Presentation
We present a case of sepsis with Saccharomyces cerevisiae fungemia during treatment with Saccharomyces boulardii. Case: A 61 years old male was admitted to the hospital. [aspergillus.org.uk]
A 5-month-old boy presented to us with fever and cough for 15 days and one episode of generalized tonic clonic convulsion. He had oral thrush at 1 month and otitis media at 2 months of age. Birth weight was 2.9 kg. [indianpediatrics.net]
The authors present a case report of a 79-year-old woman with rheumatoid arthritis, who after a bowel resection developed S boulardii fungemia. [casereports.bmj.com]

Entire Body System
-
Fatigue
Many symptoms may be associated with fungemia, including pain, acute confusion, chronic fatigue, and infections. [en.wikipedia.org]
-
Asymptomatic
However, over half of the affected patients were asymptomatic or unable to communicate, which supports the current practice of routine ophthalmic examinations for patients with fungemia. [iovs.arvojournals.org]
In our series, the most common adverse effect was an elevation of liver enzymes, though it was asymptomatic in all cases. [nature.com]
-
Anemia
Previous studies have reported anemia, probably hemolytic in origin, in adults receiving capsofungin. [nature.com]
The largest previous report was a collated review of 62 cases. 6 Approximately 90% of cases have occurred in patients with acute leukemia or aplastic anemia or bone marrow transplant recipients, and very few cases have been reported in patients who did [jamanetwork.com]

Pleural Effusion
Saccharomyces cerevisiae (also known as brewer’s or baker’s yeast) has been reported as a cause of fungemia, peritonitis, pleural effusion, esophagitis, endocarditis and arthritis in immunocompromised patients. [indianpediatrics.net]

Tachypnea
On presentation, he was lethargic, and had tachycardia and tachypnea. He had oral thrush, diffuse greyish rash all over body, and hepatosplenomegaly. Investigations showed lympho-cytopenia and thrombo-cytpoenia. HIV ELISA was negative. [indianpediatrics.net]
Neurologic
-
Somnolence
The patient presented in shock, with somnolence, exsiccosis, fever, dyspnea, nontender abdomen, vivid bowel sounds. Laboratory values (CrP 299 mg/l, leukocytes 17,9 GPt/l) indicated a severe infection. [aspergillus.org.uk]
Workup
Blood cultures from a central catheter source have a better yield than those from peripheral sites. [3] A positive peripheral blood smear, as seen in this case, is rare. [4] Malassezia fungemia should be considered in the workup of sepsis in patients [medscape.com]

Staphylococcus Aureus
Lavage fluid culture was positive for Staphylococcus aureus (10 4 CFU/mL) and Candida spp. (10 3 CFU/mL). The latter contained two populations that were first identified as C. albicans and a C. parapsilosis. [bmcinfectdis.biomedcentral.com]

aureus in four, S. epidermidis in three, group B Streptococcus in one and polymicrobial bacteremia in one). [nature.com]

Streptococcus Pneumoniae
Urinalysis was negative as were antigenuria for Legionella pneumophila and Streptococcus pneumoniae. Serologies were negative for HIV, HCV, and HTLV-1/2 and positive for anti-HBs antibodies. [bmcinfectdis.biomedcentral.com]

Treatment
After 13 days of treatment, the patient developed fungemia with S boulardii. Treatment with Sacchaflor was immediately discontinued and the patient was successfully treated with amphotericin B. [casereports.bmj.com]
Candidemia persisted in all patients despite treatment with amphotericin B. [nature.com]
Mortality by species and initial antifungal treatment for the 278 patients receiving antifungal treatment DISCUSSION This study has revealed several important issues concerning the epidemiology, diagnostics, and treatment of fungemia, with implications [jcm.asm.org]

Due to intrinsic resistance to echinocandins, poor susceptibility to polyenes, and preferred occurrence in immunocompromised patients, such infections are often of poor prognosis. Yet no consensual therapeutic guidelines are presently available. [dovepress.com]
Multiple logistic regression analysis identified that persistent neutropenia ( P .02) and septic shock ( P .002) were the only significant multiple predictors for a poor prognosis. [jamanetwork.com]
Most patients have involvement of multiple organs and the prognosis is poor, despite optimal treatment. [13] [14] Questions To access free multiple choice questions on this topic, click here. References 1. [ncbi.nlm.nih.gov]
Etiology
Although the detection of the etiologic agents of fungal infection is difficult, it has been shown that blood culture media for mycobacteria are more effective for the detection of fungemia than media for aerobes and anaerobes. [jstage.jst.go.jp]
26 patients with fungemia and cancer treated with chemotherapy (group A) were compared to 25 patients with fungemia and cancer treated with surgery (group B), to assess differences in etiology, risk factors and outcome. [tandfonline.com]
Etiology Candida albicans is the most common cause of candidemia, representing 35% to 60% of isolates. [ncbi.nlm.nih.gov]
[…] of candidemia has been reported as 72 to 290 infections per million, 30 to 66 infections per million in the case of C. neoformans and 12 to 34 infections per million for Aspergillus sp. 7 Over 17 different species of Candida have been identified as etiologic [academic.oup.com]
Rapid detection of the etiological agent causing the infection is of fundamental importance to allow early adjustment of treatment and hence, the reduction of hospital mortality by fungemia. 4 The HC-UFMG is a teaching hospital that offers public and [scielo.br]

Prevention
A genetically very similar subtype, S boulardii, is used in a probioticum (Sacchaflor) to prevent antibiotic-associated diarrhoea and in the treatment of recurrent Clostridium difficile associated diarrhoea. [casereports.bmj.com]

Conclusion: Due to the high relative prevalence of fungal infections in our study, it is necessary to take precautions for fungal infection prevention and choose the best way management to obtain optimal results in these patients. [ FULL TEXT ] [ PDF [advbiores.net]
