Hemangioendothelioma

Presentation

In the present report, we describe a case of PEH in a 39-yr-old woman. Immunohistochemically, the tumor cells were positive for CD34. [pesquisa.bvsalud.org]

The present case, on review and correlation with histomorphology, depicts most of the cytologic features of hepatic EH. [cytojournal.com]

It often presents in infancy with cardiac failure because of extensive arteriovenous shunting within the lesion. [en.wikipedia.org]

Entire Body System

  • Asymptomatic

60% women No predisposing factors; occasional cases reported in vinyl chloride workers, women on oral contraceptives, hepatitis B / C patients, following hepatic trauma Symptoms: abdominal pain, weight loss, hepatic venous outflow obstruction; 40% are asymptomatic [pathologyoutlines.com]

[…] origin of the celiac artery and a decrease in the diameter distally, indicating significant shunting, is seen on angiograms. [16] Because most hemangioendotheliomas in infants sponanteously involute and regress within the first 12 to 18 months of life, asymptomatic [en.wikipedia.org]

Some cases of partial spontaneous regression were also reported in some asymptomatic patients. [ncbi.nlm.nih.gov]

  • Weakness

In addition, few cases of EHE show weak and focal positivity for cytokeratin, so cytokeratin expression does not seem to be specific for epithelial origin. 74 Positive EMA and CD68 have been reported in bone EHE, 9 while a case of prostatic EHE showed [ncbi.nlm.nih.gov]

At the last follow-up (14 months after diagnosis), he is well appearing except for mild abdominal pain. » Discussion EH is a distinctive vascular tumor occurring mostly in soft tissues of extremities and lung. [3] Involvement of liver may be seen as a [cytojournal.com]

Liver, Gall & Pancreas

  • Jaundice

He gave a past history of jaundice 20 months back, which lasted for four weeks with complete resolution thereafter. He was a nonalcoholic, nonsmoker with no comorbid medical/ surgical ailment. [cytojournal.com]

Workup

Therefore, preoperative diagnostic workup is of utmost importance to differentiate it from other primary and metastatic tumors of the liver. [bib.irb.hr]

Biopsy

  • Hepatocellular Carcinoma

The various cytologic differential diagnoses in a smear from hepatic EH would include hepatocellular carcinoma, cholangiocarcinoma, metastatic carcinoma, malignant melanoma and angiosarcoma. [cytojournal.com]

Treatment

•Others like surgery can be done as treatment for the virus during its most severe. [en.wikipedia.org]

Prognosis

Although the metastatic rate in this series was 27%, the prognosis is considered much more favorable than that of other hepatic malignancies. [scienceopen.com]

A standardized treatment algorithm for HEH already does not exist, due to its infrequency and variable prognosis. [eprints.bice.rm.cnr.it]

Weiss in 1996 as "spindle cell hemangioma", rather than hemangioendothelioma, due to the excellent prognosis observed in a group of 78 patients. [5] Retiform hemangioendothelioma (also known as a "Hobnail hemangioendothelioma" [3] ) is a low-grade angiosarcoma [en.wikipedia.org]

Etiology

Epithelioid hemangioendothelioma (EHE) is a rare neoplasm of vascular origin that occurs in the liver and other organs; its etiology is unknown. [scienceopen.com]

The etiology is still a dilemma. Studies about suggestive hypothesis are ongoing. [ncbi.nlm.nih.gov]

Epidemiology

Epidemiology [ edit ] It is so rare that only 0.01 percent of the cancer population has it and it affects about 1 person in every 1,000,000 worldwide. [4] Around 20 cases are diagnosed in America every year – the cause is unknown. [5] It is unresponsive [en.wikipedia.org]

Prevention

Because the lesion may otherwise mimic a hepatic tumor, knowledge of these findings may prevent unnecessary biopsy or inappropriate treatment. Inflammatory pseudotumor of the liver: 13 cases of MRI. [sultowanigh.ml]

When the lesions are small and limited in number, some authors recommend surgical resection or an preventive approach to asymptomatic patients. Successful curative resection achieves good outcomes. [ncbi.nlm.nih.gov]
