Presentation
A family of 3 (husband, wife, and their son) presented with symptoms compatible with SFG rickettsiosis. For 2 patients, serum samples presented increased levels of IgM and IgG for SFG Rickettsia. [ncbi.nlm.nih.gov]
Because a delay in empiric antimicrobial therapy for RMSF can be fatal, clinicians need to be familiar with the presentation and management of this illness to avoid a poor outcome (Table 1). [pedsinreview.aappublications.org]
One of the major clues in these new descriptions has been atypical clinical presentations. The unusual feature of the case presented here was the presence of an eschar tache noire which would be most unusual for RMSF. [journals.lww.com]

including the skin, lungs, CNS, and renal systems. • Early symptoms are nonspecific, but Rocky Mountain spotted fever should be suspected in patients with high fever, headache, and a macular rash with a possible tick exposure. • Neurologic symptoms [medlink.com]
What are Rocky Mountain spotted fever symptoms and signs? RMSF typically begins with headaches, a high fever, and muscle aches. Some individuals may also develop abdominal pain, nausea, and vomiting. [medicinenet.com]
Gastrointestinal
- Abdominal Pain
The following symptoms are commonly seen with this disease; fever, rash (occurs 2-5 days after fever, may be absent in some cases; see below), headache, nausea, vomiting, abdominal pain (may mimic appendicitis or other causes of acute abdominal pain), [health.ri.gov]
Typical symptoms of RMSF/SFR include fever, headache, abdominal pain, vomiting, and muscle pain. A rash may also develop but is often absent in the first few days; some patients never develop a rash. [epi.publichealth.nc.gov]
Other symptoms are general symptoms common with other types of infections such as vomiting, diarrhea, and abdominal pain. [web.uconn.edu]

Musculoskeletal
- Arthralgia
Initially, findings of high-grade fever, mild generalised arthralgia, rashes on the trunk, mild thrombocytopenia, elevated liver enzymes and renal dysfunction led to the suspicion of rickettsiosis, which was an endemic infectious disease in the area. [ncbi.nlm.nih.gov]
Patient 3 In March 2003, a 44-year-old woman from a southern Adelaide suburb ( Box 1 ) presented after 8 days of lethargy, fever, rigors, headache, arthralgia and muscle pains; 4 days after the initial symptoms appeared, a maculopapular rash appeared [mja.com.au]

Erythema migrans or rash was observed at all combinations of seroreactivity, with symptoms including fever, muscle pain, headache and respiratory problems. [diva-portal.org]
Finally, in southern Sweden, 10 % patients with erythema migrans (EM) and/or general signs of infection following a tick bite had antibodies against the same rickettsial species (Lindblom et al. 2013 ). [link.springer.com]
- Papule
Within days the lesions progress to become papules (small lumps), petechiae (small red or purple spots due to bleeding into the skin), and ecchymoses (bruises). [dermnetnz.org]
Infection most commonly begins with formation of a papule at the site of the bite where the infection was introduced. This usually becomes necrotic and forms a typical black eschar (scab). [www2.health.vic.gov.au]
Neurologic
- Headache
[…] disorder) Spotted fever rickettsiosis Clinical Information Acute, infectious, sometimes fatal disease caused by rickettsia rickettsii usually transmitted by ixodid ticks; occurs only in north and south america; manifestations include chills, fever, rash, headache [icd9data.com]
The disease frequently begins as a sudden onset of fever and headache and most people visit a healthcare provider during the first few days of symptoms. [health.ri.gov]
The trademark rash of this infection can begin anytime up to 10 days after the fever and headache start, but it most often appears on the third to fifth day. [kidshealth.org]
Initial signs and symptoms of the disease include sudden onset of fever, headache, and muscle pain, followed by development of rash. [publichealth.lacounty.gov]
Most people who get sick with RMSF will have a fever, headache, and rash. RMSF can be deadly if not treated early with the right antibiotic. [cdc.gov]
Treatment
So if clinical suspicion is high, treatment should commence even if the test is negative. Treatment of Rocky Mountain spotted fever Tetracyclines are the preferred treatment for RMSF. [dermnetnz.org]
Treatment of RMSF Rocky Mountain spotted fever in pregnancy The most effective treatment is tetracycline. [babymed.com]
Following prompt antimicrobial treatment, titers generally decline below detectable levels within 8 to 11 months. With relapse, prior immunization, or delayed antibiotic treatment, IgG levels may remain elevated for more than a year postonset. [mayomedicallaboratories.com]
Complications may require prolonged treatment. [kidshealth.org]
Treatment RMSF can be difficult to diagnose because many of the symptoms are non-specific. It is important to tell the physician about any tick bites that have occurred. [floridahealth.gov]
Prognosis
What is the prognosis of Rocky Mountain spotted fever? If diagnosed early and treated promptly, RMSF has a very good prognosis. Most patients will fully recover without any long-term disabilities. [medicinenet.com]
Etiology
The etiologic agent is most related to Rickettsia parkeri, R. africae, and R. sibirica and probably widely distributed from Sao Paulo to Bahia in the Atlantic Forest. [ncbi.nlm.nih.gov]
In the Spotted fever group, R. rickettsii is the etiological agent of Rocky Mountain spotted fever, transmitted by the bite of a tick, whereas R. akari is the etiological agent of rickettsialpox, transmitted by the bite of a mite. [medsci.org]
J Immunol May 1, 1953, 70 (5) 461-472; Summary A toxic substance was demonstrated in yolk sac cultures of ten strains of the spotted fever rickettsia which were selected according to their virulence for guinea pigs, tick-host species, or etiologic relationship [jimmunol.org]
Howard Taylor Ricketts between 1906 and 1909 led to the isolation of the etiologic infective agent for Rocky Mountain spotted fever, Rickettsia rickettsii ( Ricketts 1906 ; Ricketts and Gomez 1908 ). [medlink.com]

Prevention
Prevention Travellers who participate in hiking, camping, or similar outdoor activities in wooded regions of endemic areas should take measures to prevent tick bites. There is no preventive medication or vaccine against rickettsial infections. [iamat.org]
Thus, to prevent diagnostic errors, leptospirosis should be potentially considered with high suspicion in patients with acute undifferentiated fever, even in non-endemic areas of leptospirosis. [ncbi.nlm.nih.gov]
Prevention Reducing exposure to ticks is the best defense against Lyme disease, Rocky Mountain spotted fever, Babesiosis, Ehrliochiosis, and other tickborne infections. [health.ri.gov]
