Presentation
We present a case of posterior reversible encephalopathy syndrome (PRES) presenting with pulsatile tinnitus. [ncbi.nlm.nih.gov]
Cardiovascular
- Thrombosis
Ten cases of pulsatile tinnitus associated with cerebral venous thrombosis have been published in the literature. In most of cases, pulsatile tinnitus was the only symptom. [ncbi.nlm.nih.gov]
- Heart Failure
After eight weeks, she developed left-sided ocular symptoms, progressing from conjunctival hyperaemia and orbital oedema to an abducens nerve palsy, and ultimately to heart failure. [ncbi.nlm.nih.gov]
- Venous Insufficiency
Ligation of the jugular vein in the neck or packing of the jugular bulb is contraindicated due to the risk of triggering intracranial venous insufficiency. [entokey.com]
Musculoskeletal
- Osteoporosis
The sigmoid sinus diverticulum may be a clinical manifestation of osteoporosis occurring in the perimenopausal period. [ncbi.nlm.nih.gov]
Face, Head & Neck
- Wide Neck
Angiographic studies revealed a wide-necked venous aneurysm of the left transverse-sigmoid sinus. Solitary stent placement across the aneurysm neck resulted in a slight modification in the lesion's characteristics. [ncbi.nlm.nih.gov]
Neurologic
- Temporal Headache
This paper reports a case of spontaneous indirect carotid cavernous fistula that presented with pulsatile tinnitus, left-sided temporal headache and left-sided ptosis. The pulsatile tinnitus, its aetiology and investigation are discussed. [ncbi.nlm.nih.gov]
- Encephalopathy
We present a case of posterior reversible encephalopathy syndrome (PRES) presenting with pulsatile tinnitus. [ncbi.nlm.nih.gov]
- Oscillopsia
Clinical symptoms: right-sided pulsatile tinnitus, unpleasant perception of the patient’s own footsteps Tullio’s phenomenon (vestibular symptoms that can be caused by loud sounds, such as dizziness, nystagmus, oscillopsia) Conductive hearing loss caused [ncbi.nlm.nih.gov]
Workup
All patients had a diagnostic workup including cranial MRI and DSA. MRI examinations were blinded to the results of DSA and retrospectively analyzed in consensus by two experienced neuroradiologists. [ncbi.nlm.nih.gov]
Despite how common pulsatile tinnitus is, its workup, diagnosis, and management pose a dilemma for the otolaryngologist [2]. [hindawi.com]
Other Pathologies
- Gliosis
– Head angioresonance - brain images in T2 showed hyperintense focus on left radial crown, which may correspond to small area of gliosis or lacuna infarction. [bjorl.org]
Treatment
The choice of treatment is not easy, even when the diagnosis is certain. Reassurance of the patient is often sufficient. Nevertheless, a surgical treatment is sometimes performed--usually ligature of the internal jugular vein. [ncbi.nlm.nih.gov]
Prognosis
The symptom of pulsatile tinnitus allowed an early diagnosis of essential thrombocythemia and a more favorable prognosis. The case demonstrates the importance of blood tests for all patients who present with pulsatile tinnitus of unknown origin. [ncbi.nlm.nih.gov]
Common conditions include: Arteriovenous fistula Atherosclerotic vascular stenosis Idiopathic intracranial hypertension Paraganglioma Paget Disease Otosclerosis Prognosis The prognosis for pulsatile tinnitus varies considerably based on the etiology. [statpearls.com]

Korean herbal medicine, honeybee larvae, other various vitamins and minerals—did not exist.[75] A 2016 Cochrane Review also concluded that evidence was not sufficient to support taking zinc supplements to reduce symptoms associated with tinnitus.[109] Prognosis [en.wikipedia.org]
Etiology
Etiology The etiology of pulsatile tinnitus can broadly break down into two categories vascular vs. nonvascular. [statpearls.com]
Specific Etiologies of Tinnitus.. [bjorl.org]
Clinical examination suggested a venous etiology. An aneurysm of the transverse-sigmoid sinus was identified on computed tomography angiography (CTA) and confirmed by digital subtraction angiography. [ncbi.nlm.nih.gov]
It can also be classified based on etiology, which can be venous or arterial. [hindawi.com]

It organizes in a way that can help practitioners narrow down the differential, which will, in turn, help guide management decisions.[1][4][2][5][6][3] Epidemiology The epidemiology of tinnitus is poorly defined as there is no standard for determining [statpearls.com]

These choices may have skewed the epidemiological statistics for underlying pathologies and may have eliminated discussion of any alternative approaches to diagnosis common in non-English speaking countries. [theajo.com]

Pathophysiology
Although treatment via an external approach or endovascular coiling has been reported, much remains unknown about the possible pathophysiological mechanisms and appropriate management of PT. [ncbi.nlm.nih.gov]
Prevention
By Jayne Wallace for the Weill Cornell Medicine Brain and Spine Center The Centers for Disease Control and Prevention estimates that 15 percent of the U.S. population, or 48 million people, have some type of tinnitus, hearing a ringing or buzzing in the [hearinghealthfoundation.org]
Prevention In many cases, tinnitus is the result of something that can't be prevented. However, some precautions can help prevent certain kinds of tinnitus. Use hearing protection. [mayoclinic.org]
Identification of the etiology of this type of tinnitus is essential to define the appropriate therapy and to prevent sequelae that may result from late or incorrect diagnosis. [bjorl.org]
The use of any hearing aid may not fully restore normal hearing and does not prevent future hearing loss. Testing is to evaluate your hearing wellness, which may include selling and fitting hearing aids. [andersonaudiology.com]
