Presentation
We present a case of S. paratyphi A enteric fever aseptic meningitis. Headache was so prominent in the case presented that a lumbar puncture was performed to rule out meningitis. Rose spots were not apparent in this dark-skinned patient. [ncbi.nlm.nih.gov]
Hence the present study was done to establish the existing epidemiology of enteric fever due to Salmonella paratyphi A-their common presenting symptoms and antibiotic sensitivity pattern- at our setting. [nepjol.info]

Fever
BACKGROUND & OBJECTIVE: Enteric fever is a major public health problem in India. It is classically caused by Salmonella enterica serotype Typhi. [ncbi.nlm.nih.gov]
-
Malaise
A patient of Pakistani-origin was admitted to Bradford Royal Infirmary, UK, following a 3-week history of cough, headache and general malaise. [ncbi.nlm.nih.gov]
After an incubation period of 5 to 21 days (generally 7 to 14 days), fever and malaise develop, often associated with cough. A small proportion of patients may have diarrhea during the incubation period. [foodpoisonjournal.com]

Splenomegaly
A case of salmonella paratyphi A fever with reversible pancytopenia in a 15 years old boy who presented with history of high grade continuous fever, epistaxis and haemoptysis, relative bradycardia and splenomegaly is described here. [ncbi.nlm.nih.gov]
When it presents as FUO, clinical and laboratory manifestations may not be typical, especially features such as relative bradycardia, leucopenia, and splenomegaly. [mjhid.org]
The syndrome is characterized by fever, splenomegaly, cytopenias, liver dysfunction, and hyperferritinemia. [omicsonline.org]
No splenomegaly or evidence of free fluid or tenderness in the abdomen. Investigations revealed anaemia of chronic disease (Hb 8.5 gm%, hematocrit 25%). [casereports.in]
He had high fever temperature (104 C), relative bradycardia and splenomegaly. Rest of the systemic examination was normal. Blood samples were taken for complete blood picture, Widal test and blood culture. [jpma.org.pk]

Diarrhea
While traveling in India, a previously healthy twenty-year-old man had febrile diarrhea. The patient was prescribed medical therapy, and all symptoms were resolved. Fourteen days later, however, similar symptoms recurred. [ncbi.nlm.nih.gov]
HEALTH HAZARD PATHOGENICITY: Bacterial enteric fever with abrupt onset, continued fever, malaise, headache, anorexia, enlargement of spleen, bradycardia, rose spots on trunk occur on approximately 25% of Caucasians, constipation is more common than diarrhea [msdsonline.com]
Clinical Criteria One or more of the following: Fever Diarrhea Abdominal cramps Constipation Anorexia Relative bradycardia Laboratory Criteria for Diagnosis Confirmatory laboratory evidence: Isolation of S. [wwwn.cdc.gov]

Abdominal Pain
He developed abdominal pain and diarrhoea in the week prior to admission, and presented to hospital with fever, tachycardia and raised inflammatory markers. [ncbi.nlm.nih.gov]
pain, nausea, vomiting, diarrhea, dehydration may result; mild and asymptomatic infections occur EPIDEMIOLOGY: Sporadically or in limited outbreaks; infrequently identified in North America; probably more frequent than reported; serotype B is most common [msdsonline.com]
However 8 days into ciprofloxacin treatment she developed fever and rigors and intermittent lower abdominal pains, which persisted requiring readmission at day 12. [nzma.org.nz]

Nausea
[…] frequently psychosis, hepatitis, cholecystitis, pneumonitis, and pericarditis; clinically similar to typhoid fever but milder with lower case fatality rate; Common enterocolitis may result without enteric fever; characterized by headache, abdominal pain, nausea [msdsonline.com]
Long term permanent carriage occurs in Clinical manifestations Salmonella infection usually presents as acute gastroenteritis, with sudden onset of headache, abdominal pain, diarrhoea, nausea and sometimes vomiting. [health.nsw.gov.au]
The vaccine manufacturer recommends that Ty21a not be administered to infants or to children aged VACCINE SAFETY AND ADVERSE REACTIONS Adverse reactions to Ty21a vaccine are rare and mainly consist of abdominal discomfort, nausea, vomiting, and rash. [wwwnc.cdc.gov]

Gagging
His four limbs were weak (grades 2-3) and he was areflexic with mild facial weakness and absent gag reflex. Brain CT and MRI were normal. Cerebrospinal fluid analysis showed high protein. [ncbi.nlm.nih.gov]

Tachycardia
He developed abdominal pain and diarrhoea in the week prior to admission, and presented to hospital with fever, tachycardia and raised inflammatory markers. [ncbi.nlm.nih.gov]
She had tachycardia, BP 120/80 mmHg; temperature of 1020F. Liver was palpable 15 cm below the right coastal margin, firm and tender. No splenomegaly or evidence of free fluid or tenderness in the abdomen. [casereports.in]
There was tachycardia, tachypnea, hypotension (blood pressure 80/46 mm Hg), jaundice, multiple cutaneous ecchymoses, bilateral cervical lymphadenopathy (maximum dimension 1.5 cm x 1 cm), mild hepatosplenomegaly (liver and spleen palpable 2 cm and 1 cm [omicsonline.org]

Both of the patients had underlying congenital heart disease. Two cases in the literatures of endocarditis and five cases of pericarditis caused by Salmonella paratyphi were reviewed. [ncbi.nlm.nih.gov]

Back Pain
A 70-year-old man was admitted to Chosun University Hospital with reported consistent low back pain with a history of having 5 days of chills and fever in another hospital a month ago. [ncbi.nlm.nih.gov]
In this case, we treated with ciprofloxacin and cefotaxime, and the patient’s clinical features including back pain and ESR level decreased. [bmcinfectdis.biomedcentral.com]
Fever was intermittent, high grade, no diurnal variation, not associated with chills, lower abdomen, back pain or hematuria and loose stools. On examination, she was poorly nourished lady with pallor, icterus and mild pitting pedal edema. [casereports.in]

Treatment
Here, we report a case of initial antibiotic treatment failure in a Korean man with Salmonella Paratyphi A infection and conducted next-generation sequencing (NGS) to determine the cause of failure of initial treatment for Salmonella Paratyphi A infection [ncbi.nlm.nih.gov]
The revived sample A was divided into two parts Gr.I (control) and Gr.II (revived; treatment); likewise, ATCC B was labeled as Gr.III (lyophilized; treatment). Biofield treatment strategy Gr. I remained as untreated. The treatment Gr. [trivedieffect.com]
Prognosis
Delayed diagnosis and multiorgan involvement are associated with poor prognosis [ 2, 3 ]. The condition is frequently fatal despite treatment. [omicsonline.org]
Prognosis Most patients with paratyphoid fever recover completely, although intestinal complications can result in death. With early treatment, the mortality rate is less than 1%. [medical-dictionary.thefreedictionary.com]
Prognosis In the days before antibiotics, mortality was 20%. Between 10-20% of patients treated with antibiotics have a relapse, usually a week after stopping the antibiotic (although it can be much later). [patient.info]
Etiology
Paratyphi A cases, and controls, calculating that a combination of six metabolites could accurately define the etiological agent. [ncbi.nlm.nih.gov]
In chronic mastitis and breast abscess Salmonella species should be considered as one of the etiological agents. [scopemed.org]
A very high index of suspicion is critical for early diagnosis of HLH, and a thorough diagnostic evaluation should be undertaken to identify the underlying etiology. [omicsonline.org]
