SIFD Syndrome
Congenital Sideroblastic Anemia - B-Cell Immunodeficiency - Periodic Fever - Developmental Delay

Presentation

The disease presents in infancy with recurrent febrile illnesses, gastrointestinal disturbances, developmental delay, seizures, ataxia and sensorineural deafness. [orpha.net]

Both bone marrow core biopsies were normocellular with morphologically abnormal megakaryocytes present in normal numbers and normal distribution. Cytogenetic studies revealed normal male karyotypes. [imagebank.hematology.org]

The presentation is variable but is typically in early childhood with SA or with fever. We describe two siblings where the presentation was with severe anaemia, marked leucocytosis and hugely elevated numbers of circulating nucleated red cells. [jcp.bmj.com]

The present observation of considerably high numbers of DN T cells has not been previously reported in SIFD patients to date, thus expanding the immunological features of SIFD. [frontiersin.org]

Entire Body System

  • Sepsis

Treatment and Outcome At the time of this systematic review, 19 patients were reported to have died at a median age of 28 months (range 0–168 months): 16 deaths were directly related to the underlying disease course (9 due to multiorgan failure during sepsis-like [link.springer.com]

Skin

  • Brittle Hair

Cutaneous Manifestations Skin abnormalities were reported in 29/58 SIFD population (49%) mainly brittle hair or ichthyotic skin, panniculitis, erythema nodosum, morfea and aspecific rash [1,2,3,4,5, 7, 9, 11,12,13,14, 17,18,19, 21,22,23,24,25, 27,28,29 [link.springer.com]

Case Presentation: We present here a 15-month old male, demonstrated brittle hair, growth hormone deficiency, recurrent fever, arthritis, recurrent infections, mild anemia, and hypogammaglobulinemia. [liebertpub.com]

Neurologic

  • Neurologic Manifestation

    Neurologic Features Neurological manifestations are reported in 48/58 (82.75%) patients with a widely variable phenotypic spectrum [1,2,3,4,5, 7,8,9,10,11,12, 14,15,16,17,18, 21,22,23, 26, 28] (Table 1). [link.springer.com]

Workup

A diagnostic workup was conducted due to persisting fever and headache > 72 h following vaccination, and he was diagnosed with Fiebig stage II acute HIV infection, 3 weeks after having unprotected anal intercourse with another man. [aidsrestherapy.biomedcentral.com]

Other Test Results

  • Schistocytosis

Peripheral blood smears typically showed hypochromasia, microcytosis, target cell, variable schistocytosis, anisocytosis, elliptocytes, basophilic stippling and nucleate erythrocytes (1–4, 6). [link.springer.com]

Treatment

Treatment Treatment Options: Allogeneic bone marrow transplantation in one patient reversed the hematologic and immunologic anomalies although retinitis subsequently developed. [disorders.eyes.arizona.edu]

The material is in no way intended to replace professional medical care by a qualified specialist and should not be used as a basis for diagnosis or treatment. [orpha.net]

Avoid activities that cause force or rubbing on the hands or feet during the first 6 weeks of treatment. This includes jogging, aerobics, and racquet sports. [cancer.net]

Furthermore, the increased knowledge about its pathophysiology is opening the route to new possible treatments. [link.springer.com]

In 4/4 patients, treatment with a TNF inhibitor suppressed inflammation, reduced the need for blood transfusions and improved growth. [zora.uzh.ch]

Prognosis

Prognosis is universally poor, with significant morbidity in survivors.5 Recognised aetiologies include congenital malformations, chromosomal abnormalities (eg, Turner’s syndrome), intrauterine infection (eg, cytomegalovirus), rhesus disease, severe anaemia [jcp.bmj.com]

Prognosis is poor and treatment is supportive, thus the development of new models for the study of Pearson syndrome and new therapy strategies is essential. [unboundmedicine.com]

Early diagnosis and initiation of antiretroviral therapy is associated with better prognosis and limits transmission of the disease. [aidsrestherapy.biomedcentral.com]

Etiology

The molecular etiology is under investigation. Figures Figure 1 Pedigree for cases 1 and 2 with descriptions of phenotypes displayed. [pubmed.ncbi.nlm.nih.gov]

Pathophysiology

Furthermore, the increased knowledge about its pathophysiology is opening the route to new possible treatments. [link.springer.com]

The underlying pathophysiology is complex and remains poorly understood, but includes impaired lymphatic function, difficulty maintaining venous return, osmotic pressure and arterial pressures, and inadequate systemic perfusion leading to hypoxic injury [jcp.bmj.com]

Conclusions Our data provides further insight into cellular pathophysiology of SIFD. Background The tRNA nucleotidyl transferase (TRNT1) gene encodes a protein involved in the maturation of all cytosolic and mitochondrial tRNAs. [ojrd.biomedcentral.com]

Prevention

Ice packs under the hands and feet while chemotherapy is being given to prevent hand-foot syndrome from paclitaxel, docetaxel, or doxorubicin. [cancer.net]

Molecular diagnosis in early life will be crucial for initiating anti-TNF therapy, which might prevent some of the severe disease consequences. [zora.uzh.ch]

Furthermore, overlooking the diagnosis in the stage of primary HIV infection misses the opportunity to prevent HIV transmission to others. [aidsrestherapy.biomedcentral.com]

Therefore, this protein has a crucial role in the protein maturation, in the mitochondrial electron transport chain and in the prevention of intracellular oxidative stress. [link.springer.com]

During treatment, Mouth Sinai recommends patients wear an eye patch to prevent any injury to the eye. (France, CNN, 6/10; Wenger, People, 6/10; Bisset, Washington Post, 6/11; Garner, People, 6/10; Scribner, Axios, 6/10; Sparks, New York Post, 6/10) [advisory.com]
