Presentation

Although with virtually no evidence, we are presenting a case that responded spectacularly to methylphenidate. [ncbi.nlm.nih.gov]

Entire Body System

  • Asymptomatic

Three quantitative EEG (QEEG) studies were performed on the subject, one during the index depersonalization episode and two subsequent studies when the subject was clinically asymptomatic. [ncbi.nlm.nih.gov]

Thyroid dysfunction is related to nervous system disorder involving GABA as reported by clinical studies. [8] The role of thyroid with regard to cognition is not yet clear. [9] Studies done recently shows that the cases previously considered asymptomatic [doi.org]

  • Unconsciousness

Patients with the most minor head injuries (no loss of consciousness or unconsciousness 23 Grigsby and Kaye 17 extrapolated from these figures and estimated that a minimum of 13% of patients would develop depersonalization following a head injury. [doi.org]

Gastrointestinal

  • Nausea

Her attacks tended to occur monthly, generally premenstrually, and consisted of intense abdominal pain radiating to both legs, accompanied by headache, nausea, and occasionally vomiting. [doi.org]

One patient experienced persistent nausea after commencing lamotrigine and stopped taking it after a week. [ncbi.nlm.nih.gov]

  • Vomiting

Her attacks tended to occur monthly, generally premenstrually, and consisted of intense abdominal pain radiating to both legs, accompanied by headache, nausea, and occasionally vomiting. [doi.org]

Eyes

  • Blurred Vision

Thivierge and Julien 20 described a patient who developed headaches, blurred vision, and depersonalization following a minor head injury with no or minimal loss of consciousness and no reported posttraumatic amnesia (PTA). [doi.org]

Psychiatrical

  • Depersonalization

Primary depersonalization disorder is believed to be resistant to treatment. [ncbi.nlm.nih.gov]

  • Anhedonia

In patients with BD, age of onset of the disease correlated with affective depersonalization factor scores (rho = -0.330, P = .001) but not with anhedonia factor scores (rho = -0.097, P = .361). [ncbi.nlm.nih.gov]

Neurologic

  • Aura

Lancet 1998 ; 351:1934 Crossref, Medline, Google Scholar 63 Ogunyemi AO: Migraine with prolonged aura: correlation of clinical and EEG features. [doi.org]

  • Giddiness

All of the patients also suffered episodes of vertigo or giddiness, and all had a psychiatric comorbidity—predominantly of anxiety. [doi.org]

  • Convulsions

Fichtner et al. 1992 ;Ratliff & Kerski 1995 ); fluoxetine and buspirone, (Abbas et al. 1995 )), SNRIs (venlafaxine, (Preve et al. 2011 )), a combination of benzodiazepines and serotonergic activity drugs (citalopram-clonazepam, (Sachdev 2002 )), anti-convulsants [doi.org]

  • Agitation

The biggest difference between the groups, in terms of greater number of manifest symptoms in the patients with depersonalization, was for psychomotor disturbances (agitation or retardation), insomnia, decrease of energetic potentials and concentration [ncbi.nlm.nih.gov]

Treatment

Severe depersonalization at times constitutes a chronic and disabling syndrome for which there is no generally established etiology or treatment. [ncbi.nlm.nih.gov]

Prognosis

U.S. and U.K. samples of those experiencing clinically significant DP symptoms [ 1, 13 ] have found DP to have an approximately an equal gender split, a common onset in late adolescence or early adulthood, and often to have a chronic and persistent prognosis [ncbi.nlm.nih.gov]

Etiology

Severe depersonalization at times constitutes a chronic and disabling syndrome for which there is no generally established etiology or treatment. [ncbi.nlm.nih.gov]

Epidemiology

In Germany, its administrative prevalence is far below the threshold for orphan diseases, although according to epidemiological surveys the diagnosis should be comparable frequent as anorexia nervosa for instance. [ncbi.nlm.nih.gov]

S. ( 2004 ) The epidemiology of depersonalisation and derealisation: a systematic review. Social Psychiatry and Psychiatric Epidemiology, 39, 9 – 18. Kaufman, K. R. & Gerner, R. ( 1998 ) Lamotrigine toxicity secondary to sertraline. [doi.org]

Pathophysiology

Previous data having indicated a role of the serotonin system in the pathophysiology of the phenomenon, a noradrenaline induced serotonin liberation of Raphe neurons is suggested as possible underlying mechanism. [ncbi.nlm.nih.gov]

Prevention

Continued Can Depersonalization Disorder Be Prevented? Although it might not be possible to prevent depersonalization disorder, it might be helpful to begin treatment in people as soon as they begin to show symptoms. [webmd.com]

Cognitive reappraisal might help people suffering from fatigue to prevent depersonalization and derealization tendencies. Copyright © 2017 Elsevier B.V. All rights reserved. [ncbi.nlm.nih.gov]
