Clinical Case A 29-year-old male presented to his primary care physician with a complaint of hemoptysis that has been going on for a year. [ncbi.nlm.nih.gov]

Weil Medicine Emerging infectious diseases 2012 An investigation of 9 new cases of lung fluke infection in Missouri found that in all cases, patients had eaten raw crayfish while on rafting or camping trips and most had been drinking alcohol, so physicians [semanticscholar.org]
Locally acquired Paragonimus kellicotti infestations are most common in young men who become infected with the organism following consumption of raw crayfish while camping or on a river float trip. [clinlabnavigator.com]
Patients consumed raw crayfish while on float (recreational river) trips (7/9, 77.8%), camping (1/9, 11.1%), or as a demonstration of wilderness survival skills (1/9, 11.1%). [go.gale.com]
To ensure timely treatment, anyone who presents with hemoptysis, eosinophilia, fever, and suspicious infiltrates on imaging and has a history of consuming crayfish while taking camping or river trips in Midwestern rivers should be tested for paragonimiasis [ncbi.nlm.nih.gov]
- Lymphadenopathy
Other common imaging findings include pleural effusion, pleural thickening, a mass or consolidation, and mediastinal and/or hilar lymphadenopathy. [ncbi.nlm.nih.gov]

Psychiatrical
- Suggestibility
In raccoons with pleural adhesions massive fibroplasia with eosinophil infiltration beneath the pulmonary pleura suggested a reaction to a migrating parasite entering the lung from the thoracic cavity. [bioone.org]
The morphologic features of the eggs in the biopsy coupled with a history of eating uncooked/undercooked crayfish were highly suggestive of paragonimiasis caused by P. kellicotti, the North American lung fluke. [ncbi.nlm.nih.gov]

- Meningism
We describe the first 2 cases of eosinophilic meningitis due to Paragonimus kellicotti. [ncbi.nlm.nih.gov]
Workup
X-Ray
- Left Pleural Effusion
A CT scan showed a mass/area of consolidation in the left upper lobe of his lung along with mediastinal lymphadenopathy and a left pleural effusion (Figure 1). A bronchoscopy was nondiagnostic, so a CT guided biopsy was performed. [ncbi.nlm.nih.gov]
- Pulmonary Infiltrate
On chest radiographs, the most common findings are pleural effusion, pulmonary infiltrates, and calcifications, but, in some cases (up to 40% in one study), no abnormality is seen [6, 7]. [ncbi.nlm.nih.gov]
- Cavitary Lesion
We describe a patient with a pleuropulmonary infection due to Paragonimus kellicotti that presented as recurrent pneumothoraces and a cavitary lesion. [ncbi.nlm.nih.gov]
Pleura
- Pleural Adhesions
In raccoons with pleural adhesions massive fibroplasia with eosinophil infiltration beneath the pulmonary pleura suggested a reaction to a migrating parasite entering the lung from the thoracic cavity. [bioone.org]
Treatment
Antibody reactivity declined after praziquantel treatment. Thus, the P. kellicotti WB appears to be superior to P. westermani WB for diagnosing Pk infections, and it may be useful for assessing responses to treatment. [ncbi.nlm.nih.gov]
All patients in this series were treated with praziquantel for 2-3 days, and 7 (77.8%) experienced rapid clinical improvement or cure after treatment. One patient had some residual dyspnea and chest tightness 4 weeks after treatment. [go.gale.com]

Prognosis
If the patient had possessed cancer with advanced metastasis, the opportunity for surgery would have been lost and the patient would receive a poor prognosis ( 11 ). [spandidos-publications.com]

What should you consider regarding the epidemiology and microbiology of paragonimiasis? A. Most cases are reported in North America B. P. kellicotti requires snail and crustacean intermediate hosts C. [wwwnc.cdc.gov]
Prevention
All 11 proven Pk infection sera and two samples from suspected cases that were negative by P. westermani WB at the Centers for Disease Control and Prevention (CDC) contained antibodies to antigens at 34 kDa and at 21/23 kDa. [ncbi.nlm.nih.gov]

Effective prevention strategies should focus on physician education to improve awareness of this disease and education targeted at the general population. [go.gale.com]
