Presentation
We report on 5 patients who presented to our ED complaining of pleuritic pain, few other symptoms, and negative routine investigations, in whom bedside lung ultrasound aided in making the diagnosis. [ncbi.nlm.nih.gov]
Enlarged mediastinal and hilar lymph nodes are also present (not shown). [erj.ersjournals.com]
Others present the pain as rather dull, being located in either the right or the left side of the chest (depending on the part of the body that was affected). The pleuritic chest pain can be aggravated by breathing, coughing or even sneezing. [mddk.com]

Chest Wall Tenderness
There is no jugular venous distention and no chest-wall tenderness. Laboratory studies reveal a serum creatinine level of 1.0 mg/dL (76.3 µmol/L). Electrocardiogram is shown. [acpinternist.org]

Hyperresonance
In contrast, pneumothorax could lead to hyperresonance on lung examination. In new-onset heart failure due to large myocardial infarction, cardiac examination may show an extra heart sound (third or fourth heart sound). [aafp.org]

Bronchial Breath Sounds
Coarse crepitations, bronchial breath sounds and increased vocal resonance are heard in the same region anteriorly and in the right axilla. A chest radiograph is taken. Q1) Interpret the chest radiographic findings. [omicsonline.org]

Workup
Extensive workups including repeated pulmonary ventilation-perfusion scans and cardiac catheterisation have not yielded other diagnoses to explain the pain. [ncbi.nlm.nih.gov]

Treatment
Because of its efficacy, colchicine has emerged as the treatment of choice for acute bouts of recurrent pericarditis and can be useful in the prevention of recurrences. Treatment duration is 6 months. [acpinternist.org]
NSAIDs (particularly indomethacin), along with opioid analgesics, remain the mainstay of treatment for pleurisy; identification and treatment of the primary cause are also important. [uspharmacist.com]
Treatment for pleurisy Treatment for pleurisy may include: treating the underlying cause – for example, treatment for tuberculosis anti-inflammatory medications other medications to manage the pain antibiotics for a bacterial infection draining off the [betterhealth.vic.gov.au]
Treatment options Treatment for pleurisy will depend on the underlying cause of the condition. For example, pleurisy that is caused by a viral infection will often resolve without needing treatment. [hse.ie]
Prognosis
Prognosis In patients with pleurisy, the outlook depends on the underlying medical illness. Lung infection — People with epidemic pleurodynia or other viral cause of pleurisy have an excellent prognosis. [drugs.com]
The recommended surgical approach is complete resection to enable excellent prognosis and survival 5, 6. [erj.ersjournals.com]
Prognosis The outcome of treatment may depend on the seriousness of the underlying disease. Most cases of pleuritis of unknown cause heal with time. [diagnose-me.com]
Prognosis Prompt diagnosis, followed by appropriate treatment, ensures a good recovery for most pleurisy patients. Generally speaking, the prognosis for pleurisy is linked to the seriousness of its cause. [medical-dictionary.thefreedictionary.com]
Generally, pleurisy treatment has an excellent prognosis, but if left untreated it can cause severe complications. [en.wikipedia.org]
Etiology
Differential Diagnosis Pleuritic chest pain has many etiologies. [aafp.org]
“Pleuritic” means accentuated by breathing or coughing; it calls to mind pulmonary disease, unless it’s also exacerbated by twisting, bending, or reaching, which would suggest musculoskeletal etiology. [diagnosisdude.com]

Pathophysiology
Pathophysiology The pleural space is a potential space that can be filled with a variety of substances including transudative fluid, exudative fluid, pus, blood, air, or chyle. [bestpractice.bmj.com]
B 36 Pathophysiology The visceral pleura does not contain pain receptors, whereas the parietal pleura is innervated by somatic nerves that sense pain due to trauma or inflammation. [aafp.org]
Many people refer to this condition as a dissecting aortic aneurysm, but this is really not a correct term, since pathophysiologically, an aneurysm is a different entity. [emsworld.com]
Prevention
You may have less pain after this happens, because the fluid prevents the two layers of the pleura from rubbing together. If there is a large amount of fluid, it may prevent the lung from expanding when you breathe in. [northshore.org]
Prevention In some cases, you can prevent pleurisy by preventing the medical condition that causes it. For example, some types of pneumonia can be prevented by vaccination. [drugs.com]
This procedure thereby obliterates the space between the pleura and prevents the reaccumulation of fluid. Can pleurisy be prevented? Some cases of pleurisy can be prevented, depending on the cause. [medicinenet.com]
