Presentation
The present study suggests that restoration of sinus rhythm is associated with improved survival. [ncbi.nlm.nih.gov]
Patients with AF and atrial flutter (AFl) at baseline were included in the present analysis. [ahajournals.org]
Entire Body System
- Impaired Exercise Tolerance
Unfortunately, atrial fibrillation is common in patients with heart failure and can impair exercise tolerance, exacerbate symptoms, and have hemodynamic and thromboembolic consequences. 1-4 Although digitalis can attenuate the ventricular response in [doi.org]
- Arm Pain
[…] as stuffy nose, sneezing, or sore throat Mild skin rash Serious Side Effects of Tikosyn Tell your doctor immediately if you experience any of the following serious side effects: Fast, pounding, or irregular heartbeat Chest, jaw, or arm pain Shortness [everydayhealth.com]
Cardiovascular
- Orthostatic Hypotension
Thioridazine is considered contraindicated for use along with agents that, when combined with a phenothiazine, may prolong the QT interval and increase the risk of TdP, and/or cause orthostatic hypotension. [pdr.net]
Neurologic
- Tremor
[…] do not interfere with the ability to interpret the QT interval in the opinion of the investigator in consultation with the medical monitor Exclusion Criteria: History of meningitis or encephalitis, epilepsy, seizures (known or suspected), migraines, tremors [clinicaltrials.gov]

Blocks
- Right Bundle Branch Block
Complete left or right bundle branch block or incomplete right bundle branch block Resting heart rate < 40 or > 100 beats per minute (bpm) Pathological Q waves (defined as Q wave > 40 msec) Ventricular pre-excitation More than 2 ectopic beats Syncope [clinicaltrials.gov]
Rhythm
- Torsades De Pointes
We report four cases of dofetilide-induced QT prolongation and torsades de pointes. We discuss the risk factors for the development of dofetilide-induced long QT and torsades de pointes and review the current literature. [ncbi.nlm.nih.gov]
Therapy may be initiated at lower dosages due to the risk of torsades de pointes. 1 2 Prescribing Limits Adults Supraventricular Tachyarrhythmias Oral Dosages >500 mcg twice daily have been associated with an increased incidence of torsades de pointes [drugs.com]
T Wave
- Notched T Wave
BACKGROUND: Congenital long QT syndrome type 2 (abnormal hERG potassium channel) patients can develop flat, asymmetric, and notched T waves. Similar observations have been made with a limited number of hERG-blocking drugs. [ncbi.nlm.nih.gov]
Treatment
METHODS AND RESULTS: In 463 congestive heart failure (CHF) patients enrolled in the Danish Investigations Of Arrhythmia and Mortality On Dofetilide-CHF (DIAMOND-CHF) study, both pre-treatment and on-treatment day 2-6 QT dispersion was available from standard [ncbi.nlm.nih.gov]
Of these, 249 were randomized to treatment with dofetilide and 257 to treatment with placebo. Table 1 summarizes baseline characteristics. [ahajournals.org]

Pathophysiology
TdP case reports for these drugs have often been associated with risk factors such as overdosing, concomitant drugs and/or existing pathophysiological conditions. A few cases appear to be devoid of these factors. [ncbi.nlm.nih.gov]
Prevention
CONCLUSIONS: Dofetilide shows promise as an agent for the prevention of atrioventricular re-entrant tachycardia in patients without structural heart disease. [ncbi.nlm.nih.gov]
MAIN OUTCOME MEASURE Responders were defined as patients in whom dofetilide prevented reinduction of atrioventricular re-entrant tachycardia at the end of the infusion. [heart.bmj.com]
