Presentation
We present a case of hypnic headache successfully managed with occipital nerve stimulation. MATERIALS AND METHODS: A 64-year-old female presented with a four-year history of a right occipital headache that regularly awakened her from sleep. [ncbi.nlm.nih.gov]
Entire Body System
- Pain
BACKGROUND: Central facilitation of trigeminal pain processing and deficient habituation was observed in different headache and facial pain disorders. This overactivation seems to be primarily associated with chronic pain states. [ncbi.nlm.nih.gov]
- Swelling
In case it is necessary for women to take multivitamins during Centrum multivitamin side effects comprise symptoms like headaches pain in the bones and joints as well as considerable weight loss. swelling around the ear. [bionexgen-fp7.eu]
The location of the pain is around or behind one eye and there is usually swelling, excessive tearing or redness. There may also be nasal congestion and agitation. [thechart.blogs.cnn.com]
Temporal Arteritis Headaches are the most common symptom of temporal (giant cell) arteritis, or TA, a disease that causes your arteries -- blood vessels that carry oxygen from your heart to the rest of your body -- to swell and narrow. [webmd.com]
- Fatigue
You have signs of dehydration for example dizziness decreased urination (no urination at least every 8 hours) or severe fatigue. [bionexgen-fp7.eu]
[…] underlying disease; is associated with local tenderness and/or exacerbates on jaw movement. [4] SRB is characterized by frequent or regular tooth-grinding sounds during sleep with at least one of the following: (i) Abnormal wear of the teeth; (ii) jaw muscle fatigue [doi.org]
- Excessive Daytime Sleepiness
daytime sleepiness that was found to be suffering from SRB. [doi.org]
Respiratoric
- Snoring
To establish if you have hypnic headache, you will need to see your doctor who will need to take a full history of your sleep patterns and night time habits (including snoring and restlessness). [migrainetrust.org]

Gastrointestinal
- Nausea
No autonomic symptoms and no more than nausea, photophobia, or phonophobia are present. We report three children between 7 and 11 years old with HH features. [ncbi.nlm.nih.gov]
Anti-nausea medication. Dehydration can affect your blood pressure leading to symptoms of dizziness. [bionexgen-fp7.eu]
Symptoms include only ONE of nausea, photophobia (sensitivity to light), and phonophobia (sensitivity to sound). Usually occurs first after the age of 50, although more data shows the age of onset to usually be between the ages of 40 and 82. [helpforheadaches.com]
- Choking
Another is that the vascular constriction that happens during migraine episodes doesn't choke off cerebral blood flow enough to cause aura symptoms. [flipper.diff.org]
Cardiovascular
- Hypertension
We suggest that 24 h ambulatory blood pressure monitoring may be included in the evaluation of nocturnal headache complaints especially in the elderly, in whom essential hypertension is a very frequent comorbidity. [ncbi.nlm.nih.gov]
As always, a new onset of headache in the elderly may be due to other causes and should be worked up to rule out hypertension and the rare possibility of tumors in the brain. [headache-adviser.com]
Other possible causes of headache developing during and causing wakening from sleep should be ruled out, with particular attention given to sleep apnoea, nocturnal hypertension, hypoglycaemia and medication overuse; intracranial disorders must also be [ichd-3.org]
• As with all primary headache disorders, the diagnosis is predicated on exclusion of secondary causes (posterior fossa and pituitary lesions, nocturnal hypertension) and other mimics. • Evidence suggests that there may be an associated decrease in [medlink.com]

Musculoskeletal
- Restless Legs Syndrome
Insomnia, restless legs syndrome (now called Ekbom isease), periodic limb movements, circadian rhythm disorders have all been associated with increased prevalence of headache as is insufficient sleep due to lifestyle choices. [thechart.blogs.cnn.com]

Eyes
- Photophobia
No patient had autonomic manifestations, 70% had phonophobia, 50% had photophobia, and 50% had both. All of them (100%) had a VAS score equal to or higher than 8. [ncbi.nlm.nih.gov]
Five of our patients (50%) reported associated phonophobia and photophobia. [elsevier.es]

Abstract Hypnic headache is a rare short-lasting headache occurring exclusively during sleep in the elderly population. [ncbi.nlm.nih.gov]

In an older person, new onset headaches may be the result of a serious disorder such as a brain tumor, stroke, or inflammation of the arteries of the head (a condition called giant cell arteritis). They therefore require extensive investigation. [headaches.org]

- Agitation
There may also be nasal congestion and agitation. Hypnic headache is a newly described headache syndrome that has been reported only in people older than 50. [thechart.blogs.cnn.com]
[…] asked about behaviour during a Hypnic Headache we found a total of 197 reported behaviours from our 80 self reported cases: 43 took painkillers, 20 stayed lying in bed, 35 rubbed their head or neck, 54 reported having to get up from bed, 12 reported agitation [severe-headache-expert.com]
Workup
We reviewed the literature to evaluate the usual diagnostic workup performed in hypnic headache patients. [ncbi.nlm.nih.gov]
Treatment
If lithium treatment does not provide a significant response, indomethacin can be commenced as second-line approach. If these treatments prove to be ineffective or poorly tolerated, other agents, such as caffeine and melatonin, can be administered. [ncbi.nlm.nih.gov]
Prognosis
We describe results of an internet based survey to identify people with Hypnic Headache and seek their descriptions of headache symptoms, treatment options, prognosis and theories on causation. [severe-headache-expert.com]

Etiology
Certain conditions have both an underlying etiology and multiple body system manifestations due to the underlying etiology. [icd10coded.com]

Pathophysiology
METHOD: Current studies that deal with the pathophysiology of HH are summarized. [ncbi.nlm.nih.gov]
Prevention
As for prevention, a wide variety of medications were reported to be of benefit in HH. The drugs that were found to be effective in at least five cases are: lithium, indomethacin, caffeine and flunarizine. [ncbi.nlm.nih.gov]
Some alarm clock headache sufferers have been prescribed Topamax (topiramate) to prevent these headaches. Anti-inflammatory drugs may also be recommended for preventing or treating them upon waking. [migrainerelief.com]
