Presentation

A family of 3 (husband, wife, and their son) presented with symptoms compatible with SFG rickettsiosis. For 2 patients, serum samples presented increased levels of IgM and IgG for SFG Rickettsia. [ncbi.nlm.nih.gov]

Because a delay in empiric antimicrobial therapy for RMSF can be fatal, clinicians need to be familiar with the presentation and management of this illness to avoid a poor outcome (Table 1). [pedsinreview.aappublications.org]

One of the major clues in these new descriptions has been atypical clinical presentations. The unusual feature of the case presented here was the presence of an eschar tache noire which would be most unusual for RMSF. [journals.lww.com]

As the infection progresses, people typically experience confusion and severe weakness—often with a cough, difficulty breathing, and sometimes vomiting. [babymed.com]

Index values from 0.8 to 1.2 were considered equivocal (weak positive), above 1.2 as positive and those below 0.8 were considered negative. For analysis, the weak positive results were included into positive group. [link.springer.com]

  • High Fever

There is a high fever — often 103°-105°F (39°-40°C) — with chills, muscle aches, and a severe headache. Eyes can become red, muscles may feel tender, and there may be abdominal pain, nausea, vomiting, poor appetite, and fatigue. [kidshealth.org]

Patient 2 In December 2002, a 74-year-old man from the south coast of SA ( Box 1 ) was referred after 5 days of high fever, rigors, malaise, and severe prostration with mild confusion. [mja.com.au]

including the skin, lungs, CNS, and renal systems. • Early symptoms are nonspecific, but Rocky Mountain spotted fever should be suspected in patients with high fever, headache, and a macular rash with a possible tick exposure. • Neurologic symptoms [medlink.com]

What are Rocky Mountain spotted fever symptoms and signs? RMSF typically begins with headaches, a high fever, and muscle aches. Some individuals may also develop abdominal pain, nausea, and vomiting. [medicinenet.com]

(Image via CDC.gov) Long-term effects: Complications for patients with untreated or improperly treated infections include brain damage, clotting problems, heart failure, kidney failure, lung failure, meningitis, pneumonitis (lung inflammation), and shock [thetickthatbitme.com]

Heart and lungs : Inflammation of heart tissue ( myocarditis ) or respiratory failure can lead to death. Eyes: Damage to the eyes may occur, often due to inflammation of the vascular structures of the eye ( uveitis or retinal vasculitis). [medicinenet.com]

Bacterial replication in host cells causes endothelial cell proliferation and inflammation, resulting in mononuclear cell infiltration into blood vessels and subsequent red blood cell leakage into surrounding tissues. [en.wikipedia.org]

Respiratoric

  • Cough

A total of four cases were from rural areas of Uttar Pradesh, referred with history of high grade fever with chills, rigor and irritability, headache, myalgia, vomiting followed by cold and cough, abrupt onset of generalised erythematous macular/maculopapular [omicsonline.org]

Initial symptoms include fever accompanied by headache, muscle pain, and sometimes nausea, vomiting, diarrhea, and cough. A red rash can develop and black tissue, also known as an 'eschar', forms at the site of the tick bite. [iamat.org]

As the infection progresses, people typically experience confusion and severe weakness—often with a cough, difficulty breathing, and sometimes vomiting. [babymed.com]

On the night of admission he awoke with dyspnoea, but there was no cough or sputum. There was no history of tick bite or recent direct animal exposure. On examination, his temperature was 38.4 ° C and there were bilateral basal chest crackles. [mja.com.au]

Skin

  • Eruptions

The phrase apparently originated in Spain in the 17th century and was ‘loosely applied in England to typhus or any fever involving petechial eruptions .’ [en.wikipedia.org]

The rash is a diffuse macular (featuring elevated discoloration) rash, is most prominent on wrists and ankles, which will usually erupt three or four days after the onset of fever. [web.uconn.edu]

  • Petechiae

Areas of petechiae may result in tiny scars. In rare cases, severe necrosis and gangrene may require amputation. [dermnetnz.org]

In about 35%-60% of patients with RMSF, a spotty rash that is red or purple (petechiae) may develop around the sixth day or later after the onset of symptoms. The appearance of this type of rash indicates that the disease has become more severe. [medicinenet.com]

Treatment

So if clinical suspicion is high, treatment should commence even if the test is negative. Treatment of Rocky Mountain spotted fever Tetracyclines are the preferred treatment for RMSF. [dermnetnz.org]

Treatment of RMSF Rocky Mountain spotted fever in pregnancy The most effective treatment is tetracycline. [babymed.com]

Following prompt antimicrobial treatment, titers generally decline below detectable levels within 8 to 11 months. With relapse, prior immunization, or delayed antibiotic treatment, IgG levels may remain elevated for more than a year postonset. [mayomedicallaboratories.com]

Complications may require prolonged treatment. [kidshealth.org]

Treatment RMSF can be difficult to diagnose because many of the symptoms are non-specific. It is important to tell the physician about any tick bites that have occurred. [floridahealth.gov]

Prognosis

What is the prognosis of Rocky Mountain spotted fever? If diagnosed early and treated promptly, RMSF has a very good prognosis. Most patients will fully recover without any long-term disabilities. [medicinenet.com]

Etiology

The etiologic agent is most related to Rickettsia parkeri, R. africae, and R. sibirica and probably widely distributed from Sao Paulo to Bahia in the Atlantic Forest. [ncbi.nlm.nih.gov]

In the Spotted fever group, R. rickettsii is the etiological agent of Rocky Mountain spotted fever, transmitted by the bite of a tick, whereas R. akari is the etiological agent of rickettsialpox, transmitted by the bite of a mite. [medsci.org]

J Immunol May 1, 1953, 70 (5) 461-472; Summary A toxic substance was demonstrated in yolk sac cultures of ten strains of the spotted fever rickettsia which were selected according to their virulence for guinea pigs, tick-host species, or etiologic relationship [jimmunol.org]

Howard Taylor Ricketts between 1906 and 1909 led to the isolation of the etiologic infective agent for Rocky Mountain spotted fever, Rickettsia rickettsii ( Ricketts 1906 ; Ricketts and Gomez 1908 ). [medlink.com]

Prevention

Prevention Travellers who participate in hiking, camping, or similar outdoor activities in wooded regions of endemic areas should take measures to prevent tick bites. There is no preventive medication or vaccine against rickettsial infections. [iamat.org]

Thus, to prevent diagnostic errors, leptospirosis should be potentially considered with high suspicion in patients with acute undifferentiated fever, even in non-endemic areas of leptospirosis. [ncbi.nlm.nih.gov]
