Presentation
Abstract To differentiate the skin-dwelling filariae Mansonella streptocerca and Onchocerca volvulus, a nested polymerase chain reaction (PCR) assay was developed from small amounts of parasite material present in skin biopsies. [ncbi.nlm.nih.gov]

Swelling
Cutaneous symptoms include localized swellings, angioedema, and pruritus. Diagnosis is by surgical extraction of the adult parasite and confirmation of species by morphology. The disease is endemic to Central and West Africa. [visualdx.com]
Swelling are similar to Calabar swellings due to L. loa nematodes. Calabar swellings are episodic angioedema of the arms and legs due to immune reactions. They are 3–10 cm, non erythematous, and non pitting. [pusware.com]
Adult forms migrate through subcutaneous tissues resulting in intermittent swellings called Calabar swellings, and also migrate beneath the eye conjunctiva (Eye worm). Microfilariae circulate in the bloodstream during the day. [the-travel-doctor.com]
Symptoms include pruritus, headache, and articular swelling. [icd10data.com]

Inflammation
Remarkable was the invasion of young, L1 protein-positive macrophages and the release of neutrophil defensin as signs of acute inflammation. We conclude that ivermectin has a strong microfilaricidal activity against M. streptocerca. [ncbi.nlm.nih.gov]
The adults live in the lymphatics and cause inflammation and scarring of the lymph vessels. [mayomedicallaboratories.com]
Most clinical manifestations associated with O volvulus infection are related to the chronic effects of repeated episodes of inflammation. [emedicine.medscape.com]

An itching acute or more often chronic papular dermatitis, predominantly on the upper parts of the body, was the typical skin lesion observed in 24% of 177 M. streptocerca mf carriers. [ncbi.nlm.nih.gov]
Dermatitis and ocular lesions are commonly associated with the infection and in long standing cases there is blindness, hence the name river blindness for those who are close to rivers in which the vectors are breeding. [cehjournal.org]

Pruritus
Cutaneous symptoms include papular dermatitis, pruritus, lichenification, atrophy, and a "leopard-skin" depigmentation. [visualdx.com]
Common adverse effects were increased pruritus and acute papular dermatitis in 45% of 86 mf carriers on day 6 after treatment. No serious adverse side-effects were noticed in about 700 treated persons. [ncbi.nlm.nih.gov]

Aside from the onchocercoma, the five classic skin eruptions associated with onchocerciasis are as follows: Acute papular onchodermatitis Chronic papular onchodermatitis Lichenified onchodermatitis Skin atrophy Skin depigmentation. [emedicine.medscape.com]

Skin Lesion

In Latin America, a variety of more acute skin lesions occurs, while chronic skin lesions are less common. [emedicine.medscape.com]

Papule
Acute papular onchodermatitis consists of pruritic papules 1-3 mm in diameter that may progress into vesicles or pustules. Chronic papular onchodermatitis consists of flat-topped, hyperpigmented, pruritic papules typically 3-9 mm in diameter. [emedicine.medscape.com]
Skin changes range from papules to lichenification to depigmentation and finally, to “hanging groin” (a pouch of swollen lymph tissue). Eye damage includes corneal injury (sclerosing keratitis), uveitis, and chorioretinitis. [the-travel-doctor.com]
