Presentation
Although the lesions are not symmetrical in the initial presentation, lesions are very common in both eyes. The diagnosis of ocular histoplasmosis is made by clinical examination. [smjournals.com]
The clinical suspicion was fairly strong in the present patient. [ijo.in]
Another patient presented only histo spots. [scielo.br]
Solitary histoplasmic chorioretinal granulomas (histoplasmoma) are a rare presentation of ocular involvement with the H. capsulatum organism.8 They present as a solitary granuloma and may mimic ocular toxocariasis. [entokey.com]

A confirmed diagnosis will include: The presence of histo spots, small retinal scars that look like "punched out" lesions Peripapillary atrophy (retinal pigment epithelium is affected around the optic nerve) An absence of inflammation of the vitreous [verywellhealth.com]
Fibrovascular tissue invades the break in Bruch’s membrane, causing an infiltrate that includes vascular endothelium, retinal pigment epithelium, photoreceptors, and inflammatory cells between the retinal pigment epithelium and Bruch’s membrane. [eyewiki.aao.org]

Both HLA-DRw2 and HLA-B7 have been found in a high percentage of patients with OHS.11 In addition, HLA-B7 appeared to be related to the presence of disciform scars.12 Several, but not all, of our patients had a chronic fungal infection of the foot or [jamanetwork.com]
Some reports suggest an increased prevalence of POHS in patients with human leukocyte antigen B7 (HLA-B7) and human leukocyte antigen DRw2 (HLA-DRw2). [emedicine.medscape.com]
Since POHS is linked to HLA haplotypes DRw2 and B7, some hypothesize that it could also represent an autoimmune inflammatory reaction triggered by certain organisms, including H. capsulatum.[6] HLA DRw2 has been found in higher concentrations in patients [eyewiki.aao.org]
In particular, POHS has been associated with individuals who have certain histocompatibility antigens, including HLA-B7 and HLA-DRw2, which may increase the risk of developing POHS [20,24]. [academic.oup.com]

Treatment
The systemic antifungal treatments used in the treatment of histoplasmosis have little use in the treatment of ophthalmologic involvement because the fungus does not appear to be in the eye at the time of ophthalmologic diagnosis. [smjournals.com]
Treatments Ocular histoplasmosis requires no treatment except when abnormal blood vessels develop in the central retina. For these patients, laser treatment, intraocular injections, or vitrectomy surgery may be necessary. [louisvillediabeticeyedoctor.com]

Prognosis
If neovascularization is treated early, before advanced scarring develops in the central macula, the prognosis for vision is generally good. Authors THANK YOU TO THE RETINA HEALTH SERIES AUTHORS Sophie J. [asrs.org]
Indeed, this ophthalmological pathology, of random evolution and with heavy clinical consequences, is very little known and rarely sought, whereas an early diagnosis improves its management and hence its functional prognosis. [smjournals.com]

Usually the prognosis is excellent. If there are histoplasmosis scars in the macula it is important to have regular eye exams and an Amsler Grid to monitor the central vision. [rvscny.com]
Etiology
Infection with histoplasmosis and the geographic distribution of presumed ocular histoplasmosis syndrome is a suggestive evidence of Histoplasma capsulatum as the etiological agent. [scielo.br]

Pathophysiology
Pathophysiology H capsulatum is a dimorphic pathogenic fungus that often grows in soil around old chicken houses and areas harboring bats, such as caves. [emedicine.medscape.com]
In addition to loss of outer retinal layers and the retinal pigment epithelium, adhesions between the outer retina and choroidal lesions have been found.[6][12] Pathophysiology Hematogenous spread of H. capsulatum to the choroid causes an inflammatory [eyewiki.aao.org]
Prevention
Each time the damage should be detected as early as possible to prevent vision loss. Eyes should be regularly checked using an Amsler Grid to monitor central vision and recurrence of CNV and histospots. [news-medical.net]
The following are recommended preventative measures: Avoid areas where the fungus is common whenever possible. Use a face mask that can effectively prevent inhalation of the spores. [nvisioncenters.com]
What can I do to prevent visual loss from histoplasmosis? There is nothing that a person with histoplasmosis can do to prevent abnormal vessels from developing. [eyecenters.com]
No special diet or medical treatment has been shown to prevent the formation of CNV. [theretinagroup.com]
Several important facts about PDT should be remembered: PDT is intended to prevent further visual loss. Only about 15% of patients treated with PDT have improvement in their vision. [iowaretina.com]
