Presentation
Presentation on theme: "Proliferating Trichilemmal Cyst Presenting as Limbal Mass Dr Pawan Prasher Sri Guru Ram Das Institute of Medical Sciences and Research, Amritsar, India. [slideplayer.com]
We present the case of a 69-year-old woman with a 25-year history of multiple enlarging scalp masses. The patient was evaluated for surgical consultation after the dominant mass presented with malignant degeneration. [ajnr.org]
It can present ulceration, inflammation or even bleeding and can cause necrosis of the adjacent tissues. [orpha.net]
This paper presents the diagnosis and management of one case of malignant proliferating trichilemmal cyst followed by review of the literature. [ncbi.nlm.nih.gov]
Entire Body System
- Foul Smelling Discharge
In most cases there is only one lesion and occur in elderly women. [1] [2] The tumor is not painful but sometimes they may grow very large and pressure and damage the skin around it resulting in wounds and foul-smelling discharge. [rarediseases.info.nih.gov]
It was initially growing slowly and later it started to ulcerate with foul smelling discharge and rapid enlargement that interfered with the patient's daily life mainly during the last 6 months. [panafrican-med-journal.com]
Ulceration and foul-smelling discharge may also occur. How is a proliferating trichilemmal cyst diagnosed? Specific features on histology of a proliferating trichilemmal cyst allow the diagnosis to be made on skin biopsy. [dermnetnz.org]
smelling discharge associated with severe pain. [amhsjournal.org]
- Weight Loss
She had no history of weight loss, gastrointestinal complaints, breathlessness, nausea, vomiting and recurrent fever. She had no eye complaints. [idoj.in]
- Soft Tissue Mass

Skin
- Lanugo
Lanugo hair follicles of the bald scalp and follicles of other areas devoid of non-terminal hair are unlikely to produce these tumors. [bioline.org.br]
- Subcutaneous Mass
Characteristic findings on physical examination On physical examination, PTT can be smooth dermal to subcutaneous masses or exophytic to fungating. Ulceration is sometimes present, with foul-smelling drainage of keratinous material (Figure 1). [dermatologyadvisor.com]

- Scalp Mass
The patient was discharged in stable condition and was doing well at the 6-month follow-up visit, without recurrence of the scalp mass. [neurologyindia.com]
We present the case of a 69-year-old woman with a 25-year history of multiple enlarging scalp masses. The patient was evaluated for surgical consultation after the dominant mass presented with malignant degeneration. [ajnr.org]
Brain CT shows a right superior cerebral, right inferior temporal-parietal parenchymal mass and a left superior scalp mass. B. Chest CT shows an enhancing mass in the right side of the pulmonary artery. Figure 3. [kjim.org]
Recently, we encountered a tumour presenting as a large scalp mass associated with a cervical lymph node, thus suggesting a malignant aetiology. Careful histologic assessment led to the correct diagnosis. [ijdvl.com]
There were multiple gradually increasing scalp masses for the last 4-5 years and the patient had been keeping long hair since birth. [ijpmonline.org]
Workup
Progress logically from the histologic pattern, through the appropriate workup, around the pitfalls, to the best diagnosis. [books.google.de]
Treatment
Radical surgical treatment and precise histological analysis are essential in order to achieve a fast and complete healing. Cooperation of the clinician and the pathologist is essential for accurate diagnosis, treatment and follow-up planning. [ncbi.nlm.nih.gov]
What is the treatment of a proliferating trichilemmal cyst ? The recommended treatment is complete surgical excision with a margin of normal tissue. Mohs micrographic surgery may be used to ensure better margin control. [dermnetnz.org]
Prognosis
Prognosis is good in most cases but the patient should be followed closely after surgery. [2] Last updated: 1/6/2016 [rarediseases.info.nih.gov]
Objectives: (1) To report the incidence of cases of PTCs in a General Hospital during a 12 years period. (2) To determine the expression of ki67 using immunohistochemical staining. (3) To correlate ki67 reaction patterns with clinical prognosis. [ijtrichology.com]
What is the Prognosis of Benign Proliferating Pilar Tumor? (Outcomes/Resolutions) The prognosis for Benign Proliferating Pilar Tumor is excellent with appropriate treatment, since it is a benign cyst. [dovemed.com]
[…] become malignant 11 Conclusions Proliferating trichilemmal cyst can present as a gradually progressing limbal mass It is a benign condition that needs to be distinguished from malignant squamous cell carcinoma which has similar presentation but different prognosis [slideplayer.com]
Etiology
Eruptive collagenomas are non familial connective tissue nevi of unknown etiology presented with an abrupt onset. While most cases are reported in young adults, there is a paucity of literature in children. [ingentaconnect.com]
The granulomatous lymphadenitis was presumed to be of tubercular etiology in an Indian patient, despite a failure to demonstrate bacilli in the tissue. [ijdvl.com]
Rare sites described include wrist, elbow, mons pubis, vulva, buttock and chest. [1] The most characteristic feature described for PTT is trichilemmal keratinization and etiologies proposed include trauma and inflammation in TC, a finding further corroborated [ijpmonline.org]
Etiology Trichilemmal carcinoma is a carcinoma of the outer root sheath. The exact etiology is unknown, but it appears to be related to actinic damage. [clinicaladvisor.com]
(Etiology) The cause of development of Benign Proliferating Pilar Tumor is unknown. [dovemed.com]
Epidemiology
Epidemiology Frequency Proliferating pilar tumor (PPT) are rare. Sex These tumors occur more commonly in females than in males. [emedicine.medscape.com]

Pathophysiology
Pathophysiology A proliferating pilar tumor (PPT) usually arises in the setting of single or multiple pilar cysts. An asymptomatic nodule is often present for months to years before a rapid increase occurs in the size of the lesion. [emedicine.medscape.com]
Pathophysiology It is unknown whether trichilemmal carcinoma develops from an existing benign lesion or de novo. Few trichilemmal carcinomas show evidence of a coexisting trichilemmoma. [clinicaladvisor.com]
Prevention
Adjuvant chemotherapy and radiotherapy have been used to prevent recurrence in MPTT. [3] Our patient received adjuvant radiotherapy and has been kept under close follow-up. [jpgmonline.com]
Their correct recognition has an impact on treatment, outcome, and prognosis [2], [4], [5] and can prevent a therapeutic misadventure. References 1. Erdem H, Yildirim U, Uzunlar AK, Sahiner C, Ozcelik D, TopluG. [ijdvl.com]
