Presentation
Primary rectal lymphoma is a rare presentation of gastrointestinal lymphomas. Its clinical presentation is indistinguishable from that of rectal carcinoma. [ncbi.nlm.nih.gov]
For each case, we reviewed demographic information as well as medical records regarding symptoms at presentation, diagnosis, histological features, treatment, and follow-up. [synapse.koreamed.org]
Intraepithelial lymphocytes were present in 9 cases, and 5 cases showed nondestructive lymphoepithelial lesions. During a mean follow-up of 5.8 years, none showed a recurrence or developed lymphoma. [biblioteca.universia.net]

Immune System
- Massive Splenomegaly
Physical examination and computed tomography demonstrated massive splenomegaly, and a complete blood count revealed microcytic anemia and lymphopenia. [current-oncology.com]
- Tender Spleen
Physical examination revealed a markedly enlarged, firm, non-tender spleen extending to the pelvis. [current-oncology.com]
Gastrointestinal
- Rectal Discharge
Symptoms included rectal bleeding in three, pain on defecation in two, and mucoid rectal discharge in two. Systemic "B" symptoms (ie, fever, night sweats, and/or weight loss), as well as a rectal mass, were present in all four. [ncbi.nlm.nih.gov]
Workup
Upon completion of further workup, patient was found to have synchronous diagnoses of a stage II non-Hodgkin’s lymphoma and a cT3 N0 M0 proximal rectal adenocarcinoma with moderate obstruction. [jgo.amegroups.com]
Serum
- Anisocytosis
Laboratory tests revealed hypochromic and microcytic anemia, leucopenia, abundant teardrop cells, and anisocytosis. Further evaluation was recommended, but the patient elected to delay additional work-up. [current-oncology.com]
- Microcytic Anemia
Physical examination and computed tomography demonstrated massive splenomegaly, and a complete blood count revealed microcytic anemia and lymphopenia. [current-oncology.com]
- Anisopoikilocytosis
Peripheral blood smear revealed severely hypochromic microcytic anemia, with moderate anisopoikilocytosis, decreased reticulocyte response, moderate lymphopenia, and severe monocytopenia. [current-oncology.com]
Biopsy
- Hypercellular Bone Marrow
Left posterior iliac crest biopsy exhibited hypercellular bone marrow with non-Hodgkin lymphoma involvement, consistent with marginal zone lymphoma. Those findings led to a classification of stage iv smzl 7. [current-oncology.com]
Treatment
Cases failing initial therapies were responsive to various second-line treatments. Two cases spontaneously regressed with observation alone. [ncbi.nlm.nih.gov]
The treatment for gastrointestinal lymphomas remains uncertain. [synapse.koreamed.org]
For patients with stage III–IV FL who have indications for treatment, immunochemotherapy is the most commonly used treatment modality locally and abroad. The treatment plan consists of 8 courses of rituximab therapy. [journals.lww.com]

Prognosis
It is unclear whether surgery, chemotherapy, radiotherapy or combinations of these treatments improve the prognosis and survival rate in such patients. The role of surgery in primary rectal lymphoma of the rectum is still controversial. [ncbi.nlm.nih.gov]

Etiology
Etiological factors involved in the development of primary colorectal lymphoma are unknown. [californiacolorectalsurgeons.com]
The underlying etiology is unknown but an infective/inflammatory pathogenesis is suspected as their is higher incidence in patients with HIV/AIDS, inflammatory bowel disease and post-transplantation immunosuppression 1. [radiopaedia.org]
Although an association with infections such as chlamydia or Epstein–Barr virus has been reported [ 5, 6 ], its etiology remains unknown. [e-ce.org]
The underlying etiology remains unclear. However, the incidence of this tumor is increasing, and its presentations are varied. [scielo.isciii.es]
