Lipodermatosclerosis

Presentation

The clinical presentation of cutaneous sarcoidosis is highly variable. Rare presentations include ulcerated plaques, morpheaform lesions, and unilateral lower extremity edema. [ncbi.nlm.nih.gov]

Methods: This was a retrospective study of patients presenting to Mayo Clinic between 1976 and 1998 with a diagnosis of lipodermatosclerosis. Results: Of 97 patients, 84 (87%) were women. Mean age was 62 years (range, 25-88 years). [mayoclinic.pure.elsevier.com]

Other clinical features of chronic venous insufficiency may be present, including hyperpigmentation of the skin from haemosiderin deposition, atrophie blanche, varicose veins, venous eczema, and venous ulcers. [dermnetnz.org]

Skin

  • Ulcer

Pentoxifylline has been shown to be useful in venous ulcers in conjunction with compression or in patients who cannot tolerate compression. [dermnetnz.org]

Rare presentations include ulcerated plaques, morpheaform lesions, and unilateral lower extremity edema. [ncbi.nlm.nih.gov]

Fibrotic tissue may predispose the tissue to ulceration. Recurrent ulceration and fat necrosis is associated with lipodermatosclerosis. [en.wikipedia.org]

Conclusions: We suggest that the severe morphological changes in LDS and ulcer tissue play an important role in the pathogenesis of venous ulceration. 1. Falanga, V, McKenzie, A, Eaglstein, WH. Heterogeneity in oxygen diffusion around venous ulcers. [journals.sagepub.com]

Treatment

Surgical treatment Treatment of underlying venous insufficiency can improve symptoms and may reduce the risk of ulcer recurrence. Referral to a vascular surgeon should be considered. [dermnetnz.org]

Cutaneous reactions due to gemcitabine treatment, including alopecia and maculopapular eruption, are well known. [ncbi.nlm.nih.gov]

Etiology

Etiology: Progressive venous disease Pathophysiology Knowledge is evolving, but incomplete. The problem begins with venous hypertension and extravasation of fluid from veins, and end result is skin hardening and fibrosis. [infectiousdiseaseadvisor.com]

Prevention

(Effects of compression therapy: prevention of extravasation of fluid into extravascular spaces not only as a mechanical aid, but also through decreasing vascular permeability by tightening the paracellular barrier) Campbell, L, Miller, OF. [infectiousdiseaseadvisor.com]
