In addition to CD, several missense mutations of Nod2 have been associated with the development of Blau syndrome (BS), a dominantly inherited disease characterized by early-onset arthritis, uveitis, and skin rashes. 3 EOS is another rare but sporadic [bloodjournal.org]

There were a few grouped, tiny, flat-topped scaly papules around the bilateral knee joint and the upper third of both thighs [Figure 1] and [Figure 2]. [ijdvl.com]
Late arthritis is not associated with erythema nodosum but rather with some of the more chronic dermatologic manifestations of sarcoidosis, including plaques, papules, and nodular and scaly lesions. [rheumaknowledgy.com]

- Eruptions
Symptoms of early onset sarcoidosis characterized by skin eruptions, arthritis, and uveitis mimic those of systemic type juvenile rheumatoid arthritis (JRA). [ncbi.nlm.nih.gov]
The skin involvement in juvenile idiopathic arthritis is a transient erythematous macular rash whereas sarcoidosis can present with varied skin eruptions including erythema, papules, plaques and ichthyosiform lesions. [3] As skin lesions may appear before [ijdvl.com]
The most frequent cutaneous eruptions include soft, red to yellowish brown, or violaceous, flat-topped papules, most frequently found on the face. [emedicine.medscape.com]
Lesions can range from non-specific maculopapular eruptions, such as plaques and nodules, to erythema nodosum and lupus pernio. Erythema nodosum is the characteristic lesion of acute sarcoid, which has a good prognosis. [aafp.org]
- Skin Disease
A 10-year-old female was diagnosed as having early-onset sarcoidosis (EOS) after a prolonged skin disease. A granuloma emerged on the face at age 2 and massive lesions extended to the rest of the body. [ncbi.nlm.nih.gov]
Antimalarial agents are used frequently for skin lesions, and they have limited success in the treatment of pulmonary disease. Lung and cardiac transplantation is reserved for end-stage disease. [aafp.org]
Skin This is commonly affected. Sarcoidosis may cause a diverse variety of skin manifestations and may resemble both common and rare cutaneous diseases [ 7 ]. [patient.info]

Skin Test
- Anergy
Patients with systemic sarcoidosis characteristically exhibit peripheral anergy on skin testing due to depression in delayed-type hypersensitivity, but at the target organ site, an active macrophage- and helper T lymphocyte (CD4 + )–driven immunologic [aao.org]
This paradoxic state of simultaneous hyper- and hypoactivity is suggestive of a state of anergy. The anergy may also be responsible for the increased risk of infections and cancer. [en.wikipedia.org]
This paradox is characterized by intense cellular immune response in the affected organs in contrast to a situation of peripheral immunological anergy, observed as a lack of response to PPD and other intradermal tests. [elsevier.es]
Treatment
Involvement of the orbit and posterior segment of the eye in early-onset sarcoidosis may necessitate prompt and aggressive immunosuppressive treatment to prevent a poor visual outcome. [ncbi.nlm.nih.gov]
[…] for further treatment. 5 There are no specific recommendations about when to initiate corticosteroid treatment. [aafp.org]

Prognosis
Early and correct diagnosis is important in light of the severe disease prognosis, and the possible therapeutic options. [ncbi.nlm.nih.gov]
Secondary glaucoma, together with sarcoid uveitis, may be severe and portends a poor prognosis with associated severe visual loss. Posterior segment lesions occur in up to 20% of patients with ocular sarcoidosis. [aao.org]
Early sarcoid arthritis is often self-limited, lasting for days to months, and the prognosis is excellent. [rheumaknowledgy.com]
Genetic factors may also be important in defining the pattern of disease presentation, its severity, and the prognosis. [emedicine.medscape.com]
However, the activity of the disease may fluctuate over time. 2.10 What is the long-term prognosis (predicted outcome and course) of the disease? The available data concerning the prognosis in the long-term is limited. [printo.it]
Etiology
The aim of this study was to assess the efficacy of thalidomide in patients with severe EOS, based on etiology supporting an initial role of NF-kappaB in activation of this disease. [ncbi.nlm.nih.gov]
These findings indicate that the majority of EOS cases reported in Japan shows the genetic etiology of NOD2 mutations that cause constitutive NF-kappaB activation. [jstage.jst.go.jp]
The possibility of genetic etiology has also been explored by Baughman et al. in an expert review [ 4 ]. The focus of the review was an update on sarcoidosis etiology, diagnosis and treatment. [omicsonline.org]
Sarcoidosis is a multisystem granulomatous disorder of unknown etiology with protean systemic and ocular manifestations. [aao.org]

Pathophysiology

The exact mechanism of this relation is not known, but some evidence supports the hypothesis that this is a consequence of Th1 lymphokine prevalence. [55] [82] Tests of delayed cutaneous hypersensitivity have been used to measure progression. [83] Pathophysiology [en.wikipedia.org]
