Presentation
We report a case of a KS in homosexual man with AIDS that was originally presented and diagnosed as UC. We report a 31-year-old homosexual man who presented with rectal pain and bleeding and was found to have Kaposi sarcoma in both skin and rectum. [omicsonline.org]
No evidence of non-Hodgkin lymphoma was present on lymph node or bone marrow biopsies. [ncbi.nlm.nih.gov]
At physical examination he presented with an excellent general state, without skin lesions or palpable adenopathies. Rectal digital examination detected an irregular and painful rectal mass with rough consistence. [scielo.isciii.es]
Entire Body System
- Pain
Despite treatment with HAART, he developed diffuse cutaneous, confluent and nodular red changes in the back, which were 1 cm in diameter, accompanied with pain and itching. During the last four months, he experienced rectal pain and bleeding. [omicsonline.org]
We report a case of a 24-year-old homosexual man who presented with rectal pain and bleeding and was found to have skin-sparing, disseminated Kaposi’s sarcoma with rectum, liver, lungs, and lymph node involvement. [ncbi.nlm.nih.gov]

Respiratoric
- Pleural Effusion
Despite aggressive measures, his condition continued to deteriorate, leading to pulmonary edema and pleural effusion requiring mechanical ventilation, multi-factorial kidney failure requiring dialysis, anemia, and coagulopathy from worsening liver failure [ncbi.nlm.nih.gov]
Gastrointestinal
- Rectal Pain

- Rectal Bleeding
Leiria, Portugal Case report A 40-year old male patient, professor, without previous relevant medical history searched medical assistance for rectal bleeding for the previous two weeks associated with proctalgia. [scielo.isciii.es]
- Abdominal Pain
Like in our case, the patients were presented with bloody or watery diarrhea, abdominal pain and weight loss. The majority of cases of gastrointestinal KS (GI-KS) is asymptomatic, and for this reason, remains undiagnosed. [omicsonline.org]
Workup
Proctoscopy
- Rectal Ulcer
Colonoscopy revealed erythematous rectal mucosa with hemorrhagic nodules and a 3-cm friable necrotic ulcer in the rectum (Figure 1). [ncbi.nlm.nih.gov]
Pleura
- Pleural Effusion
Despite aggressive measures, his condition continued to deteriorate, leading to pulmonary edema and pleural effusion requiring mechanical ventilation, multi-factorial kidney failure requiring dialysis, anemia, and coagulopathy from worsening liver failure [ncbi.nlm.nih.gov]
Treatment
Cases with widespread visceral involvement are commonly poorly responsive to treatment. UC is a chronic inflammatory bowel disease that affects large bowel, causing inflammation and ulcers. [omicsonline.org]

Five patients with advanced AIDS received no specific treatment for anorectal KS. Follow-up ranged from 1 month to 5 years. [pubmed.ncbi.nlm.nih.gov]

KS can have four epidemiological variants: classic, African or endemic, iatrogenic or related to immunosuppressive treatment, and epidemic or AIDS-related [11]. HHV-8 is the main etiological agent of the four clinical-epidemiological forms of KS. [omicsonline.org]
