Presentation
Abstract We present the case of a 47-year-old man who presented with asymptomatic ischemic cardiomyopathy. [ncbi.nlm.nih.gov]

Gagging
This enzyme is involved in the degradation of the glycosaminoglycans (GAGs) dermatan and heparan sulphate and its deficiency results in the accumulation of GAGs and a progressive multisystem disease. [ncbi.nlm.nih.gov]

Heart Disease
Duchenne muscular dystrophy (DMD) leads to structural heart disease, including dilated cardiomyopathy, in 90% of patients 18 years of age. [ncbi.nlm.nih.gov]

Though LVA occasionally occurs in congenital, infective, connective tissue and traumatic heart disease, myocardial infarction remains the most frequent cause. [slideshare.net]

Fetus

Hydrops Fetalis
Symptomatic patients presented with arrhythmias (LVA 16% vs LVD 0%), hydrops fetalis (LVA 27% vs LVD 18.8%), or rupture (LVA 4% vs LVD 38%; p 0.02). Size changes of the LVA/LVD during pregnancy were reported in 17%. [ncbi.nlm.nih.gov]

Skin
-
Skin Edema
A 22-week fetus presented with a large left ventricular aneurysm, 24 21 18 mm in size, detected by abnormal four-chamber view, and severe fetal hydrops with pericardial effusion, ascites and skin edema. [ncbi.nlm.nih.gov]

Suggestibility
M-mode or two-dimensional echocardiograms showed intact posterior wall function in all patients, suggesting severe regional myocardial disease rather than global dysfunction. [jhu.pure.elsevier.com]
Because symptoms and signs can vary in patients with ventricular aneurysm, we strongly suggest a close clinical follow-up, preferably with chest X-ray or echocardiography, for patients experiencing a blunt chest trauma. [ncbi.nlm.nih.gov]
Their presence may only suggest the presence of an LVA. Absence of the above ECG abnormalities does not exclude the presence of LVA. [metealpaslan.com]
Workup
The workup of the fetus at 26 weeks' gestation led to the identification of a left ventricle aneurysm. Initially, the value of ICT of the left ventricle indicated adequate cardiac function. [ncbi.nlm.nih.gov]
Angiography
-
Mural Thrombus
A calcified left ventricular aneurysm with mural thrombus was confirmed with cardiac MRI and a CT scan. [ncbi.nlm.nih.gov]
Mural thrombus and subsequent embolisation. Causes The following conditions may cause an LV aneurysm: Acute myocardial infarction (by far the most common). Cardiomyopathy. Cardiac infection. Congenital abnormalities. [lifeinthefastlane.com]
ECHOcardiography showed a very depressed left ventricle with an ejection fraction of about 15%, apical aneurysm and mural thrombus. Anterior leads show Q waves and domed ST segment elevation. Leads V4 to V6 also show negative T waves. [metealpaslan.com]
X-Ray

Mediastinal Mass
Most likely vascular in origin, however mediastinal masses could not be ruled out. Case Report Figure 2: Organised thrombus removed from aneurysmal sac. A 63-year-old female presented with a large inferior myocardial infarction. [ctsnet.org]
Treatment
Treatment should be individually tailored depending on clinical severity, and treatment options are limited. [ncbi.nlm.nih.gov]
Prognosis
Given that this combination of congenital LV aneurysm and LVNC is significantly associated with poor prognosis, it appears that patients with congenital LV aneurysm and LVNC are candidates for early, aggressive intervention, including surgical aneurysmectomy [ncbi.nlm.nih.gov]
In our case, LVA/LV ratio was 1 throughout the pregnancy that suggested poor prognosis ( Figure 2a ). With the relative enlargement of aneurysm, we expected poor prognosis of this fetus. [nature.com]

Pathophysiology
The increasing use of noninvasive techniques has allowed earlier recognition and better appreciation of LVA genesis and pathophysiology. Improvements in surgical anesthesia and techniques have resulted in more successful LVA surgery. [ncbi.nlm.nih.gov]

Optimal physiologic management may differ among all the existing pathophysiologies, making the anesthetic management challenging. Intraoperative transesophageal echocardiography (TEE) is essential for several reasons. [clinicalpainadvisor.com]
Prevention
Once diagnosis was made by chest X-ray, echocardiography and computed tomography, an open heart surgery was necessary to prevent complications. [ncbi.nlm.nih.gov]
