Presentation

This report concerns a patient who presented with CHF and severe thrombocytopenia (5,000/microl). Serum concentrations of K, Mg and digitoxin were 3.2mmol/L, 1.4mg/L and 57.2ng/ml, respectively. [ncbi.nlm.nih.gov]

Lymphoma and leukemia are presented as a single group in the tables due to the close relationship between them. [doi.org]

Entire Body System

  • Hodgkin Lymphoma

Our ratios are well in agreement with a previous study focusing on drug use and other factors preceding non Hodgkin lymphoma [ 15 ]. [doi.org]

Gastrointestinal

  • Vomiting

Thus, neither the vomiting nor the gastric lavage eliminated significant amounts of the drug which had left the stomach without delay. [ncbi.nlm.nih.gov]

Rule out digitalis toxicity if nausea, vomiting, or arrhythmias develop Therapeutic range: 9-25 ng/ml [medicalcorps.org]

Vomiting may be induced with syrup of Ipecac. If elapsed time since ingestion of digitoxin is unknown or suspected to be greater than 30 minutes, do not induce vomiting and proceed to Step 4. [cameochemicals.noaa.gov]

  • Nausea

Rule out digitalis toxicity if nausea, vomiting, or arrhythmias develop Therapeutic range: 9-25 ng/ml [medicalcorps.org]

Two Italian adults arrived at the Emergency Department referring diarrhea, nausea and vomiting for 4 days; weakness, fatigue and visual hallucinations were also complained of. [ncbi.nlm.nih.gov]

Digitoxin exhibits similar toxic effects to the more commonly used digoxin, namely: anorexia, nausea, vomiting, diarrhea, confusion, visual disturbances, and cardiac arrhythmias. [en.wikipedia.org]

Remarks Stability After separation from cells: Ambient: 5 days; Refrigerated: 5 days; Frozen: 2 months Effective November 16, 2015 Therapeutic Range: 10.0-30.0 ng/mL Toxic: Greater than 45.0 ng/mL Toxic concentrations may cause nausea, vomiting and cardiac [ltd.aruplab.com]

Toxicity : Digitoxin exhibits similar toxic effects to the more-commonly used digoxin, namely: anorexia, nausea, vomiting, diarrhoea, confusion, visual disturbances, and cardiac arrhythmias. [lgmpharma.com]

  • Diarrhea

Two Italian adults arrived at the Emergency Department referring diarrhea, nausea and vomiting for 4 days; weakness, fatigue and visual hallucinations were also complained of. [ncbi.nlm.nih.gov]

[…] seizures, weakness CV: atrial fibrillation, AV block, bradycardia, premature ventricular contractions (PVC’s), ventricular fibrillation, ventricular tachycardia EENT: visual disturbances (blurred, yellow or green halo effect) GI: abdominal discomfort, diarrhea [medicalcorps.org]

Case report A married couple (a 61 year old man and his wife aged 60) arrived at the Maggiore della Carità Hospital (Novara, Italy) because of diarrhea, nausea and vomiting for 4 days; they also complained weakness, fatigue and visual hallucinations [biochemia-medica.com]

If digitalis toxicity is suspected, cardioversion should be postponed {01} ) » Electrolyte disorders (resulting from dialysis, diarrhea, diuretics or other medications, malnutrition, prolonged vomiting, etc.) {01}, especially {01} » Hypercalcemia {01} [drugs.com]

Cardiovascular

  • Slow Pulse

Signs and Symptoms of Digitoxin Exposure: Acute digitoxin ingestion may produce the following signs and symptoms: nausea, vomiting, headache, lethargy, fatigue, weakness, drowsiness, confusion, slowed pulse, varying degrees of heart block, heart arrhythmias [cameochemicals.noaa.gov]

pulse, palpitations, and/or fainting as possible signs of overdose; initial overdose signs may occur as gastrointestinal effects in adults and as cardiovascular effects in children Carrying medical identification card » Not taking other medications without [drugs.com]

Eyes

  • Blurred Vision

Eye Blurred vision Photophobia Optic neuritis Scotomas (blind spots on retina) Disturbance of colour vision (esp. yellow) Xanthopsia 6. Serum Potassium Acute digitalis over dosage causes hyperkalemia due to inhibition of Na+ K+ ATPase. [howmed.net]

Musculoskeletal

  • Muscle Weakness

Anorexia Nausea Vomiting Headache Fatigue Malaise Drowsiness Muscle weakness Dizziness Vertigo Digitoxin may also cause side-effects not listed here. If you notice other side-effects not listed above, contact your doctor for medical advice. [tabletwise.com]

Workup

Rhythm

  • Sinus Arrest

Toxic symptoms (sinus arrest) reappeared twice and were accompanied by increasing amounts of non-antibody-bound DGTX in the serum. Additional application of FAB (2×80 mg) resulted in the immediate disappearance of arrhythmia. [doi.org]

  • Nodal Tachycardia

[…] are associated with excessive dosing in infants and children are cardiac arrhythmias, especially conduction disturbances and supraventricular tachycardias ( atrial tachycardia [with or without block] and junctional [nodal] tachycardia ) {01}. [drugs.com]

Treatment

Blood and urine samples were collected over a 98 h period for monitoring the efficacy and adequacy of FAB treatment. [doi.org]

Collectively, this study provides a basis for the reevaluation of cardiac glycosides in the treatment of breast cancer and more importantly reveals their potential in the treatment of triple-negative breast cancers. [ncbi.nlm.nih.gov]

Epidemiology

Epidemiological studies indicate that patients on digitalis therapy are more protected from cancer. Evidence of a selective cytotoxicity against cancer cells has suggested their potential use as anticancer drugs. [ncbi.nlm.nih.gov]

Hasford 2 1 Department of Clinical Pharmacology, Friedrich Schiller University of Jena, 2 Department of Medical Informatics, Biometry, Epidemiology, Pharmacoepidemiology Research Group, Ludwig Maximilians University, Munich DOI 10.5414/CPP39336 Abstract [doi.org]

More recently, digitoxin is recognized for its profound anticancer activity from both epidemiological and in vitro research studies. Concern remains, however, due to the low threshold for cardiotoxicity and its narrow therapeutic window. [northeastern.edu]

However, some early epidemiologic studies, describing significantly lower growth potential of tumors on patients on digitalis treatment, sparked new interest in the anticancer properties of these drugs ( 2 ). [mct.aacrjournals.org]

Pathophysiology

The inflammatory process is believed to be caused by massive overproduction of the proinflammatory protein IL-8, and the high levels of IL-8 in the CF lung are therefore believed to be the central mechanism behind CF lung pathophysiology. [ncbi.nlm.nih.gov]

Thus digitoxin and its related active analogues can be considered as potentially useful in addressing the central mechanism of CF lung pathophysiology. [doi.org]

Prevention

Additionally, digitoxin prevented TNF-alpha-induced apoptosis in EC accompanied by activation of Akt. [ncbi.nlm.nih.gov]
