Presentation
We hereby present a case of a 40-year-old man with such a finding who presented with chest pain and facial congestion. Evaluation revealed a mediastinal mass causing marked superior venacaval obstruction. [ncbi.nlm.nih.gov]
Early stages (IA, IB, IIA and IIB) were present in 46 patients (74%) and more advanced stages (IIIA, IIIB and IV) were present in 16 patients. [scielo.br]
Entire Body System
- Pain
Pain Management We also advise our patients to stay ahead of the pain with pre-emptive pain control. Instead of taking pain medication after having pain, we ask patients to take pain medication in regular intervals. [drminkim.com]
Do not wait until the pain is severe before you take your medicine. Tell caregivers if your pain does not decrease. Pain medicine can make you dizzy or sleepy. Prevent falls by calling someone when you get out of bed or if you need help. [drugs.com]
Your pain is not relieved by medication. [lhsc.on.ca]
You won’t feel pain during the procedure. Once you’re unconscious, the doctor will place a flexible tube into your mouth and maneuver it into your lungs to help you breathe. [healthline.com]
- Lymphadenopathy
RESULTS: The indication for Cervical Mediastinoscopy was Isolated Mediastinal Lymphadenopathy in 24 patients (60%) and lung cancer staging in 16 patient (40%). The mean age of the patients was 52.7 + 15.1 years. [ncbi.nlm.nih.gov]
Introduction Mediastinal lymphadenopathy may occur either as suspected mediastinal extension of lung cancer or isolated mediastinal lymphadenopathy (IML). [panafrican-med-journal.com]
In 43%, a CT-angiogram for suspected pulmonary embolus identified the incidental lymphadenopathy. Most (>50%) had at least 3 stations with lymph nodes > 10 mm. Most (77%) had hilar lymphadenopathy, too. [pulmccm.org]
- Surgical Procedure
In fact, the surgeon may immediately perform a surgical procedure if a malignant tumor is confirmed while the patient is undergoing mediastinoscopy, thus combining the diagnostic exam and surgical procedure into one operation when possible. [medical-dictionary.thefreedictionary.com]
The surgical procedure was performed twice on 16 patients and three times on 2 patients. Mediastinal shadow was the indication in 6 patients and bronchogenic carcinoma in 3 patients. Average time lag between the procedures was 35 months. [ncbi.nlm.nih.gov]
Anterior mediastinotomy (Chamberlain procedure) A surgical procedure to look at the organs and tissues between the lungs and between the breastbone and heart for abnormal areas. [surgery.ucsf.edu]
- Swelling
They have special cells that fight off infection, swelling, and cancer. Lymph nodes may swell (get bigger) if you have an infection or illness. You may need a standard, anterior, or extended mediastinoscopy. [drugs.com]
Call your surgeon's office if: You have any excess: Bleeding Swelling Discharge (pus) You have a temperature of 38.5° C or 101.50° F. Your pain is not relieved by medication. [lhsc.on.ca]
Immediately alert your doctor if you develop a cough, shortness of breath, bleeding from your stitches, severe chest pain, fever, swelling in the neck or difficulty swallowing. [healthcommunities.com]
Swelling in the neck. Shortness of breath. Difficulty swallowing. Hoarseness of your voice that lasts more than a few days or continues to worsen. [patient.info]
- Fever
CONTACT A CAREGIVER IF: You have a fever. You have nausea (upset stomach) or vomiting (throwing up). Your skin is itchy, swollen, or has a rash. You have questions or concerns about your procedure, or medicine. [drugs.com]
Immediately alert your doctor if you develop a cough, shortness of breath, bleeding from your stitches, severe chest pain, fever, swelling in the neck or difficulty swallowing. [healthcommunities.com]
A high temperature (fever). Severe chest pain. Swelling in the neck. Shortness of breath. Difficulty swallowing. Hoarseness of your voice that lasts more than a few days or continues to worsen. [patient.info]
Respiratoric
- Hoarseness
Transient laryngeal recurrent nerve palsy manifested as prolonged hoarseness of voice was the only minor complication in 3 cases (5.5%). The sensitivity of the procedure was 72%, and the specificity was 100%. [ncbi.nlm.nih.gov]
Rarer but more serious complications include bleeding caused by damage to one of the large blood vessels around the heart and injury to a nerve in the chest (which can cause hoarseness). [merckmanuals.com]
Sometimes the mediastinoscope can temporarily injure a nerve, which may cause hoarseness. This can occasionally be permanent. In rare cases, bleeding complications might require a transfusion or larger chest surgery. [patient.info]
- Mediastinal Disease
CONTEXT: With the advent of modern anatomical and functional imaging technologies, application of cervical mediastinoscopy has decreased in diagnosis and staging of mediastinal diseases. [ncbi.nlm.nih.gov]
- Pleural Effusion
The main results: 55 reactive lymph nodes 18 granulomatous disease, likely sarcoidosis 1 non-TB mycobacterial infection 1 metastatic breast cancer - who had a history of breast cancer and a loculated (cytology-negative) pleural effusion No new diagnoses [pulmccm.org]
effusions, or demonstrate bronchoscopic evidence of extrinsic tracheobronchial compression); (3) enlargement of the mediastinal shadow of unknown cause; and (4) suspected sarcoidosis. [mdedge.com]
Both pleuroscopy and VATS have utility in the diagnosis of pleural effusions but also in their management. [clinicaladvisor.com]
Often done to remove fluid from the affected area, these biopsy types are also used as palliative treatments to relieve pressure and reduce symptoms of pleural, peritoneal and pericardial effusion. [mesothelioma.com]
Evidence should be sought for associated comorbid conditions such as impaired pulmonary function resulting from associated pleural effusions, lung parenchymal infiltration by tumor, airway obstruction, or chronic smoking-related COPD or CAD. [clinicalpainadvisor.com]
- Dyspnea
A history of smoking, cough, sputum production, and orthopnea or dyspnea are hallmarks of cardiopulmonary disease, and impaired exercise tolerance can be helpful in anticipating the patient's response to the stress of anesthesia and surgery. [tele.med.ru]
Musculoskeletal
- Muscle Weakness
In contrast to myasthenia gravis, the muscle weakness improves with exercise and is not reversed by acetyl cholinesterase inhibitor therapy. [academic.oup.com]
Neurologic
- Paresthesia
Although cryoanalgesia was initially shown to effectively relieve pain and improve postoperative pulmonary function, more detailed study has revealed a significant incidence of paresthesia and postthoracotomy pain syndrome. [tele.med.ru]
Workup
You may also be required to undergo several diagnostic tests, including: Blood Workup EKG PET Scan What to Expect Generally an outpatient procedure, mediastinoscopy is performed under general anesthesia. [northshore.org]
X-Ray
- Pericardial Effusion
Often done to remove fluid from the affected area, these biopsy types are also used as palliative treatments to relieve pressure and reduce symptoms of pleural, peritoneal and pericardial effusion. [mesothelioma.com]

Treatment
This case report shows an uncommon complication after mediastinoscopy: pseudoaneurysm of the aortic arch and its minimally invasive endovascular stenting treatment in order to facilitate the recovery and to allow minimal delay to oncological treatment [ncbi.nlm.nih.gov]
Staging directs treatment and implementation of protocols and permits comparison of treatment between patients. [accesssurgery.mhmedical.com]
Prognosis
The involvement of ipsilateral and contralateral mediastinal lymph nodes is associated with poor prognosis and usually excludes patients from upfront surgical treatment. [ncbi.nlm.nih.gov]
Etiology
We reviewed 54 consecutive patients (34 men and 20 women) with mediastinal pathology of varied etiologies who underwent a standard cervical mediastinoscopy from January to December 2012. [ncbi.nlm.nih.gov]
When tubes are inserted for pleural collections of unknown etiology, examination and biochemical analysis of the fluid may yield insight into the etiology. [clinicaladvisor.com]
Other indications include mediastinal lymphadenopathy of unknown etiology, mediastinal masses, primary tracheal tumors, and occasional esophageal tumors. [thoracickey.com]
