Patent Urachus

Persistent Umbilical Sinus

Presentation

  • This case is of clinical relevance as the diagnosis was missed 18 months prior with a milder presentation.[ncbi.nlm.nih.gov]
  • The clinical and radiographic features and magnetic resonance imaging are presented. Stone formation and the huge capacity of the urachal cyst are first reported.[ncbi.nlm.nih.gov]
  • In the second case, a 36-year-old woman presented at 19 weeks gestation for evaluation of moderate bilateral hydronephrosis and an abdominal cyst.[ncbi.nlm.nih.gov]
  • In this report, we present a case of a patent urachus in a15-day-old female who presented with leakage from the umbilical site.[ncbi.nlm.nih.gov]
  • A patent urachus, which is typically diagnosed as leakage from the umbilicus postnatally, can present as an allantoic cyst in the umbilical cord antenatally.[ncbi.nlm.nih.gov]
Vietnamese
  • A 26-year-old Vietnamese primigravida presented at 20 weeks of gestation for evaluation of a cystic mass in the umbilical cord, which was first discovered at week 13 of pregnancy by ultrasound scan.[f1000research.com]
Fecal Incontinence
  • There was no fecal incontinence. She complained of chest pain occasionally.[webmedcentral.com]
Flushing
  • In the following days, we recorded a stream of urine flushing out from the umbilical mass every time the infant cried. The umbilical tape was changed three times a day using normal saline and sterilized gauze.[f1000research.com]
Skin Discoloration
  • If umbilical swelling, skin discoloration and dermatitis develop soon after birth, vigorous therapy and amputation of the urachus are then necessary to avoid complications.[gopetsamerica.com]
Bladder Distention
  • So the obliteration of the urachus may be independent from the level of bladder distention (6, 7). In our case the bladder was well within the pelvis with a normal outlet and orthotopic ureteric orifices.[webmedcentral.com]

Workup

  • Given the current practice to minimize radiation exposure in children to a level as low as possible, CT should not be considered an integral component of the routine workup.[pediatricurologybook.com]

Treatment

  • Empirical anti-microbial treatment should be started immediately following assessment; choices for treatment including a β-lactam and an aminoglycoside.[vetmed.illinois.edu]
  • The recommended treatment option is surgical excision due to the potential risk of malignant change, with urachal adenocarcinoma accounting for 0.3% of all bladder cancers.[ncbi.nlm.nih.gov]
  • The condition was managed by excision of the urachus, the cyst, the umbilicus and dome of the bladder, which is the preferred treatment. The author reviews other urachal anomalies and the complications arising from them.[ncbi.nlm.nih.gov]
  • We describe the occurrence and surgical treatment of Meckel's diverticulum and an urachal anomaly in a 1-year-old boy. The patient had intermittent production of clear fluid from the patent urachus, which disappeared after surgical resection.[ncbi.nlm.nih.gov]
  • Treatment in the past has relied on immediate surgery. Conservative therapy with bladder catheter drainage, resulting in spontaneous closure, has not been described.[ncbi.nlm.nih.gov]

Prognosis

  • The prognosis of this condition is generally good; however, fetal death can occur secondary to compression of umbilical vessels due to the expanding allantoic cyst.[ncbi.nlm.nih.gov]
  • However, patent urachus is associated with a much lower rate of abnormalities than omphalocele, yielding a better fetal prognosis.[ncbi.nlm.nih.gov]
  • The differential diagnosis includes other fetal abdominal wall anomalies that can have severe implications in neonatal prognosis.[omicsonline.org]

Etiology

  • A giant umbilical cord is a rare finding in mature newborns and originates from different developmental etiologies. We report on a case of a mature female newborn presenting a 50 8-cm giant umbilical cord without further malformations.[ncbi.nlm.nih.gov]

Pathophysiology

  • A discussion of urachal anomalies (ie, urachal sinus, urachal cyst, urachal diverticulum, patent urachus) is also included. [1] Pathophysiology The timing of altered embryogenesis leading to bladder anomalies has been a matter of speculation, and the[emedicine.medscape.com]

Prevention

  • A membranous urethral diaphragm prevented closure of the urachus in a female calf. The patent urachus was complicated by an ascending infection of the intraabdominal umbilical remnants.[ncbi.nlm.nih.gov]

  • You may get a prescription for antibiotics to prevent infection, and for a safe medicine to use for pain. The outcome is most often excellent. Patent urachal tube repair Frimberger D, Kropp BP. Bladder anomalies in children.[nlm.nih.gov]
  • Barrier creams, such as Vaseline , can be applied to the area around the umbilicus and the insides of the legs to prevent urine scalding the skin. Summary Patent urachus can occur from birth or appear a few days after birth.[vevs.com.au]
