Presentation
The Late presenting group experienced acute otitis media several weeks prior to presentation and in this group neurologic symptoms dominated the clinical picture at admission. All patients received antibiotics. [ncbi.nlm.nih.gov]
Entire Body System
- Swinging Fever
The third group comprises all causes of inflammation, infection, or damage to the sinus wall. [18] The classic presentation of cerebral venous sinus thrombosis is severe illness associated with typically high and swinging fever. [sjmms.net]
Respiratoric
- Aspiration
Because pus was not aspirated via punctures of the other two cases, their sinuses were not explored. Only radical mastoidectomy was performed surgically for these cases. [ncbi.nlm.nih.gov]
- Respiratory Distress
There were no mortalities; morbidities included anacusis, acute respiratory distress syndrome, reoperation, seizures, septic cardiomyopathy, transfusion, ventriculoperitoneal shunt and nutritional supplementation. [ncbi.nlm.nih.gov]
Morbidity, related to LST, is approximately 30% and is as sociated withseptic cardiomyopathy, acute respiratory distress syndrome, anacusis and seizures 3. [yumpu.com]
She was referred to our unit due to respiratory distress. The physical exam showed signs of septicemia, requiring orotracheal intubation. Otoscopy revealed a yellowish fetid secretion in the right ear. [oldfiles.bjorl.org]
Skin
- Vesicular Rash
Zoster sine herpete (ZSH) is an uncommon manifestation of VZV infection and presents with similar symptoms but without the vesicular rash. [ncbi.nlm.nih.gov]
Ears
- Otalgia
The patient presented with otorrhoea, otalgia, neck pain, fever and chills. Cortical mastoidectomy was performed. Intravenous antibiotics and heparin were administered. The patient had a complete recovery with no sequelae. [ncbi.nlm.nih.gov]
- Ear Discharge
They presented with low-grade fever, headache, nausea, vomiting and ear discharge. One patient had facial nerve palsy. CT scan was helpful in managing these patients. They were treated with antibiotics followed by surgery. [ncbi.nlm.nih.gov]
Examination showed slight discharge with an inflamed right ear drum and no neurological signs apart from neck stiffness. A CT scan revealed right transverse sinus thrombosis. He was treated with intravenous antibiotics and heparin. [123doc.com]
A haemogram, a routine urine study and the culture and sensitivity (c/s) of the ear discharge was done in all the cases. [jcdr.net]
- Postauricular Pain
A 55-year-old woman presented with a 3-day history of left temporal and postauricular pain, nausea, vomiting, and mild photophobia. She denied otalgia, otorrhea, and hearing loss. [ncbi.nlm.nih.gov]
Neurologic
- Dysphasia
Dysphasia was the most common one (8 patients). [ncbi.nlm.nih.gov]
The main focal sign was dysphasia due to a left temporal ischemic or hemorrhagic lesion secondary to thrombosis of the vein of Labbé, which drains in LS. Its frequency (13%) was the same as in other CVT. [stroke.ahajournals.org]
- Hyperesthesia
Examination revealed hyperesthesia in the left C2 nerve root distribution without evidence of herpetic rash. [ncbi.nlm.nih.gov]
Workup
All children underwent laboratory workup for hypercoagulability. All the five children were found to be heterozygote for the C677T MTHFR mutation and a child presented also heterozygosity for factor V Leiden mutation. [ncbi.nlm.nih.gov]

Symptoms and signs, presence or absence of papilledema, modes of onset, neuroimaging findings, causes and risk factors, including a thrombophilic workup, and outcome at discharge and at 1 year were systematically recorded. [stroke.ahajournals.org]
Treatment
Intracranial complications in children resulted from unsuccessful treatment of AOM, which led to acute mastoiditis and lateral sinus thrombosis. [ncbi.nlm.nih.gov]
Prognosis
The prognosis of LST is good if treatment is instituted early with broad-spectrum intravenous antibiotics and surgery. The role of clot removal at surgery and the use of anticoagulation are discussed in this paper. [ncbi.nlm.nih.gov]

Complications include seizures, hydrocephalus, intracranial hypertension and neurological deterioration. [ 10 ] Prognosis Long-term follow-up suggests a generally good prognosis but there may be residual pyramidal symptoms, epilepsy, visual field defects [patient.info]
Etiology
Etiologic investigation lead to the diagnosis of left lateral sinus thrombosis. Anticoagulation was started with heparin followed by dicumarinics. This treatment was maintained during eight months. [actamedicaportuguesa.com]
OBJECTIVE: Our review aims: (1) to highlight the suspicion of lateral sinus thrombosis (LST) following a minor head injury in the pediatric population; (2) to discuss the different etiologies of lateral sinus thrombosis; (3) to identify an exact mechanism [ncbi.nlm.nih.gov]
Etiology :1. Associated lateral sinus thrombosis obstruction of cerebral venous return. 2. Superior sagittal sinus thrombosis ed C.S.F. absorption Both of these factors result in raised ICP 16. Clinical features • Symptoms 1. [slideshare.net]
In 183 short chapters, the book provides the essentials clinicians need on symptoms/signs, diagnostic tests, and neurologic disorders of all etiologies. For this edition, Timothy A. [books.google.com]

Please refer to the article on cerebral venous thrombosis for a broad discussion on epidemiology and risk factors. Presentation is variable and can range from asymptomatic to coma and death. [radiopaedia.org]
Epidemiology Intracranial venous thrombosis appears to have become more common in recent years. However, this has occurred as modern imaging techniques have improved diagnosis. [patient.info]
Pathophysiology
To describe a case of lateral sinus insufficiency, presumably occurring just prior to lateral sinus thrombosis (LST), and to discuss the importance of early surgical intervention and the pathophysiology of full-blown LST. [ncbi.nlm.nih.gov]
Further inflammation of outer and inner walls of sinus results in the formation of mural thrombosis. [3] In our case, the pathophysiology was osteothrombophlebitis of small venules as evident from radiology of temporal bone (HRCT temporal bone). [indianjotol.org]

Prevention
Anticoagulants have been advocated to prevent extension of the thrombus to the distal sinuses. [emedicine.medscape.com]
Anticoagulation has been advocated to prevent extension of the thrombus to distal sinuses(27,28). However, it is rarely used now as most infections can be controlled with antibiotics and surgery, and this tends to prevent thrombus propagating. [arquivosdeorl.org.br]
This prevents blood from draining out of the brain. As a result, blood cells may break and leak blood into the brain tissues, forming a hemorrhage. This chain of events is part of a stroke that can occur in adults and children. [hopkinsmedicine.org]
Recently, anti-coagulant treatment has been proposed to prevent complications associated with thrombus persistence or its possible propagation 11. [ncbi.nlm.nih.gov]
