Presentation
We present a case of H. parainfluenzae complicating pregnancy in the second trimester. Predisposing factors for infection with Haemophilus species and Australasian data on invasive neonatal Haemophilus infections are presented. [ncbi.nlm.nih.gov]
Nuchal rigidity was not present. Kernig and Brudzinski signs were negative. [journals.sagepub.com]
Five substitutions were present in all the strains [S385T, I442F, V511A, N526K and V562I], while three additional changes [K276N, A307N and V329I] were only present in HUB12435, HUB12445 and HUB12640. [nature.com]
Entire Body System
- Chills
All presented with fever, chills and malaise of less than two weeks' duration. Mitral valve involvement led to congestive heart failure in two of three cases. [ncbi.nlm.nih.gov]
His complaint of neck pain associated with intermittent fever and chills was initially concerning for meningitis; therefore, after obtaining a cranial computed tomography (CT) scan, a lumbar puncture was performed. [journals.sagepub.com]

He denied having any history of intravenous drug abuse, any recent dental work, and did not have any body piercings. [journals.sagepub.com]
- Swelling
Abstract Septic arthritis of the acromioclavicular (AC) joint is a rare entity with symptoms that include erythema, swelling, and tenderness over the AC joint, fever, and limitation of shoulder motion with pain. [ncbi.nlm.nih.gov]
An otherwise healthy 12-year-old boy was seen in our Emergency Department with swelling and pain in his right knee along with limp for the past day. There was no history of trauma or fever. [elsevier.es]
She denied having any rashes, lower extremity swelling, or dyspnoea, and she was without additional gastrointestinal or genitourinary complaints. The patient had no past medical history. [emjreviews.com]
They are gram-negative or gram-variable pleomorphic rods with bulbous swelling of both ends that are characteristically grouped in chains, clusters, or rosettes. [emedicine.medscape.com]
- Malaise
All presented with fever, chills and malaise of less than two weeks' duration. Mitral valve involvement led to congestive heart failure in two of three cases. [ncbi.nlm.nih.gov]
Case report A 41 year old woman presented to her GP with a 10 day history of pyrexia, cough with purulent sputum, and general malaise. [thorax.bmj.com]
He also reported having extreme fatigue and malaise for a few weeks prior to admission. He denied having any history of intravenous drug abuse, any recent dental work, and did not have any body piercings. [journals.sagepub.com]
[…] endocarditis was acute: she was retrospectively questioned for any potential symptoms that would prove a mild, subacute disease form (one that would also be consistent with the vegetations' size), but she did not recall any symptoms apart from ill-defined malaise [jmedicalcasereports.biomedcentral.com]
- Fatigue
He also reported having extreme fatigue and malaise for a few weeks prior to admission. He denied having any history of intravenous drug abuse, any recent dental work, and did not have any body piercings. [journals.sagepub.com]
“Fatigue” was self reported as being unable to go any further. Susceptibility to “fatigue” was associated with a short walk on a flat surface, or with stair walk to the first or second floors. [bmcinfectdis.biomedcentral.com]
More recent research on infective endocarditis has been performed on a difficult clinical case of a pregnant patient who presented with a 104 degree fever, severe headaches, and fatigue. [microbewiki.kenyon.edu]
Respiratoric
- Pharyngitis
Three patients had pneumonia, while the others had epiglottitis with meningitis, pharyngitis, arthritis, and endocarditis, respectively. Nonendocarditic manifestations of adult H parainfluenzae infection were reported in four other cases. [jamanetwork.com]
CONCLUSION: The multiplex PCR assays were very rapid, reliable and feasible methods for detection of H.influenzae and H.parainfluenzae in pharyngeal swab specimens which were compared to conventional isolation and culture methods. 95.5% of H.influenzae [pubag.nal.usda.gov]
Gastrointestinal
- Regurgitation
Echocardiography showed bi-leaflet mitral valve prolapse and severe mitral regurgitation. A small vegetation was seen at the flail anterior valve leaflet. [ncbi.nlm.nih.gov]
Figure 1: A repeat transthoracic echocardiogram revealing severe mitral regurgitation in the parasternal long-axis view (A) and an apical four-chamber view (B). Cardiothoracic surgery was considered. [emjreviews.com]
In our patient, while TTE showed only mitral valve regurgitation, TEE revealed mitral valve perforation and vegetation. [journals.sagepub.com]
Symptoms associated with infective endocarditis include presence of a new regurgitant murmur and feverFootnote 7. The mortality rate of Haemophilus-associated endocarditis is approximately 4-5%Footnote 8,Footnote 9. [canada.ca]
[…] a modern mortality risk closer to 10%-15%. [6] The morbidity of IE caused by the HACEK group is similar to that of other types of endocarditis and includes embolization, local extension into the perivalvular area, congestive heart failure (CHF), and regurgitant [emedicine.medscape.com]

Cardiovascular
- Systolic Murmur
Her blood pressure was 110/80 mm Hg, pulse 106/minute, and at the cardiac apex a discreet systolic murmur could be heard. No further particular events to report from her history. [signavitae.com]
Initially, the systolic murmur heard on exam was not the focus of the diagnostic workup given that his symptoms were highly suggestive of meningitis. Mitral valve perforation and vegetation were eventually discovered via TEE. [journals.sagepub.com]

Psychiatrical
- Suggestibility
These results suggest that the infection of HP in the tonsil may be involved in the etiology of IgAN. [ncbi.nlm.nih.gov]
Whilst he was admitted, he underwent an abdominal CAT scan that reported the presence of findings suggestive of infectious or inflammatory non-specific colitis, affecting segments of the ascending colon. [elsevier.es]

Treatment
This report is unique in that it documents successful treatment with a cephalosporin and what is, to our knowledge, the third reported case of a beta-lactamase producing H. parainfluenzae causing endocarditis. [ncbi.nlm.nih.gov]
Prognosis
In a series of 57 cases of A actinomycetemcomitans endocarditis, poor dentition (46%) and abnormal cardiac valves (60%) were found to be predisposing factors. [12] Prognosis The prognosis is quite variable, depending on many factors, such as delay in [emedicine.medscape.com]
Etiology
Although the facts suggest that this case of urethritis may have been caused by H. parainfluenzae, a possible etiologic role of other microorganisms is not ruled out. [ncbi.nlm.nih.gov]
Overall, these organisms are the leading cause of exacerbations of chronic bronchitis, and the second most common etiology of acute otitis media and sinusitis. [encyclopedia2.thefreedictionary.com]
