Presentation
Six cases presented with cutaneous lesions, and five of these were found to have other systemic involvement. [ncbi.nlm.nih.gov]
A 39-year-old man presented with acute hemoptysis and chills. Computed tomography (CT) of his chest showed an infiltrate in the right upper lobe (Panel A), and he was treated with levofloxacin. [nejm.org]
Case Presentation: The patient was a 27 year old male who presented to the ED in July 2016 with persistent left sided chest pain and a lump over his sternum of several weeks in duration. Pain was exacerbated by touch and exertion. [shmabstracts.com]
Primary cutaneous blastomycosis has infrequently been reported after dog bites also; such patients may not disease clinically active pulmonary as in our present case. [3] However, the parents denied any history of dog bite in the present patient. [ijpd.in]
Immune System
- Generalized Lymphadenopathy
There was generalized lymphadenopathy and no organomegaly, and all other systems were normal. There was an airway obstruction and the case was referred to an ENT specialist and ophthalmologist for further examination. [ijdvl.com]

When it resembles bacterial pneumonia, symptoms include high fever, chills, a productive cough with brown or bloody-looking sputum, and chest pain of the lungs. [encyclopedia.com]
- Malaise
Acute Blastomycosis Blastomycosis typically presents as a flulike illness characterized by fever, malaise, fatigue, weight loss, and pulmonary involvement. [hivinsite.ucsf.edu]
Fever, chills, night sweats, malaise, and weight loss are common. When the infection is severe, the patient can present in septic shock, can have symptoms and signs of adrenal insufficiency, and can develop disseminated intravascular coagulation. [mdpi.com]
- Rigor
Our case was a 47-year-old man with past medical history significant for diabetes, hyperlipidemia, and chronic pancreatitis who presented with a 2-week history of fever, chills, rigors, constipation, and 10 pound weight loss. [ncbi.nlm.nih.gov]
Respiratoric
- Dyspnea
The symptoms include fever, night sweats, weight loss, fatigue, dyspnea, cough, sputum production, and hemoptysis. The chest radiograph shows upper lobe cavitary infiltrates, masslike lesions, or multiple nodular lesions (see Figure 40-4). [aibolita.com]
Signs & Symptoms Blastomycosis is an infectious disease characterized by fever, chills, headaches, chest pain, weight loss, night sweats, cough, and/or difficulty breathing (dyspnea). [rarediseases.org]
Symptoms include a productive or dry hacking cough, chest pain, dyspnea, fever, chills, and drenching sweats. Pleural effusion occurs occasionally. [merckmanuals.com]
Cough, sputum production, hemoptysis, and dyspnea are noted. The lesions may be cavitary, nodular, fi-brotic, or masslike in appearance.31,32 Hilar and mediastinal lymphadenopathy and pleural effusions are uncommonly seen. [what-when-how.com]
In a patient who is not markedly immunosuppressed, focal pneumonia can occur, presenting with fever, myalgias, dry cough, and dyspnea. [mdpi.com]
- Pleural Effusion
Lymphadenopathy and pleural effusions may be uncommonly present. Promoted articles (advertising) [radiopaedia.org]
Pleural effusion occurs occasionally. Some patients have rapidly progressive infections, and acute respiratory distress syndrome may develop. In extrapulmonary disseminated blastomycosis, symptoms depend on the organ involved. [merckmanuals.com]

Gastrointestinal
- Diarrhea
Patients with gastrointestinal tract involvement frequently have diarrhea, weight loss, and abdominal pain; endoscopy often shows diffuse involvement throughout the gut. [mdpi.com]
Skin
- Night Sweats
Symptoms include high fever, chills, cough, fatigue, shortness of breath, night sweats and sputum production. -A small percentage of these patients develop nodules or cavities in the lungs. [quizlet.com]

She had progressively worse pain and hemoptysis as well as fevers and night sweats, with weight loss. A chest x-ray and CT scan showed a left upper lobe mass- like infiltrate suspicious for a carcinoma. [labmedicineblog.com]
When it looks like tuberculosis or lung cancer, symptoms include a low-grade fever, productive cough, night sweats, and weight loss. [encyclopedia.com]
- Papule
Papules, pustules, or nodules are most frequently found on exposed body areas. They may look like warts or ulcers. They are usually painless. They may vary from gray to violet in color. [medlineplus.gov]
Papules or papulopustules usually appear on exposed surfaces and spread slowly. Painless abscesses, varying from pinpoint to 1 mm in diameter, develop on the advancing borders. Irregular, wartlike papillae may form on surfaces. [merckmanuals.com]
In addition, there were multiple umbilicated papules and nodules over the extremities. There was a huge flesh-colored mass protruding from the left eye [Figure 1] a and b. [ijdvl.com]
Disseminated Blastomycosis Many patients with blastomycosis have no pulmonary symp-toms.26,31 They present with cutaneous lesions that are well circumscribed, nonpainful papules, nodules, or plaques that often become verrucous and develop punctate drainage [what-when-how.com]
Case report: We present a 14-year-old cardiac transplant patient, who presented 36 days following his transplantation with acute respiratory distress followed a few days later by erythematous cutaneous papules. [thedoctorsdoctor.com]
Workup
After admission, workup of his symptoms was initiated including CT chest which revealed several sternal and thyroid lesions. Other lab work included fungal serologies and biopsies of both his sternal and thyroid lesions. [shmabstracts.com]
As the physician does a workup usually a chest x-ray is ordered, which will alert the physician that there is something different from the ordinary pneumonia going on. [nethealthbook.com]
Laboratory Identification: The workup revealed a thermally dimorphic fungus with a mold form growing in the laboratory at 25°C and a yeast form present in the surgical pathology specimen. [labmedicineblog.com]
We have reviewed the utilization of diagnostic techniques in the workup of patients with pulmonary blastomycosis, defined their diagnostic yields, and proposed an optimal diagnostic approach for the patient in whom pulmonary blastomycosis is considered [thedoctorsdoctor.com]

There are limited data on the occurrence of blastomycosis in this population, and the optimal treatment is unknown. [hivinsite.ucsf.edu]

Therapy must be individualized and must be given for life.49 Prognosis Of all the endemic mycoses, infection with C. immitis is the least responsive to currently available antifungal agents. [what-when-how.com]
Etiology
The etiology of the patient's pulmonary infection was not known, but tuberculosis was an important consideration. [ncbi.nlm.nih.gov]
In spite of diligent investigations by many competent investigators, we still do not know the ecological niche inhabited by the etiologic agent. We have many tantalizing clues but no definite answers. [books.google.com]
Radiograph of both knees and elbows revealed creeping type of periosteal reaction, which was suggestive of fungal etiology [Figure 5] a and b. [ijdvl.com]
