Presentation
Hyperhidrosis, another hallmark of SS, was present in only 42.3% of the cases and mainly in adult onset. [ncbi.nlm.nih.gov]
The presentation is one of periodic hypothermia and hyperhidrosis, without warning and often lasting hours before spontaneous resolution back to baseline 1-3. [radiopaedia.org]
As with our patient, the majority of reported cases of Shapiro's syndrome have presented spontaneously in adulthood [1, 3–7]. The reason for late presentation, as well as the reason symptoms tend to spontaneously occur and remit, is unknown. [academic.oup.com]
CASE A 8-year-old boy in otherwise good health, who, since 5 years of age, presented with sporadic episodes of nocturnal hypothermia, associated with pallor and profuse sweating. [scielo.br]

Asymptomatic
However, during the next few days, we noted that the patient's temperature decreased during each episode of sweating/chilling to 32 C–35 C and never increased to 36.7 C when he was asymptomatic (figure 1). [academic.oup.com]

Hyperthermia
A problem here may lead to hyperthermia [3]. Periodic hypothermia has been observed with agenesis of the corpus callosum, although the exact mechanism for this is not understood. [academic.oup.com]

Suggestibility
The presence of SS in siblings of different genders suggests an autosomal recessive inheritance model, however a gonadic mosaicism responsible for an autosomal de novo mutation cannot be ruled out. [ncbi.nlm.nih.gov]
