Presentation

The therapy is guided by the patient’s clinical presentation and hemodynamic status. [ncbi.nlm.nih.gov]

Entire Body System

  • Severe Pain

The main symptoms are chest pain and shortness of breath. [health.harvard.edu]

Furthermore, she received a quite large dose of fentanyl (16 μg/kg), thus, it seems unlikely that she suffered from severe pain during surgery. [ncbi.nlm.nih.gov]

  • Surgical Procedure

In the majority of cases described to date, triggering conditions that precede onset were described, consisting of exposure to internal (emotional) and external stresses (trauma, surgical procedure, exacerbation of a pre-existing condition). [doi.org]

Respiratoric

  • Dyspnea

A 101 years-old woman was admitted to our Emergency Department (ED) for acute dyspnea which onset nearly two hours before presentation. She had been on hydrochlorothyazyde-amiloride therapy due to mild hypertension. [ncbi.nlm.nih.gov]

Cardiovascular

  • Chest Pain

Our patient had typical substernal chest pain induced with exercise and was relieved by sublingual nitroglycerin in the Emergency Department. The pain started after 2 h of intensive Zumba workout. [ncbi.nlm.nih.gov]

  • Tachycardia

The day after his SS symptoms disappeared, the patient's blood pressure decreased, and he developed tachycardia. [ncbi.nlm.nih.gov]

Her tachycardia was diagnosed as the short-coupled variant of idiopathic VF, and an implantable cardioverter defibrillator was implanted. [doi.org]

  • Retrosternal Chest Pain

A 72-year-old woman with asthma attack and retrosternal chest pain was presented to our emergency unit. She had used albuterol spray ten-times before admission due to accentuated dyspnea. [ncbi.nlm.nih.gov]

  • Vascular Disease

diseases Rheumatic fever, rheumatoid arthritis, systemic lupus erythematosus, dermatomyositis, Kawasaki disease Hematological diseases Thalassemia, sickle cell disease, iron deficiency anemia Coronary artery diseases Anomalous left coronary artery from [emedicine.medscape.com]

Aspirin can be discontinued unless coronary disease or peripheral vascular disease is concomitant. β-Blockers may be continued long-term to protect against catecholamine sensitivity, which may predispose to this syndrome. [doi.org]

Musculoskeletal

  • Fracture

A 79-year-old woman was admitted with a left femoral neck fracture and she immediately developed circulatory shock. [ncbi.nlm.nih.gov]

The patient had no thoracic pain but was receiving an infusion of tramadol for pain caused by her rib fractures. She complained only of increasing dyspnoea. An urgent coronary angiography was performed. [doi.org]

Neurologic

  • Confusion

We noticed intermittent confusion and agitation. MRI of the brain showed areas of deep white matter ischaemia as well as microhaemorrhages, suggesting posterior reversible leucoencephalopathy syndrome. [ncbi.nlm.nih.gov]

Whenever a new and previously unrecognized medical condition enters our consciousness, considerable confusion and uncertainly can ensue. [circ.ahajournals.org]

The ECG findings often are confused with those found during an acute anterior wall myocardial infarction.[34][35] It classically mimics ST-segment elevation myocardial infarction, and is characterised by acute onset of transient ventricular apical wall [en.wikipedia.org]

Workup

Workup raised suspicion for non-accidental trauma. Despite progression to brain death, the severely decreased ventricular function and apical akinesis of the left ventricle improved within 40 hours of admission. [ncbi.nlm.nih.gov]

(See Presentation and Workup .) [3] Electrocardiogram of a patient with takotsubo cardiomyopathy demonstrating ST-segment elevation in anterior and inferior leads. [emedicine.medscape.com]

QT, RR, ST Intervals

  • ST Elevation

Despite this ST elevation persisted; however the coronary angiogram was negative for obstructive coronary artery disease. [ncbi.nlm.nih.gov]

Clinically it is characterised by the acute onset of chest pain and is associated with ECG changes such as ST elevation, ST depression or deep T wave inversion which mimic an acute coronary syndrome. [doi.org]

Treatment

She was discharged with medical therapy including a beta-blocker, which would not be routinely employed in the treatment of a patient with TCM. [ncbi.nlm.nih.gov]

Prognosis

Prognosis of takotsubo cardiomyopathy is generally favorable, but serious complications can occur, especially in the early stage. In this review, we summarize the current knowledge on the clinical management of takotsubo cardiomyopathy. [ncbi.nlm.nih.gov]

Etiology

Data on longterm follow-up is pending and further studies will be necessary to clarify the etiology and reach consensus in acute and longterm management of TLVABS. [ncbi.nlm.nih.gov]

Epidemiology The real incidence of Takotsubo cardiomyopathy is uncertain. [ncbi.nlm.nih.gov]

Pathophysiology

The pathophysiology of the disorder remains to be elucidated but may involve catecholamine excess and vasospasm. [ncbi.nlm.nih.gov]

Prevention

Excess catecholamines have been postulated as a contributing mechanism to the disease, therefore, catecholamines were avoided to prevent further myocardial injury. [ncbi.nlm.nih.gov]

Although there's little evidence on long-term therapy, beta blockers (or combined alpha and beta blockers) may be continued indefinitely to help prevent recurrence by reducing the effects of adrenaline and other stress hormones. [health.harvard.edu]
