Presentation
The present study reports a human case of cutaneous gnathostomiasis with recurrent migratory nodule and persistent eosinophilia in China. [ncbi.nlm.nih.gov]
Accordingly, the present study reports for the first time a human case of cutaneous gnathostomiasis in Henan Province, China. [parasitol.kr]
Keratic precipitates were present. The condition was diagnosed as irridocyclitis and the usual treatment recommended. [ijo.in]
Entire Body System
Respiratoric
- Cough
Pulmonary: cough, chest pain, dyspnoea or haemoptysis. Patient may cough up worm. Genitourinary: haematuria, urinary retention. Ophthalmological: decreased visual acuity, blindness, photophobia, pain, uveitis, iritis, intraocular haemorrhage. [patient.info]
After admission he developed a non-productive cough with left sided chest pain, a chest x-ray showed a left pleural effusion. Serological findings were positive for Gnathostoma larval antigen but not Fasciola antigen. [ncbi.nlm.nih.gov]
Migration to other tissues can result in cough, blood in the urine, eye involvement and, most seriously, meningitis and encephalitis. People become infected by eating undercooked fish or poultry or drinking water containing the worm's larvae. [medicinenet.com]
Migration to other tissues (visceral larva migrans), can result in cough, hematuria, ocular ( eye ) involvement, meningitis, encephalitis and eosinophilia. [wikidoc.org]
- Hemoptysis
The radiological changes in the chest have been described as resembling tuberculosis (particularly when there has been hemoptysis). Localized but changeable pulmonary consolidation has also been recorded, with areas of segmental atelectasis. [isradiology.org]
Gastrointestinal
- Vomiting
Here we report five adult patients, three women and two men, having an acute episode of vomiting and abdominal pain a few minutes after eating a dish of cebiche prepared from a spotted sleeper perch (Eleotris picta) fished from a nearby lake in southern [ncbi.nlm.nih.gov]
Vomiting and abdominal cramps followed, and settled down over 10 days. When she returned to Melbourne 4 months later, the patient’s blood samples revealed an eosinophilia of 1.6 x 10 9 /L. [mja.com.au]
Initial nonspecific symptoms include fever, malaise, nausea, vomiting, diarrhea, and epigastric pain lasting 2–3 weeks and usually accompanied by a marked eosinophilia. [histopathology-india.net]
This state is called ‘visceral larva migrans’ and manifests with any of these features like fever, cough, vomiting or blood in urine. Eye or ear involvement may seldom be encountered. [surgerydoor.co.uk]
- Nausea
The side effects of albendazole were nausea, dizziness, and an increased alkaline phosphatase. [ncbi.nlm.nih.gov]
Commonly found in countries such as Japan where raw meat is often eaten, gnathostomiasis is a food-borne parasite that can induce symptoms including fever, anorexia, chest pains, nausea, headaches, vomiting, swelling, skin lesions and paralysis. [thewest.com.au]
Following ingestion of viable larvae, patients often develop fever, anorexia, abdominal discomfort, nausea and vomiting as larvae penetrate the gastrointestinal wall. This is usually associated with a marked eosinophilia. [mja.com.au]
- Abdominal Pain
Here we report five adult patients, three women and two men, having an acute episode of vomiting and abdominal pain a few minutes after eating a dish of cebiche prepared from a spotted sleeper perch (Eleotris picta) fished from a nearby lake in southern [ncbi.nlm.nih.gov]
Occasionally the nodule ruptures the stomach wall allowing the stomach contents to leak into the abdominal cavity causing a potentially fatal peritonitis. Dogs with peritonitis have a fever and abdominal pain. [second-opinion-doc.com]
Such symptoms include: Fever and malaise Urticaria Gastrointestinal symptoms: diarrhoea, nausea, vomiting, anorexia and epigastric/right upper quadrant abdominal pain (epigastric or pain) These symptoms may last for 2–3 weeks, as the larva migrates through [dermnetnz.org]
Gastrointestinal disease may be asymptomatic, but is more frequently associated with sharp abdominal pains or inflammatory masses, or may mimic an “acute surgical abdomen”. [mja.com.au]
There will be nausea, salivation, vomiting, flushing, pruritus, urticaria, and upper abdominal pain. At this stage there is a marked eosinophilia, which may reach 90%. [isradiology.org]
- Epigastric Pain
The first symptom noted by the patient was epigastric pain, which occurred 3 days after he had consumed the raw flesh of a brook trout, Oncorhynchus masou masou (Brevoort), from a trout farm in neighbouring Kumamoto prefecture. [ncbi.nlm.nih.gov]
Initial nonspecific symptoms include fever, malaise, nausea, vomiting, diarrhea, and epigastric pain lasting 2–3 weeks and usually accompanied by a marked eosinophilia. [histopathology-india.net]
These initial indicators can include malaise, fever, urticaria, anorexia, nausea, vomiting, diarrhea, and epigastric pain (Rusnak and Lucey 1993). Eosinophilia often develops early on due to larval penetration of the gastric or intestinal walls. [web.stanford.edu]
Clinical features include: General: mild malaise, fever, urticaria, anorexia, nausea, vomiting, diarrhoea and epigastric pain. [patient.info]
- Constipation
The adverse events were malaise (35%), myalgia (30%), drowsiness (30%), pruritus (20%), nausea/vomiting (20%), dizziness (15%), diarrhea (15%), feeling of shortness of breath (10%), feeling of palpitations (10%), constipation (5%), anorexia (5%), and [ncbi.nlm.nih.gov]
Skin
- Larva Migrans
Clinically, the main differential diagnoses includes angiostrongyliasis, trichinosis, and cutaneous larva migrans. [histopathology-india.net]
On the basis of several clinical and therapeutic criteria, larva currens, visceral larva migrans and cutaneous larva migrans were excluded. [moh-it.pure.elsevier.com]
Abstract Gnathostomiasis, or larva migrans profundus, is a significant cause of morbidity in many parts of the world, especially the Far East. [ncbi.nlm.nih.gov]

- Cutaneous Manifestation
The cutaneous manifestations are protean yet often involve intermittent migratory swellings and creeping skin eruptions with abscesses or nodules, which vary in onset and duration. [ncbi.nlm.nih.gov]
Subsequently, symptomatic disease may have either cutaneous or visceral manifestations depending on the larval migration pattern. [mja.com.au]
However, Moore et al. have commented that it could be used as a marker of treatment response in those with eosinophilia at baseline. [12] In the present case, there was no cutaneous manifestation and no eosinophilia. [ijo.in]
However, David et al. have commenced that it could be used as a marker of treatment response in those with eosinophilia at baseline. [7] In the present case, there was no cutaneous manifestation. [tropicalparasitology.org]

BACKGROUND: A finding of antibodies to Gnathostoma spinigerum 24-kDa antigen by immunoblot analysis is currently used to confirm a diagnosis of gnathostomiasis. [ncbi.nlm.nih.gov]
On further inquiring, she recalled having eaten undercooked eels and specific antibodies to the larvae of Gnathostoma spinigerum were detected. [parasitol.kr]
Other Pathologies
- Tissue Nematode
Gnathostomiasis, Gnathostoma spinigerum, G. doloresi, G. nipponicum, G. hispidum, G binucleatum (Pathogen - Tissue Nematode) Organism: Human gnathostomiasis is an emerging food-borne parasitic disease caused by nematodes in the genus Gnathostoma. [med-chem.com]

Treatment
One day after the beginning of the treatment, the migrating swellings in the left upper arm tended to be confined and a papule developed ( Fig. 1C ); subsequent blister emerged on the 9th day of treatment ( Fig. 1D ). [parasitol.kr]
Abstract Comparative treatment of ivermectin in 21 patients (Group 1) and albendazole in 49 patients (Group 2) of gnathostomiasis gave the cure at 95.2% and 93.8% respectively. The ELISA OD and eosinophil counts were reduction after treatment. [ncbi.nlm.nih.gov]
Prognosis
Complications Subarachnoid haemorrhage Pneumonia Sepsis Paralysis Intrauterine transmission Long-term neurological sequelae Prognosis Gnathostomiasis is seldom fatal, except in CNS disease. [patient.info]
(Outcomes/Resolutions) The prognosis of Gnathostomiasis is generally positive with proper treatment. [dovemed.com]
However, the recent case reports indicate that the current prognosis might be better. Of 15 patients in whom neurognathostomiasis was diagnosed after 2001, poor outcomes were reported in 3 (20%) patients. [doi.org]
Etiology
Abstract Subarachnoid hemorrhage (SAH) is a serious neurological condition, commonly of vascular etiology. Gnathostomiasis is a common parasitic disease in Thailand and may also cause SAH. [ncbi.nlm.nih.gov]
Etiology : Gnathostomiasis is a food-born parasite zoonosis causes by a round worm in the genus Gnathostoma e.g. G.spinigirum, G.hispidum. It can cause both cutaneous and visceral symptoms in humans. [tm.mahidol.ac.th]
Nematodes of the genus Gnathostoma (Nematoda: Gnathostomatidae) are the etiological agents of human gnathostomiasis and may also infest dogs, cats, tigers and leopards (3). [med-chem.com]

Epidemiology
However, the epidemiology of this disease in Mexico has never been described in detail. Here we review the current status of gnathostomiasis in Mexico. [ncbi.nlm.nih.gov]

Further studies on the diagnostic techniques and epidemiology of gnathostomiasis seem imperative. [scopemed.org]
Pathophysiology
There were correlations between MRI and electrophysiological findings that confirmed pathophysiology of the disease. Absence of SSEP and MEP response correlated with poor neurological outcomes in radiculomyelitis caused by Gnathostoma spp. infection. [ncbi.nlm.nih.gov]
Pathophysiology of Neurognathostomiasis Gnathostoma spp. nematodes are highly invasive parasites. After being ingested, the third-stage larvae penetrate the mucosal wall of the gastrointestinal tract. [doi.org]
A classic triad that indicates infection is patient complaint of intermittent migratory swelling, predominance of eosinophilia in laboratory tests, and report of travel or residence in gnathostomiasis endemic areas (mainly Southeast Asia). [2] Pathophysiology [emedicine.com]
Prevention
The objectives of this review are to enhance clinician awareness of this infection by describing the behavioral risk factors for its acquisition, life-cycle, clinical manifestations, diagnosis, management, and prevention. [ncbi.nlm.nih.gov]
