Presentation

The spotted fever rickettsia, Rickettsia helvetica, is mostly reported to present a flu-like self-limiting disease. [ncbi.nlm.nih.gov]

Entire Body System

  • Fever

PATHOGENESIS: Typhus, spotted fever and trench fever are transmitted via arthropod vectors; Q fever is acquired via inhalation or ingestion of contaminated milk or food. [myplace.frontier.com]

A 17-year-old otherwise healthy girl was admitted to Department of Zoonotic and Tropical Diseases because of fever, eschar and rash. [ncbi.nlm.nih.gov]

On day 7 after the second onset of fever, the patient was able to work but still felt ill and had a slight fever (37.6°C). [doi.org]

  • Inflammation

Once inside your bloodstream, the Rickettsia invade the cells that line your blood vessels, causing inflammation and leading to an immune response. It is this inflammation and host immune reaction that leads to the symptoms of the disease. [study.com]

Histopathology Histological changes have been classified into the broad category of necrosis and inflammation. [thefishsite.com]

Complications of typhus include: Inflammation of the lungs or liver Meningoencephalitis Acute renal failure Multiple organ failure. [dermnetnz.org]

  • Lymphadenopathy

[…] necrosis erythema and lymphadenopathy (DEBONEL). [ncbi.nlm.nih.gov]

The most frequent acute symptoms observed were fever (100%), headache (71%), arthralgia (43%), myalgia (43%), cough (43%), rash (maculopapular/petechial) (43%), nausea (29%), pharyngitis (29%), and lymphadenopathy (29%). [doi.org]

  • Fishing

    P. salmonis replicates within membrane-bound cytoplasmic vacuoles in selected fish cell lines and in the cells of tissues throughout infected fish. P. salmonis is the first of the RLO of fish to be fully characterized. [thefishsite.com]

Patient 6’s illness may have been rickettsial in nature because of the appropriate incubation time after a fishing trip in an area endemic for ticks. [doi.org]

  • Chills

    Other symptoms included myalgia (n = 3), chills (n = 3), fatigue (n = 2), arthralgia (n = 2), and headache (n = 2). [ncbi.nlm.nih.gov]

Case Report 24 year old male was admitted in downtown hospital with history of high grade fever associated with chill, headache, yellowish discoloration of urine, rash in abdomen and chest of 3weeks duration. [pubs.sciepub.com]

This is often accompanied by muscle aches and chills. The fever can be so high that the patient hallucinates or loses consciousness. 'Typhus' in Greek literally means 'stuporous' after this classic symptom. [study.com]

Patient 1: A 58-year-old male who owned cats was admitted to the hospital because of fever, chills, oppressive headache and diffuse abdominal pain in the last four days. Physical exam only revealed moderate tenderness to abdominal palpation. [elsevier.es]

Gastrointestinal

  • Nausea

Tabs Content Clinical Overview Diagnosis Indications for Testing Tick bite exposure and relevant epidemiologic setting Fever, headache, myalgia, nausea, vomiting; rash 2-5 days after fever Laboratory Testing Diagnosis and laboratory confirmation recommendations [arupconsult.com]

Symptoms (incubation time: 5-7 days): Fever, nausea, vomiting, severe headache, lymphadenitis, exanthema. [arminlabs.com]

Experience of the self-reported symptoms nausea (p = 0.006) and radiating pain (p = 0.041) was more common among those with recent, current or probable infection compared to those who did not seroconvert. [ncbi.nlm.nih.gov]

She returned four days later with persistent fever, anorexia, irritability, photophobia, cough, diffuse myalgias, nausea, vomiting, and a rash. [web.uconn.edu]

Patients infected with R. rickettsii generally have nonspecific symptoms including fever, nausea, vomiting, muscle pain, lack of appetite, and severe headache after an incubation period about 5-10 days following an infected tick bite. [microbewiki.kenyon.edu]

Musculoskeletal

  • Myalgia

Most patients infected with R. sibirica showed typical clinical symptoms, including high-grade fever (38.9-39.5°С), myalgia, rash, eschar at the site of the tick bite, and elevated levels of serum aminotransferases. [ncbi.nlm.nih.gov]

Five days after removing the tick (day 5), an influenzalike illness with myalgia and arthralgia developed. [doi.org]

Tabs Content Clinical Overview Diagnosis Indications for Testing Tick bite exposure and relevant epidemiologic setting Fever, headache, myalgia, nausea, vomiting; rash 2-5 days after fever Laboratory Testing Diagnosis and laboratory confirmation recommendations [arupconsult.com]

Complications (app. 13%): peri-/myocarditis, kidney insufficiency, pneumonia, encephalitis, gastrointestinal bleedings, anemia, hepatitis, myalgia [arminlabs.com]

Skin

  • Skin Rash

Most rickettisal species produce a constellation of high fever, headaches, and skin rash. Most Rickettsial species are transmitted by arthropod vectors during blood meals. Treatment for all Rickettsial diseases is doxycycline or chloramphenicol. [pathwaymedicine.org]

This is followed by dissemination via the vascular system producing vasculitis and a skin rash. These lesions may become necrotic. [myplace.frontier.com]

Rash (small, tiny bumps on the skin) normally starts on the limbs and may eventually involve the whole body. [tickbites.net]

Some other conditions that may be confused with tick bite fever are infection with Neisseria meningitidis (the meningococcus, which causes meningitis and a skin rash), measles and German measles. [health24.com]

  • Eruptions

Several days after an eschar appears, the following can develop: Fever Headache Rash (sparse maculopapular or papulovesicular eruptions on the trunk and extremities) Muscle aches NOTE: R. parkeri rickettsiosis can be difficult to distinguish from RMSF [cdc.gov]

The infection tends to be moderate to severe and a generalised maculopapular eruption is seen. The systemic features of epidemic louse-borne typhus include neurological symptoms (eg, deafness and delirium) and abdominal pain. [dermnetnz.org]

The rash is a diffuse macular (featuring elevated discoloration) rash, is most prominent on wrists and ankles, which will usually erupt three or four days after the onset of fever. [web.uconn.edu]

Cutaneous eruptions in Indian tick typhus. Int J Dermatol. In article [7] Current medical diagnosis and treatment-2015. Chapter 32-rickettsial disease. [pubs.sciepub.com]

She had a temperature of 39°C and a widespread macular, blanching eruption over her trunk and limbs, including the palms and soles. [mja.com.au]

Neurologic

  • Headache

Clinical features are generally mild, characterized by eschar, fever, headache, malaise and lymphadenopathy. Antigenic similarity among rickettsiae leads to cross-reactive antibody responses; definitive diagnosis requires molecular methods. [ncbi.nlm.nih.gov]

She was febrile (38.5°C) and reported headache, nausea, and abdominal pain. She had no eschar or rash. [doi.org]

Usually a fever accompanies the appearance of the eschar along with other discomforts such as headache. Lymph nodes near the site usually swell. [tickbites.net]

Epidemiology

Epidemiological and clinical evidences, coupled with our serological analysis, suggest that R. parkeri is the etiological agent of human cases of spotted fever in Uruguay, a disease that has been recognized in that country as cutaneous-ganglionar rickettsiosis [ncbi.nlm.nih.gov]
