
  • We report on a case of a male patient presenting with urinary retention owing to a large hydatid cyst of the seminal vesicles. The literature regarding this rare clinical entity is also reviewed.[ncbi.nlm.nih.gov]
  • retention for 3 months was slightly hypertensive, normoglycaemic, with normal renal biochemical parameters.[jbsr.be]
  • A 30-year-old man, catheterised following urinary retention, was found to have a complex pelvic retrovesical cyst on imaging. At operation, origin from the right seminal vesicle was evident with histopathology confirming a GIST.[casereports.bmj.com]
  • The urinary tract obstruction leads to urinary retention and risk for infection. In severe, prolonged cases, hydroureter with hydronephrosis and renal failure can ensue.[library.med.utah.edu]

Burning Micturition
  • Case Report A 30-year-old man presented with complaints of right abdominal pain, abdominal distension, vomiting, irregular bowel movements, burning micturition and suprapubic pain. Patient had appendectomy 17 years ago.[ispub.com]

Workup

Hyperviscosity
  • Ejaculates showed very low concentration of fructose, leukocytospermia, teratozoospermia, asthenozoospermia, hyperviscosity, marked sperm agglutination and a poor hypoosmotic swelling test. Clinical value of these findings is discussed.[link.springer.com]

Treatment

  • The patient received also adjuvant chemotherapy and radiation treatment. After 10 years, the follow-up is still negative.[ncbi.nlm.nih.gov]
  • Surgical treatment is needed in symptomatic patients.[edoriumjournals.com]
  • Treating blood in semen The treatment your GP or urologist recommends will depend on what they think is the underlying reason for the blood in your semen.[nhs.uk]
  • In case of an ectopic ureter, open or laparoscopic nephroureterectomy with removal of seminal vesicles is the most common treatment alternative, since renal function is often absent.[urology-textbook.com]

Prognosis

  • The current literature suggests that stromal tumor hemorrhage indicates poor prognosis.[dovepress.com]
  • Treatment and prognosis Depending on the size of the seminal vesicle cyst, surgery may be helpful. Other causes of male pelvic cystic masses can be included as a differential diagnosis.[radiopaedia.org]
  • The grade and the stage correlate well with each other and with the prognosis. The prognosis of prostatic adenocarcinoma varies widely with tumor stage and grade.[library.med.utah.edu]

Etiology

  • Etiology Congenital Congenital cysts occur due to insufficient drainage as a result of atresia of the ejaculatory ducts causing distension of seminal vesicles and further leading to the formation of a cyst.[radiopaedia.org]
  • The efficacy of seminal vesiculectomy for eliminating our patient’s symptoms further implicates the seminal vesicles as a prime component in the etiology of dysorgasmia.[bjuinternational.com]
  • CONCLUSIONS: Seminal vesicle cysts are rare but should be considered in men with hematospermia and otherwise inexplicable bladder irritation symptoms, perineal discomfort or other genitourinary complaints of unknown etiology.[ncbi.nlm.nih.gov]
  • Etiological factors include chronic prostatitis as a cause of lateral prostatic cysts and congenital disease as a cause of midline cysts.[elsevier.es]
  • Open surgery was the final choice since the case was asymptomatic, giant cysts were present and etiological cause could not be completely explained through examination. Operation cystectomy and partial seminal vesiculectomy were made.[edoriumjournals.com]

Pathophysiology

  • If problematic, the pathophysiology relates to local obstruction of the neck of the bladder. [1] Pathophysiologic variants Blocked duct Blockages of the posttesticular portion of the reproductive tract may occur.[emedicine.medscape.com]

Prevention

  • Transrectal ultrasound, computed tomography and magnetic resonance imaging together with aspiration of the cyst establish diagnosis and probably prevent unnecessary exploration.[ncbi.nlm.nih.gov]
  • Care was taken to spare the bladder neck and verumontanum to prevent retrograde ejaculation ( Fig. 2 ). The resection resulted in drainage of a clear fluid.[elsevier.es]
