Presentation
Cystic endosalpingiosis presenting as chronic back pain, a case report. Diagn Pathol 8, 196 (2013). [diagnosticpathology.biomedcentral.com]
Case Presentation A 46-year-old woman presented with intermenstrual bleeding since several months before. She had 2 previous cesareans and no other abdominal or pelvic surgery. [hindawi.com]
RESULTS: Mean age at diagnosis was 43 (range 24-82), of the five patients presenting with pelvic pain, the localization of endosalpingiosis was consistent with the localization of pain in only four (30%) patients. [ncbi.nlm.nih.gov]
The presentation is one of pelvic pain, operative findings are varied, and the clinical course and response is similar to that of endometriosis. [jpagonline.org]

Abdominal Pain
However, in this patient, we followed up 3 months and the first 6 months post-surgery and abdominal pain was not felt anymore. [article.sciencepublishinggroup.com]
No significant difference was seen between patients with (7.3%) and without (7.9%) lower abdominal pain (p 0.7027). Conclusion. [jmig.org]

Macroscopically, though rarely evident, it can appear as multiple small cysts colored in yellowish white (1-2 mm) as shown in Figure 1 and more rarely as large cysts that can be confused as neoplasms. [1, 5] Microscopically it appears glands and tubules [article.sciencepublishinggroup.com]

Common symptoms include, pelvic pain, infertility, menstrual irregularities and dyspareunia. [en.wikipedia.org]
The symptoms complained also depend on the location of the lesion; generally, complained symptoms are chronic pelvic pain, infertility, irregular menstruation, dyspareunia, gross hematuria, gastrointestinal tract obstruction, and jaundice. [article.sciencepublishinggroup.com]
This woman had no history of infertility (with 2 pregnancies at 19 and 21 years old) and denied present or past symptoms of dysmenorrhea or dyspareunia. Her clinical evaluation revealed a polypoid neoplasm, in the posterior vaginal cuff, about 2 cm. [hindawi.com]
The most common symptoms include: acquired dysmenorrhoea, lower abdominal, pelvic and back pain, dyspareunia, irregular bleeding and infertility. Back pain was reported in up to 29% of cases [1] and infertility in up to 30% [10]. [diagnosticpathology.biomedcentral.com]

Vaginal Discharge
Other presenting symptoms included: acyclic pain 4 (36%), cyclic pain I (9%), cyclic and acyclic pain 6 (55%), irregular menses 5 (45%), gastrointestinal symptoms 6 (55%), urinary symptoms 4 (36%), and vaginal discharge I (9%). [jpagonline.org]
History of vaginal discharge and fever denied. She also has history of vaginal delivery in 2004 and never used contraceptives. In the physical examination we obtained vital signs within normal limits, normal nutritional status with IMT of 23.29. [article.sciencepublishinggroup.com]

Adnexal Mass
Preoperative pelvic examination revealed: localized tenderness 5 (45%), diffuse tenderness 3 (27%), and adnexal mass I (9%). [jpagonline.org]
The results of trans-vaginal USG gynecology appears the presence of adnexal masses hypoechoic on the right and left of each measure 7.97 x 3.28 x 7.95 cm and 2.87 cm (figure 3). Figure 3. [article.sciencepublishinggroup.com]
Treatment
Treatment[edit] As endosalpingiosis, generally, is not considered a pathology, treatment is not always necessary. [en.wikipedia.org]
Since the benign nature of the lesion, medical treatment with an oral contraceptive was chosen. Conclusions: Acromegaly appears to be associated with an increased risk of benign and malignant tumors. [endocrine-abstracts.org]
It is also necessary for clinicians to be more professional to avoid mistakes in diagnose and prevent over-treatment. [article.sciencepublishinggroup.com]
Remedial actions may ameliorate or abolish symptoms yet in general the treatment of endometriosis is complicated and protracted. [diagnosticpathology.biomedcentral.com]

Discussion Endosalpingiosis’ origin, risk factors, prognosis, and follow-up recommendations remain unestablished. In the presented case either the embryonic remnant theory, the implantation theory, or the metaplastic theory is valid hypothesis. [hindawi.com]
Etiology
[…] observed that the rate of previous gynecologic and abdominal surgery was 59.1% and a history of tubal disease was documented in 33.6% of cases in their retrospective study, which raises the possibility that peritoneal implantation may be a factor in the etiology [ncbi.nlm.nih.gov]
The etiology and the histogenesis of cystic endosalpingiosis is not clear, but may be related to inflammatory conditions, previous adnexal surgery or peritoneal mesothelial metaplasia depending on the growth factor stimuli or steroid hormones13. [turkjpath.org]

Endosalpingiosis is a benign and rare entity whose pathophysiology remains unknown. It has been described in pelvic organs, the abdomen, or axillar lymph nodes. [hindawi.com]
