Presentation

Testicle exploration shows a mass, and ultrasonography is very helpful for screening of hematocele, but presents some difficulties to its final diagnosis (2). [scielo.br]

We report a 39-year-old male who presented non-traumatic testicular swelling and pain. Physical examination and sonography presented a suspicion of testicular tumor and both surgical exploration and inguinal orchiectomy were performed. [ncbi.nlm.nih.gov]

Entire Body System

  • Inflammation

The disease should be considered even in the absence of a particular cause such as injury and inflammation of scrotal content. [ncbi.nlm.nih.gov]

In such cases there have probably been prior adhesive inflammations which have obliterated or closed these parts. [homeoint.org]

Scrotal Pearls Caused by.... ..... epididymitis torsion, torsion of the appendix testis, inflammation, or a chronic hydrocele. [proprofs.com]

It can also be seen after inflammation, infection or torsion. Blood in the tunica vaginalis can be seen after trauma, tumor rupture or surgery. A pyocele is usually associated with infection, likely from a non-treated epididymo-orchitis. Fig 3 – a. [med-ed.virginia.edu]

  • Fever

He had no history of trauma of scrotum, fever, pain or dysuria. Physical examination revealed a tumor larger than a fist in the left scrotum. Ultrasonography revealed an echogenic mass with echolucent area in the scrotum. [ncbi.nlm.nih.gov]

Orchitis May have..... .... scrotal swelling and pain, fever, and nausea and vomiting. [proprofs.com]

If this is all we see and the child does not have fever or elevated white count, which can be seen in cellulitis, than we can make the diagnosis of Idiopathic scrotal edema. [radiologyassistant.nl]

  • Collapse

Or, again, a first hemorrhage may be followed by a second, with a more profound collapse, and speedy death. [homeoint.org]

Collapse may be met with stimulants such as Ether, Alcohol, or Sal Volatile. Opium is the only reliable hz'mostatic and restorative in such cases, and in the presence of great pain it may he given fearlessly. [gluedideas.com]

  • Anemia

  • Plethora

Purpura, scorbutis, chlorosis, plethora and anaemia are the blood states which may be said to predispose to hematocele. Chronic ovarian disease, pelvic peritonitis, and the hemorrhagic diathesis may also be included in the predisposing causes. [homeoint.org]

Respiratoric

  • Aspiration

Most authors, however, favor non-interference, believing that experience has shown that nature does better without any surgical aid, whether the latter be by abdominal section, by puncture with the bistoury, or by aspiration. [homeoint.org]

Gastrointestinal

  • Regurgitation

Hemorrhagic peritonitis. " It is then clear," says Thomas, "that the mere presence of a large clot of blood in the pelvis, apart from general symptoms, is a matter of very doubtful significance, since on the one hand it may be the result of a mere regurgitation [homeoint.org]

  • Sweating

[…] suddenly of an intense, excruciating pain about the region of the pelvis ; this is soon accompanied by more or less vomiting of bile and rapidly followed by symptoms of shock and collapse, pallor, faintness and exhaustion, coldness of the extremities, cold sweat [homeoint.org]

Neurologic

  • Excitement

The exciting causes are sudden checking of the menstrual flow ; violent exercise during menstruation ; violent coitus, especially during menstruation ; blows or falls ; obstruction of the cervical canal ; obstruction of the oviducts. [homeoint.org]

Urogenital

  • Scrotal Mass

Because hematocele may clinically and sonographically resemble a testicular tumor, the proper management of a complex multiseptated scrotal mass without obvious history suggestive of hematocele is surgical exploration and orchiectomy. [ncbi.nlm.nih.gov]

Results and discussion: Chronic scrotal hematocele presented as slowly progressing scrotal mass, differentiation from testicular neoplasm is difficult and, in most cases, only possible after surgical removal of the mass. [medcraveonline.com]

  • Testicular Mass

Intra-testicular Mass Most intra-testicular malignant masses.... ..... are hypoechoic Intra-testicular Mass Almost all.... ...benign intra-testicular masses are echogenic. [proprofs.com]

Scrotal examination revealed an enlarging hard, nontender mass in the right side. Testicular tumor markers were within normal range. [scielo.br]

  • Dysuria

He had no history of trauma of scrotum, fever, pain or dysuria. Physical examination revealed a tumor larger than a fist in the left scrotum. Ultrasonography revealed an echogenic mass with echolucent area in the scrotum. [ncbi.nlm.nih.gov]

Other Pathologies

  • Lymphocytic Infiltrate

Reactive lymphocyte infiltration and severe hemosiderin deposition were seen in the broken-down wall where tissue organization was processing (Fig. 3). [ncbi.nlm.nih.gov]

Treatment

The best routine treatment is to open freely the tunica as in the modern operation for the radical cure of hydrocele. [gluedideas.com]

Upon diagnosis proper treatment can be given. What is the recommended treatment for a small bleeding hole on the scrotum? It is not uncommon for a hematocele in this area to have ruptured. This is a collection of blood around the testicle. [justanswer.com]

Treatment: It is important to discuss your condition with your physician and talk with him/her about a treatment plan. Hematocele benefit from a holistic treatment approach. [medigoo.com]

The presence of a hematocele requires early surgical treatment. [ncbi.nlm.nih.gov]

Prognosis

Lymphoma Prognosis poor survival Lymphoma Symptoms Painless testicular mass Lymphoma Sonographic appearance Enlargement of testis with a uniform decreased echogenicity. [proprofs.com]

When the extravasation is slight, or is well encapsulated, or is extra-peritoneal in character, the prognosis is favorable. [homeoint.org]

Etiology

The etiology of the hematocele formation in our patient is unknown. Hematocele is difficult to diagnose preoperatively because its symptoms may mimic cysts or neoplasms. [scielo.br]

Signs and Symptoms of Testicular Trauma Testicular pain, scrotal hematoma, soft tissue injuries depending on the etiology of testicular trauma. [urology-textbook.com]

The etiology of hematocele in our case was a previous perineal trauma. [ispub.com]

Epidemiology

Epidemiology Frequency Testicular trauma is relatively uncommon. Blunt trauma accounts for approximately 85% of cases, and penetrating trauma accounts for 15%. [emedicine.medscape.com]

Pathophysiology

Pathophysiology The testis is enveloped by layers of white fibrous connective tissue called the tunica vaginalis and the tunica albuginea. [emedicine.medscape.com]

Prevention

The area should be kept clean and antiseptic creams can be used to prevent infection. A doctor can perform an examination to give definitive diagnosis. If it is a hematocele, measures can be taking to prevent it from forming again. [justanswer.com]

To prevent recurrence following surgical drainage, a drain may be left at the surgical site. Coelom [en.wikipedia.org]

High index of suspicion, especially in slowly growing scrotal mass in men older than 50 years, is required to diagnose this pathology and prevent unnecessary orchiectomy. [medcraveonline.com]

Correct management includes early recognition and complete evacuation of the hematoma to avoid testicular compression (2), preventing epididymo-orchitis, abscess formation and necrosis, as unresorbed hematocele can eventually be infected. [scielo.br]
