Presentation

In this report, we present two cases with all four symptoms of Gerstmann's syndrome each caused by an uncommon lesion in the left frontal lobe without angular gyrus lesion. [omicsonline.org]

Two Cases with Cerebral Infarction in the Left Middle Frontal Lobe Presented as Gerstmann’s Syndrome. J Neurol Disord. 2016;04(07). [redalyc.org]

A case of Gerstmann syndrome following a trauma is presented. After one month the patient showed the four symptoms of the Gerstmann syndrome associated with slight visual memory and constructional praxis deficits. [ncbi.nlm.nih.gov]

Entire Body System

  • Disability

Current research avenues focus on developing techniques to diagnose and treat learning disabilities and increase understanding of the biological basis of them. Information from the National Library of Medicine’s MedlinePlus Learning Disabilities [ninds.nih.gov]

The more frequent and disabling deficit is the syndrome of unilateral spatial neglect that, in a nutshell, consists in a bias of spatial representation and attention ipsilateral to of extrapersonal, personal (ie, the body) space, or both, toward the side [ncbi.nlm.nih.gov]

Two weeks later the patient developed severe headache with intractable vomiting. Repeat MRI head revealed increase in size of the previously detected lesion and three new lesions (Figure 1 H-L). [bmcneurol.biomedcentral.com]

Psychiatrical

  • Suggestibility

He suggested that a common cognitive denominator, or Grundstörung, essentially underlies to these four characteristic cognitive faculties. [omicsonline.org]

Although it has been suggested that in children symptoms may diminish over time, it appears likely that most children probably do not overcome their deficits, but learn to adjust to them. [ninds.nih.gov]

He suggested that it is a disorder of the body schema restricted to the hand and fingers. The existence of a Grundstörung has since been contested. [ncbi.nlm.nih.gov]

Whereas Critchley11 suggested that finger knowledge and calculations were closely related, we found no correlation between these functions, suggesting that they did not share a common network. [jnnp.bmj.com]

  • Dyslexia

The reader will get a review of most aspects of SLD, including the different clinical syndromes (mostly dyslexia and dyscalculia), the clinical manifestations and the therapeutic approaches. [books.google.com]

The subjects were divided into two groups, those with normal-or-better reading ability and those with dyslexia equal in degree to their dyscalculia. [ncbi.nlm.nih.gov]

Frequently, there is also impairment in reading (dyslexia). Children with good intellectual function as well as those with brain damage may be affected. Eliciting features of Gerstmann's syndrome Agraphia: Illegible or very poor writing. [patient.info]

Neurologic

Damage to these posterior parietal regions may also impair visuospatial short-term memory, which may be associated with and worsen spatial neglect. [ncbi.nlm.nih.gov]

  • Dysphasia

Complicating the issue is his obvious nonfluent aphasia (expressive dysphasia) with paraphasic errors (replacing words with associated words (e.g. says 'fork' instead of 'spoon')) and some comprehension issues. [youtube.com]

There was subtle expressive and receptive dysphasia alongside acalculia, agraphia, finger agnosia and left-right disorientation (Additional file 1 ), and right-sided homonymous hemianopia. [bmcneurol.biomedcentral.com]

Also, the syndrome was accompanied by “numerous other neurological deficits, notably dysphasia” and all patients were aphasic. 10 Moreover, Poeck and Orgass observed that the four cardinal symptoms are more frequent in aphasic patients than in non-aphasic [tmj.ro]

Workup

As a result, emergent organic workups should be arranged, especially for elderly patients at high risk for cerebral vascular accident. [dovepress.com]

Workup Evaluation for Gerstmann syndrome begins with confirmation of the clinical findings. Finger agnosia can be tested by: Covering the patient's eyes, touching one of their fingers, and having them report which finger was touched. [eyewiki.aao.org]

Treatment

Treatment There is no cure for Gerstmann's syndrome. Treatment is symptomatic and supportive. Occupational and speech therapies may help diminish the dysgraphia and apraxia. [ninds.nih.gov]

With the right treatment, cases of Gerstmann syndrome can improve and early treatment has been shown to be most effective. [medic8.com]

Prognosis

Prognosis In adults, many of the symptoms diminish over time. [ninds.nih.gov]

Etiology

In the case of neurological assessment, it is important to identify the etiologic cause and the location of the lesion or brain lesions. [lifepersona.com]

While GS is commonly associated with focal lesions to the region of the left angular gyrus, it has also been associated with numerous diffuse etiologies including atrophy, alcoholism, carbon monoxide poisoning, lead intoxication and anaphylactic shock [ncbi.nlm.nih.gov]
