Presentation
Patients present with pain, photophobia, and injection. See Clinical Presentation for more detail. [emedicine.medscape.com]
Most commonly Type 1 herpes presents around the lip and around the eye, and it is the eye disease that can be devastating. Presentation is varied and similar to many other corneal conditions. [reviewofophthalmology.com]
In addition, a palpable preauricular lymph node may be present. [eyewiki.org]
Like many viruses, the herpes simplex 1 and varicella- zoster viruses are present in most adults. The viruses in the herpes family usually live around the nerve fibers in humans without ever causing a problem. [webmd.com]

Entire Body System
- Recurrent Infection
A recurrent infection may occur any time between a few weeks and many years after the first active infection. At least half of people who have one episode of active infection will have a recurrence within 10 years of the first. [patient.info]
Although more common as a manifestation of recurrent HSV infection, HSV keratitis may also be seen during a primary HSV infection. (See Workup.) [emedicine.medscape.com]
Skin
- Blister
Infection can be transferred to the eye by touching an active lesion (a cold sore or blister) and then your eye. [aao.org]
Sometimes vesicular blepharitis (blisters on the eyelid) follows, symptoms worsen, vision blurs, and blisters break down and ulcerate, then resolve without scarring in about a week. [merckmanuals.com]
- Flushing
Most commonly, home care consists of flushing the eye with water. See the home care section under eye injuries for techniques on how to flush your eye with water. If you think a foreign body is in your eye, do not rub your eye. [emedicinehealth.com]
In addition, there may be mild conjunctival injection, ciliary flush, mild stromal edema and subpeithelial white blood cell infiltration. [eyewiki.org]
Eyes
- Blepharitis
Sometimes vesicular blepharitis (blisters on the eyelid) follows, symptoms worsen, vision blurs, and blisters break down and ulcerate, then resolve without scarring in about a week. [merckmanuals.com]
- Foreign Body Sensation
If the cornea is involved, early symptoms include foreign body sensation, lacrimation, photophobia, and conjunctival hyperemia. [merckmanuals.com]
Other characteristics of eye herpes include: Swelling around the eyes Tearing Recurrent eye infections Irritation Foreign body sensation Eye redness Eye sores Watery eye discharge Sensitivity to light Due to these numerous symptoms, your eye doctor may [allaboutvision.com]
- Corneal Edema
Characteristic findings include a branching dendritic or serpentine corneal lesion (indicating dendritic keratitis) or disc-shaped, localized corneal edema and haze plus anterior uveitis (indicating disciform keratitis). [merckmanuals.com]
There is usually an immune disease or some element of uveitis or endothelitis with the hallmark elements of corneal edema, clear infiltrates in the cornea, and/or glaucoma. [reviewofophthalmology.com]
- Anterior Uveitis
Disciform keratitis is a deeper, disc-shaped, localized area of secondary corneal edema and haze accompanied by anterior uveitis. This form may cause pain and reversible vision loss. [merckmanuals.com]
- Conjunctival Injection
In addition, there may be mild conjunctival injection, ciliary flush, mild stromal edema and subpeithelial white blood cell infiltration. [eyewiki.org]
Neurologic
- Headache
Sometimes pain originating from the eye is confused with other symptoms, such as a headache, sinus pain, toothache, or a migraine. [emedicinehealth.com]
The problem is likely to be herpes zoster ophthalmicus if your doctor finds some or all of these symptoms: Pain in and around only one eye Headache and fever Redness, rash, or sores on the eyelids and around the eyes, especially on the forehead A rash [webmd.com]

(See Workup.) Patient education For patient education information, see the Eye and Vision Center, as well as Corneal Ulcer. [emedicine.medscape.com]
Treatment
Eye Herpes Treatment Treatment for eye herpes depends on where the infection is located in the eye — in the corneal epithelium, corneal stroma, iris, retina, etc. [allaboutvision.com]
What is the treatment for herpes simplex eye infection? The treatment depends on which part of the eye is affected. [patient.info]
Since most cases of HSV epithelial keratitis resolve spontaneously within three weeks, the rationale for treatment is to minimize damage and scarring. Antiviral therapy, topical or oral, is an effective treatment of ophthalmic herpes infections. [emedicinehealth.com]
Topical corticosteroids are contraindicated in the treatment of active HSV epithelial keratitis. Medical therapy Please see General Treatment. [eyewiki.org]
Treatment for some stages is contraindicated for treatment of other stages. Unfortunately improper treatment is a common occurrence and the patient's condition often worsens quickly. Figure 1. [reviewofophthalmology.com]
Prognosis
The prognosis in HSV keratitis, however, is generally favorable with aggressive treatment. [emedicine.medscape.com]
Prognosis Prognosis is usually good, but greatly varies depending on severity and number of recurrences of the disease. [eyewiki.org]
Kpro-1 is associated with an excellent prognosis for graft retention, acceptably low prevalence of sight-threatening complications, and highly satisfactory visual improvement in eyes with HSV keratopathy, but not in eyes with HZV keratopathy. * Department [journals.lww.com]
What Is the Prognosis of Eye Herpes? Contact lens wear, although not contraindicated in patients with previous HSV keratitis, should be used with caution. [emedicinehealth.com]
Etiology
Etiology and pathogenesis HERPETIC DISEASES OF THE CORNEA (HERPES CORNEAE) Etiology. The disease is a violation of the trophic corneal, apparently, on the ground of damage to the Central nervous system. [lecheniebolezni.com]
(See Pathophysiology, Etiology, and Presentation.) Although more common as a manifestation of recurrent HSV infection, HSV keratitis may also be seen during a primary HSV infection. (See Workup.) [emedicine.medscape.com]
In New England dendritic keratitis appears to be the most frequent corneal infection recognizable on an etiologic or morphologic basis. During the past forty-two months 221 patients with this disease have been observed and treated personally by me. [jamanetwork.com]
Etiology HSV Keratitis is caused by the herpes simplex virus, a double stranded DNA virus made up of an icosahedral shaped capsid surrounding a core of DNA and phosphoproteins of viral chromatin. [eyewiki.org]
Epidemiology
Epidemiology Occurrence in the United States Of adults in the United States, 50-90% have antibodies to HSV-1, indicating previous exposure to the virus. [emedicine.medscape.com]
Pathophysiology
Pathophysiology HSV is a DNA virus that commonly affects humans. Infection occurs by direct contact of skin or mucous membrane with virus-laden lesions or secretions. [emedicine.medscape.com]

If recurrences are frequent or severe, your eye specialist may advise that you take antiviral tablets each day to prevent episodes of active infection. [patient.info]

This will help the eye's natural fluids flow, which prevents the pressure from increasing. Unfortunately, herpetic eye disease can be painful even after several days of treatment and even when the eye is starting to look better. [webmd.com]
